2 Introduction The putting on and taking off of therapeutic elastic stockings, tights and arm stockings can be a difficult task that needs to be repeated every day. To make this easier to do and less laborious, Arion has developed various application and removal aids. This range of products is called The Slide Solution. Problem definition In Dutch legislation, the putting on and taking off of elastic stockings is identified as the third main source of physical strain. The practical guideline given in this respect is to use an application and removal aid. The risk of physical strain when putting on and taking off elastic stockings arises if a person has the wrong work posture (resulting in a risk of back and shoulder complaints) and if too much force is applied when pulling and pushing (resulting in a risk of hand, wrist and shoulder complaints). This applies both for caregivers and for clients who put the elastic stockings on themselves. Other risks are damage to the skin of the client as a result of shearing and compressive forces and a reduction in the quality of the elastic stocking due to tensile forces (the stocking stretches or ladders). The solution Our application and removal aids have been developed specifically to resolve the problems outlined above for elastic stockings and tights with a closed or open toe as well as for elastic stockings up to the knee or groin, regardless of the pressure class. The Slide Solution products are made from very smooth material that has been treated with a special coating. The very high friction reduction that is achieved as a result ensures that physical strain is kept to a minimum for caregivers and clients alike. Objectives Combined with the right work posture and methods, the three application aids ensure that the following objectives are achieved: -- no strain for the client (on skin or joints); -- the even distribution of pressure from the elastic stocking over the leg and, as such, an optimal effect; -- the elastic stocking maintains its quality; -- no physical strain for the caregiver. 2

3 Application aids The following three aids will be necessary when putting on an elastic stocking: 1) The Easy-Slide, Sim-Slide (both for a stocking with an open toe) or Magnide (for stockings with a closed toe) that make it easy to slide the elastic stocking over the heel. 2) The SlideX, over which the stocking is slid higher up the leg: up to the knee or the groin. If a caregiver is putting on elastic stockings for a client, this will be made even easier for him if the client pulls on the SlideX at a certain point and, by doing this, helps the caregiver to slide the last part of the elastic stocking upwards. The SlideX makes it possible to slide the elastic stocking over the full length of the lower leg (knee stocking) or upper leg (groin stocking) over a smooth dual layer, so that no pressure load or stretching strain is needed at any point. 3) Gloves with a rough side, giving you a good grip on the elastic stocking and making it possible to push the stocking upwards along the leg rather than the stocking being pushed into the leg. 4 Tips and tricks for the caregiver Work posture: - Preferably, the caregiver should be seated opposite the client when putting on an elastic stocking and the client should place his heel on the knee on the caregiver. The lower the client is sitting in the chair, the easier it will be to raise his leg. The correct starting position is crucial as it ensures that the elastic stocking is put on in a way that is physically safe and comfortable. - Work close to your own body and avoid stretching. - Keep your back straight by facing the bed, whether sitting on a chair or standing next to the bed, making sure that the bed has been raised to elbow height. A rolled up towel: - Chair: a rolled up towel placed under the upper leg of the client ensures that the knee does not sag and reduces the pressure of the heel on the knee of the caregiver. - Bed: if the client who is lying on the bed is not able to lift his leg himself, place a tightly rolled up towel under both his upper and lower legs. By doing this, the heel is lifted away from the bed and as soon as the 'calf roll' has been moved to the heel the same will apply for the entire lower leg, making it easy to slide the elastic stocking upwards. 4

