1 DERBYSHIRE JOINT AREA PRESCRIBING COMMITTEE (JAPC) JAPC Briefing FreeStyle Libre JAPC has classified Freestyle Libre as BROWN after diabetes consultant/specialist initiation within a Derbyshire diabetes service*. The criteria and process below must be strictly adhered to. Identification Consultant identifies those with Type 1 diabetes aged four and above on intensive insulin therapy meeting RMOC criteria: Patients who undertake intensive blood glucose monitoring 8 times daily (confirm with Diasend download) Those who meet the current NICE criteria for insulin pump therapy (HbA1c 8.5%, 69.4mmol/mol) or disabling hypoglycaemia as described in NICE TA151) where a successful trial of FreeStyle Libre may avoid the need for pump therapy (has attended structured education and is monitoring glucose 4/day) Those who have recently developed impaired awareness of hypoglycaemia (Gold score 4) Frequent admissions (>2 per year) with DKA or hypoglycaemia. Those who require third parties to carry out monitoring and where conventional blood testing is not possible Baseline clinical data Complete baseline FreeStyle Libre Audit form ( documenting reason for initiation. Ensure recent HbA1c available Consultant referral for Freestyle Libre initiation Freestyle Libre Education Session Contract for Freestyle Libre use completed Person with diabetes attends FreeStyle Libre education session Provided with FreeStyle Libre starter kit of reader and 1 sensor (a sensor lasts 14 days) Ensure baseline data collected and HbA1c available from within past 2 months GP prescription request FreeStyle Libre proforma completed to request GP prescription only if baseline HbA1c available, audit data captured, reason for initiation clearly documented and person with diabetes attends education session. If any of above missing, delay initiation until complete. Follow up data collection Consultant responsible for the patient must ensure follow up audit data is captured within 6 months of initiation. Consultant must document clinical benefit (e.g. fall in HbA1c by 0.5% or a reduction in hypoglycaemia or a reduced frequency of admissions or reversal of hypoglycaemia unawareness or improved quality of life); stop prescription if no benefit or DNA 1 year audit At 3 monthls JAPC monitors uptake 6-12 months diabetes service review audit data with JAPC * Permitted for NHS patients started outside of Derbyshire that comply with RMOC criteria and undertaking the national audit or following an APC shared care agreement Freestyle Libre JAPC briefing Page 1 of 7
2 Existing patients Patients currently self-funding FreeStyle Libre will only be prescribed the device on the NHS if the above criteria (same as new patients) have been met.
