1 Evidence-based Interventions (EBI) promoted by the NYS QTAC Center for Excellence in Aging & Community Wellness NYS Quality & Technical Assistance Center University at Albany 135 Western Avenue, Richardson 388 Albany, NY Phone: Fax: www. ceacw.org Chronic Disease (CDSMP)* Adults 18+, or caregivers of adults, with different chronic health conditions The CDSMP workshop is facilitated by two trained Leaders and/or Master Trainers. This workshop teaches the skills needed in the day-to-day management of any type of ongoing health condition in order to maintain and/or increase an active and fulfilling lifestyle. The curriculum teaches selfmanaged behavior modification and coping strategies to enable participants to manage their health condition(s), medications and increase physical activity levels. Over 1,000 people with heart disease, lung disease, stroke or arthritis participated in a randomized, controlled test of the and were followed for up to three years. People who took the, when compared to those who did not, demonstrated significant improvements in exercise, cognitive symptom management, communication with physicians, self-reported general health, health distress, fatigue, disability, and social/role activities limitations. They also spent fewer days in the hospital, and there was also a trend toward fewer outpatient visits and hospitalizations. These data yield a cost to savings ratio of approximately 1:4. 2 Diabetes (DSMP)* Adults 18+ diagnosed with type 2 diabetes The DSMP workshop is facilitated by two trained Leaders and/or Master Trainers. This workshop teaches the skills needed in the day-to-day management of diabetes in order to maintain and/or increase an active and fulfilling lifestyle. The curriculum teaches self-managed behavior modification and coping strategies to enable participants to manage their diabetes, medications and increase physical activity levels. The original Diabetes was developed in Spanish. Results from the Spanish program showed that the program participants, as compared with usual-care control subjects, demonstrated improved health status, health behavior, and self-efficacy, as well as fewer emergency room visits at four months. At one year, the improvements were maintained and remained significantly different from baseline condition. *This workshop is also available in Spanish in some areas. Positive for HIV (PSMP) Adults 18+ affected and/or living with HIV/AIDS 7 weeks Facilitated by two trained Leaders and/or Master Trainers, the PSMP workshop teaches the skills needed in the day-to-day management of HIV in order to maintain and/or increase an active and fulfilling lifestyle. The curriculum teaches self-managed behavior modification and coping strategies to enable participants to manage HIV, medications and increase physical activity levels. Over 200 people with HIV have now participated in a randomized, controlled test of the, and were followed for six months each. s randomized to the PSMP were found to have improvements in their symptoms in the original developmental study. More recently, they have found that HIV patients in PSMP who are using antiretroviral medications have significantly better medication adherence and have better HIV suppression in the blood after 6 months. In spite of this, they have no more side effects than non-psmp participants. Page 2 of 7

3 Chronic Pain (CPSMP) Adults 18+ with a primary or secondary diagnosis of chronic pain Pain is defined as being chronic or long term when it lasts for longer than 3 to 6 months, or beyond the normal healing time of an injury. Facilitated by two trained Leaders and/or Master Trainers, this workshop teaches the skills needed in the day-to-day management of chronic pain in order to maintain and/or increase an active and fulfilling lifestyle. The curriculum teaches self-managed behavior modification and coping strategies to enable participants to manage chronic pain, medications and increase physical activity levels. Like the other s, the CPSMP has also been rigorously evaluated in two randomized clinical trials funded by Health Canada and the Canadian Institutes of Health Research (CIHR). The research studies found that, on average, people who have participated in the CPSMP have more vitality or energy, less pain, less dependence on others, improved mental health, are more involved in everyday activities and are more satisfied with their lives compared to those who have not taken the program. The program has also been delivered and evaluated across 10 pain clinics in Ontario, Canada. Evaluation of the program found it to be beneficial for participants in terms of coping skills, education and overall quality of life. To date, the program has been delivered to hundreds of individuals with chronic pain. Page 3 of 7