5 SlideX - The SlideX reduces the frictional resistance from the ankle up to the knee or groin, with the result that: it becomes very easy for the client to put on the elastic stocking by himself or for the caregiver to push the elastic stocking up the leg of the client; it becomes possible to distribute the pressure from the elastic stocking evenly over the leg, which is important for the stocking to work properly; the elastic stocking can be put on properly simply by pushing it up carefully; 'pulling on the ribbing', which places strain on the elastic stocking and on the hand and shoulder joints of the caregiver, is not necessary. - The client can help the caregiver to push up the stocking by pulling on the loop of the SlideX as soon as the ribbing of the elastic stocking is on the SlideX. Gloves: - The special gloves have a rough side that gives the caregiver the grip he needs. It is not necessary to place any extra pressure on the elastic stocking; this would only make the pushing upwards movement more difficult. Keep the tension in the arms and legs low. - When pushing up the elastic stocking, work more with the full hand; do not put any pressure on the finger tips. When pushing the stocking upwards, the hands will not drop under the leg, but will continue to be oriented towards the knee. This will avoid any strain on the wrist joints. 5

6 Method: (1) Putting the elastic knee stocking on when the client is sitting down Aids: - The Easy-Slide or Sim-Slide - The SlideX - Gloves for elastic stockings - A rolled up towel Preparation:: - The client is sitting on a chair (preferably an easy chair), places a rolled up towel under his upper leg and places his heel on the knee of the caregiver. - The caregiver is sitting on a chair or stool and has an active posture. 6

7 Method for putting on an elastic stocking - Place the Easy-Slide, Sim-Slide or Magnide over the foot. - Put on the gloves. - When using the SlideX, click the magnets together and place the SlideX on the front of the leg lengthwise as follows: the long side underneath (over the upper and lower leg) and the short side on the top (over the lower leg); wrap the lowest edge around the ankle so that the SlideX fits into the Easy- Slide, Sim-Slide or the Magnide; the loop is positioned above the knee. - Hold the elastic stocking more on the back of the ribbing and (without stretching it) move the stocking over the foot until it is positioned above the ankle. 7

8 - Push the elastic stocking upwards from the foot and around the heel; the hands will follow the line of the leg. - Avoid any gathering up of the elastic stocking around the ankle: use your hands to push the ribbing in the direction of the knee. - Keep pushing the stocking upwards from the foot and around the heel until the heel of the elastic stocking is positioned correctly. - Check that the stocking has been put on properly; correct any twists. - The caregiver will move his own chair slightly closer so that any stretching is avoided. - Slide the stocking more towards the direction of the knee: Place your hands on the side/back of the lower leg and push the elastic stocking up to about 2 cm under the lower part of the kneecap. Help from the client: as soon as the ribbing of the elastic stocking is resting on the SlideX, the caregiver will keep his hands on the elastic stocking while the client will help to pull the stocking up more by pulling the loop of the SlideX towards him. A slight push by the caregiver at the same time will be enough to ensure that the stocking is applied properly. If the stocking is put on properly, the pressure produced by the stocking will be distributed evenly over the entire lower leg. 8

9 - Removing the Easy-Slide or Sim-Slide:u Grip the loop of the Easy- Slide with one hand, placing the other (gloved) hand around the heel of the client and, by doing this, keep the elastic stocking in place; Keeping the upper arm of the arm that you are using to pull the stocking on close to your body, lean back a little and wait for the Easy-Slide to start moving; Grip the Easy-Slide again, but this time closer to the elastic stocking and repeat this step again; If you keep your movements steady, the elastic stocking will remain in place. This will also ensure that the Easy-Slide is not damaged. 9

10 Points of attention when pushing up an elastic stocking: - While pushing the stocking upwards, the hands will follow the shape of the leg. In other words (in the order indicated below): via the side of the foot to the heel; with open hands around the curve of the heel; using the hands to cover the side/back of the lower leg more, towards the knee; the wrists will remain in a neutral position (with the finger tips pointing towards the knee). 10

