1 Major Haemorrhage Transfusion Pathway SENIOR CLINICIAN ASSESSMENT: DECLARES MAJOR HAEMORRHAGE ( Call for help ( Telephone via switchboard: Consultant or Senior Clinician Duty Anaesthetist Porters (if will be needed for sample delivery / blood collection) State there is a major haemorrhage. State: Location of the patient. Name of Doctor in charge. Contact telephone number and name for co-ordination. RESUSCITATE Airway Breathing Circulation ( Call Transfusion ( CRH HRI State there is a major haemorrhage. State: Patient information: Name, ID number, date of birth, sex, location. Whether AAA case or not. Name of Doctor in charge. Contact telephone number and name for co-ordination. Whether O neg or Group specific blood required. Take blood & hand deliver to lab URGENTLY: Crossmatch, FBC, PT, APPT, fibrinogen, U&E, ABG Senior clinician to decide type of blood according to urgency: O negative only if immediate use required Group specific available in 15 mins of lab receiving crossmatch sample Fully crossmatched within 45 mins ACHIEVE HAEMOSTASIS Request: Major Haemorrhage Pack 1 8 red cells 4 FFP 1Plts SUPPORTIVE CARE Haemorrhage Control Direct pressure/tourniquet if appropriate Stabilise fractures Surgical intervention Interventional radiology Endoscopic techniques Obstetric techniques Haemostatic Agents If a trauma case : give Tranexamic acid 1g bolus followed by 1g over 8 hours. Vit K and Prothombin Complex Concentrate for warfarin reversal. Other haemostatic agents eg. Recombinant VIIa: Discuss with Duty Haematologist STAND DOWN Inform Lab. Return unused blood products. Inform lab of O neg use so stocks are replenished. Complete documentation. Administer Blood will arrive first in 2 boxes each containing 4 units. Give FFP & Plts alongside blood as soon as they arrive (>45mins) Reassess Take blood: FBC, PT, APPT, fibrinogen, ABG, U&E. Inform duty Haematologist. If bleeding continues: Request: Major Haemorrhage Pack 2 8 red cells* 4 FFP 1Plts 2 packs cryoprecipitate * 4 red cells in AAA case Administer Reassess FBC, PT, APPT, fibrinogen, ABG, U&E to inform further blood component requesting. Discuss with duty haematologist. Continuous cardiac monitoring Prevent Hypothermia Use fluid warming device. Use forced air warming blanket Monitor for hypocalcaemia Consider 10mls Calcium Chloride 10% over 10 min Aims for therapy Hb 8-10 g/dl Platelets >75 x 10 9 /l PT ratio < 1.5 APPT ratio < 1.5 Fibrinogen >1 mmol/l >2 mmol/l** Ionised Ca 2+ >1 mmol/l (on ABG) Temp > 36 o C ph > 7.35 Monitor for hyperkalaemia ** in obstetric cases

Request Major Haemorrhage Pack 1 from Transfusion lab. CRH HRI Arrange collection of blood products. While pack 1 being given, call for FBC, clotting results. Request Major Haemorrhage Pack 2 if clinically indicated. After pack 1 administered send repeat bloods. Continue to monitor FBC, clotting, U&E, ABG. Document results and order blood products. Call consultant haematologist if advice needed. Inform Transfusion of stand down and whether O neg stocks need to be replaced. Return unused products. Ensure ALL blood components fated & documented. Time FBC results Time Clotting results Time U&E, ABG results