4 National Diabetes Prevention in NYS (NDPP in NYS) Adults 18+ who have been diagnosed with pre-diabetes or who are at risk of developing diabetes 1 1 hour Followed by 6-8 months of 1 hour monthly The National Diabetes Prevention in NYS is an evidence-based lifestyle modification program. The NDPP in NYS is an implementation of the Centers for Disease Control and Prevention s National Diabetes Prevention. Trained Lifestyle Coaches facilitate 16 one-hour and help participants set and achieve personal lifestyle goals. Each session covers diet, exercise, and behavior modification to help participants achieve two primary goals: Reduce and maintain individual weight loss by 5-7% Participate in regular physical activity The program also provides a 6 8 month follow-up to help participants maintain lifestyle changes. A national research study demonstrated that participants in the lifestyle intervention group: Prevented or delayed type 2 diabetes by losing a modest amount of weight through diet and physical activity Reduced their risk of developing the disease by an average of 58% Reduced their risk of developing diabetes by an average of 71% if the participant was aged 60+. In addition, participants who attended nine or more classes lost a greater percentage of their body weight as compared to other participants. Selfreported general health status improved by the end of the program and was sustained at the six-month follow-up. A higher proportion of participants reported that their health was very good or excellent at the end of the 16 weeks. More than 70% of participants reported at six months that their general health was much better or somewhat better than before the program. Page 4 of 7

5 Arthritis Foundation Walk With Ease Directed (WWE) Adults 18+ living with arthritis and/or other chronic health conditions who want to become more physically active Directed The Arthritis Foundation Walk With Ease self-directed program is a six-week physical activity and self-management intervention developed for people living with arthritis and/or other chronic health conditions who want to be more physically active. Beginners to physical activity can benefit; the only pre-requisite for participation is the ability of individuals to be on their feet for 10 minutes without increased pain. Individuals use the Walk With Ease guidebook to participate in the program on their own. Participants may also form informal walking groups with other walkers. Participants are encouraged to work up to walking at least three times a week for six weeks. Walk With Ease focuses on motivational strategies including action planning and goal setting, and provides participants with the information, tips and tools they need to develop a safe exercise routine that fits their unique needs and goals. Walk With Ease is evidence-based and has been shown to increase balance, strength, walking pace, self-efficacy and perceived control, as well as reduce pain, fatigue, stiffness and disability for participants. This is a safe, easy and inexpensive program to implement the results in the improved health and well-being of adults with arthritis and other ongoing health conditions. Page 5 of 7

6 Active Living Every Day (ALED) The Cooper Institute Adults 18+ who seek to integrate physical activity into their daily lifestyle 12 weeks 1 hour Active Living Every Day is an evidence-based behavior change program developed by The Cooper Institute, and offered through a partnership with Human Kinetics, that focuses on helping people become and stay physically active for a lifetime. The seeks to help people integrate physical activity into their daily lives so that it becomes a natural part of their lifestyle. The Active Living philosophy is based on three principles: Moderate amounts and intensities of physical activity result in significant health benefits, especially for sedentary people Lifestyle physical activity provides an important alternative for people to fit exercise into their daily lives People are more likely to become and stay physically active when taught lifestyle skills that enhance individual readiness to change Active Living Every Day is based on the findings of Project Active, groundbreaking research conducted at The Cooper Institute. Project Active was a two year study comparing the effects of structured exercise with lifestyle physical activity. The study concluded that this intervention provided comparable increases in physical activity and cardiorespiratory fitness from baseline over 24 months. They also found the lifestyle approach to provide a useful alternative in helping people meet the public health recommendation for physical activity. Page 6 of 7

7 Active Choices (AC) the University Prevention Adults 18+ who would like to be physically active and prefer to exercise on their own or with minimal guidance personal telephone support Active Choices is a personal, telephone-support program designed to encourage regular physical activity. An Activity Coach meets with the participant once for an introductory session and then proceeds with telephone and mail follow-ups to develop a physical activity routine customized to the needs, abilities and goals of each participant. Together, they set goals, track progress, problem-solve difficulties, and discuss other options for lifestyle improvement. When asked in large-scale surveys, approximately 65% of adults aged 40 and older expressed a preference to exercise on their own, in or around their homes, and with minimal guidance and supervision, as opposed to participating in formal exercise classes. While formal structured group exercise classes are helpful for many, they only appeal to about one-third of the population. Active Choices has been shown to be effective in helping people to reach a health promoting level of physical activity that is safe and effective. Page 7 of 7