11 - The curves of the heel and the calf make the back of the leg the long side: by placing the hands here in particular, the elastic stocking will naturally move to the front (the short side) and it will not be necessary to touch the shinbone (which is often sensitive). - Do not exert any force in the hands or from the shoulder; the stocking will be pushed upwards using a careful 'leaning' movement from the trunk. - With gloves on, push the elastic stocking all the way to the top. Finally, the client will help by pulling the SlideX towards himself. If the client were to use his hands to pull up the elastic stocking at this stage, this would cause strain on the hands of the client, stretching of the ribbing and an uneven distribution of pressure from the stocking over the lower leg. - Putting on a sock, knee stocking or tights over an elastic stocking with an open toe or after ACT (Ambulant Compression Therapy): place the SlideX over the lower leg and around the foot. Doing this will avoid the edge of the elastic stocking rolling up at the toes or at the edges of the bandage. - The SlideX Large can be used for clients with larger legs. 11

12 Method: (2) Putting on an elastic groin stocking when the client is sitting down The preparation and method are the same as those described for the knee stocking. A slightly bigger SlideX (size Large) will ensure that the entire upper leg is enclosed in an extra smooth layer, which will also make it easier to push the stocking upwards from the knee. Points of attention: - Push enough of the support stocking upwards from the ankle to make it possible to position the rest of the stocking comfortably in the upper area of the upper leg. - As soon as the stocking is over the knee, ask the client to place his foot against your upper leg, remove the rolled up towel and push the stocking upwards more; the client will help by pulling on the SlideX. - The client will need to stand for the last part of the upper leg. The method and work posture are as follows: the client will put weight on the leg over which the support stocking is being pulled; 12

13 now adopt a 'squatting position', with the leg of the client between your legs; place your hands on the upper leg and, while the client pulls the SlideX towards himself, slowly stretch out your legs and use the gloves to pull the stocking upwards. 13

14 Method: (3) Putting on an elastic knee stocking when the client is lying down Aids: - The Easy-slide / Sim-Slide / Magnide - The SlideX - Gloves for elastic stockings - Two rolled up towels Work posture: - Work close to your own body; avoid stretching. - Keep your back straight; place the high-low bed at a level that enables you to work at a b o u t elbow height. - Work in a standing position facing the bed, or do the first part of the stocking from the foot of the bed. If you decide to work from the foot of the bed, raise the level of the foot support from the bed and lower the whole bed a little. It is sometimes possible to remove the footrest. 14

15 Preparation: - Place all aids within hand's reach. - Place the bed in the appropriate position and at the right height. - Working from the long side of the bed: ask the client to angle his leg towards you slightly more. - Place one rolled up towel under the upper leg of the client and the other under his calf. The entire foot is now free of the bed (later the same will apply for the whole lower leg, too). The rolled up towels are not necessary if the client is able to lift his leg himself. Method for putting on an elastic stocking - The method here is the same as the one that is used when the client is sitting down. Push the elastic stocking upwards from the foot and around3 the heel; your hands will follow the line of the leg. Avoid any accumulation of the elastic stocking around the ankle; push the ribbing of the stocking slightly more towards the knee. - As soon as the heel of the elastic stocking is positioned correctly, move the rolled up towel from the calf to the heel, after which the lower leg will be free. Place your hand on the side/back of the lower leg and push the elastic stocking upwards until the ribbing of the stocking is resting on the SlideX. 15

16 - Ask the client to pull up the stocking more by pulling the loop of the SlideX towards himself. A slight push from the caregiver at the same time will be enough to ensure that the stocking is applied correctly. - As soon as the elastic stocking is about 2 cm under the underside of the kneecap, the client will be able to extend the SlideX more. Move to the other side of the bed for the other leg. 16

17 Method: (4) Taking off an elastic knee stocking when the client is sitting down Preparation: - The client is sitting on a chair (preferably an easy chair), places a rolled up towel under his upper leg and places his heel on the knee of the caregiver. - The caregiver is sitting on a chair or stool and has an active posture. - Put on the gloves. Method to use when taking off an elastic stocking: - Place the Easy-Off or the Sim-Slide over the foot. - Easy-Off: secure the Velcro above the ankle (making sure that it is not too tight) and fold back the material (left) over the Velcro. - Sim-Slide: using the elastic band, pull the Sim-Slide until it has achieved a reasonably tight fit around the ankle. 17

