Y of Greater Indianapolis successfully translates group based DPP at lower cost Greater Louisville validates in nonresearch environment. CHAPTER Scaling begins through national partnership with CDC and the Diabetes Prevention and Control Alliance. First time payors are paying for prevention delivered in a group setting. CHAPTER Scaling and Dissemination Plan complete and implementation begins. Program performance standards must be met. 4 HEALTHY LIVING SIGNATURE PROGRAM: YMCA S DIABETES PREVENTION PROGRAM Who? Overweight* Adults (18+) with prediabetes Confirmed via one of 3 blood tests or previous diagnosis of gestational diabetes Or a qualifying risk score What? 12 month program: includes a 16 session weekly sessions followed by monthly sessions 1 hour sessions 8-15 people in group based, classroom setting When? Where? Anytime, anywhere (classroom-type setting) How? Weigh-in at every session Weight recorded in online tracking system Program goals: 7% weight loss and 150 minutes of physical activity Facilitated by Y Lifestyle Coach (person skilled in motivational interviewing and group facilitation) 4 YMCA S DIABETES PREVENTION PROGRAM 2013 YMCA of the USA

10 THE OPPORTUNITY Increase diabetes/prediabetes screening by health care providers through changes to health policy and medical training Increase the number of payers (both private and public) for prevention programs like the YMCA s Diabetes Prevention Program If you build it Continue efforts in states and communities to build more opportunities for health eating and activity where people live, work learn and play 10 HEALTHY LIVING AT THE Y 2012 YMCA of the USA

11 PARTICIPANT HIGHLIGHT MEET LUPE At age 67, Lupe says, I had my blood sugar tested and found out I was at risk for type 2 diabetes. I couldn t afford the program, but I was able to get a partial scholarship from the YMCA of Central Florida in Orlando, which really helped. Lupe wasn t sure what to expect and says, I thought they were going to teach me what to eat to prevent diabetes. Instead she felt empowered to make her own choices and small changes over time. Through her increasing self-awareness, Lupe realized she overate when stressed or bored. She says, I still get those feelings, but I choose low fat options, or recognize it s something in the environment causing me to eat (rather than hunger) and it s just a habit I can change. I realized I do have self-control. I am getting back to who I used to be. Lupe s final thoughts: One thing I see, and especially in older people, is that when it comes to health care we have to be more educated and take control. Programs like the YMCA s Diabetes Prevention Program teach you about a disease and give you tools to help prevent it. And, when it is community based, you feel less intimidated and are more willing to share. The program has been awesome!