18 - Hold the elastic stocking more on the side and back of the ribbing and use a steady and smooth movement to slide the stocking towards the foot and around the heel (without stretching the stocking), until the point at which too much tensile force is being exerted on the stocking - Place your hands around the heel and possibly the instep too and slide the elastic stocking off of the foot. - Making a round movement, the elastic stocking will slide around the heel without any strain being exerted. 18

19 Method: (5) Taking off an elastic groin stocking when the client is sitting down Preparation: - The client is sitting on a chair (preferably an easy chair). - The caregiver is sitting in front of the client, on a chair or stool. - Place the open SlideX over the leg and place the end of the SlideX (the side that has no loop attached) at the edge of the support stocking and place the ribbing of the support stocking over the SlideX. - Make sure that the SlideX remains positioned high in the edge of the turned over stocking at all times. Method to use when taking off an elastic stocking: - As soon as the stocking has been turned over the SlideX by about 10 cm, place your hands on both sides of the upper leg, on the turned over part of the support stocking, and steadily peel back the stocking in a downward direction. - As soon as the support stocking has been peeled back to just above the knee, the client will be able to place the rolled up towel under his upper leg and then put his heel on the knee of the caregiver. 19

20 - Slide the elastic stocking steadily downwards over the SlideX. - Once the SlideX is also covering the foot and heel, it will also be easy to pull off the support stocking over the foot. 20

21 Method: (6) Taking off an elastic knee stocking when the client is lying down Work posture: - Work close to your own body; avoid stretching. - Place the high-low bed at a level that enables you to work at about elbow height. - Work in a standing position facing the bed or do the first part from the foot of the bed (if this does not mean stretching too far). If you decide to work from the foot of the bed, raise the level of the foot support from the bed and lower the whole bed a little. - Place one rolled up towel under the upper leg of the client and the other under his calf or heel. The rolled up towels are not necessary if the client is able to lift his leg himself. Method - Place the Easy-Off or the Sim-Slide over the foot. - Easy-Off: secure the Velcro above the ankle (making sure that it is not too tight) and fold back the material (left) over the Velcro. - Sim-Slide: using the elastic band, pull the Sim-Slide until it has achieved a reasonably tight fit around the ankle. - Put on the gloves. 21

23 Development of the SlideX The SlideX has been developed in collaboration with Tinie Hake, the owner of TRANSFERS belicht. This company is a training agency and consultancy that specialises in the reduction of the physical strain experienced by employees in the healthcare industry. As the owner of TRANSFERS belicht, she delivers many of the training courses herself, as a result of which she has gained very extensive experience in various sectors of the healthcare industry. It is not for nothing that her personal statement is: "It is possible to provide any form of care without this resulting in physical strain of any kind." After completing her course at the Academy for Physiotherapy in Groningen, Tinie Hake worked as a physiotherapist until Over the years, she specialised in haptonomy (the Academy for Haptonomy and Kinesionomy in Doorn). After becoming involved in a problem that a care home was experiencing, namely how to lift and move residents, she decided to use her physiotherapy and haptonomy knowledge and skills to deliver courses. After completing the teacher training programme in haptonomic lifting and moving techniques, she launched her training agency TRANSFERS belicht in Tinie Hake is also the author of the book Basisboek verplaatsingstechnieken voor zorgverleners (book about moving techniques for caregivers). The SlideX was developed on the basis of an idea that originates directly from the field in which Tinie Hake operates. The product solves problems that nurses and caregivers experience every day on the work floor when putting on and taking off compression stockings. Tinie Hake worked with Arion to develop the SlideX into a multifunctional and versatile aid that will be welcomed by any caregiver faced with the problem of applying or removing elastic stockings. By focusing particularly on the back of the (lower side of the) heel when pushing up the stocking, it is easy to slide the stocking upwards towards the knee. 23
