3 Welcome to In partnership with the Pasco County Sheriff s Office, we are pleased to offer a FREE & CONFIDENTIAL path to wellness for all employees and their families that are on the Sheriff s medical insurance plan. Some of the benefits and services that you now have access to include: CareHere Health and Wellness Center- Your physicians, ARNP s and PA s will give you on site health care for Acute and Chronic medical problems or concerns. Your providers treat patients age 8 years old and up. Health Risk Assessment - Provides a current evaluation of your overall health through a 28 panel blood draw. Helps identify high cholesterol, diabetes, liver functions, chemistry levels, nutrition, prostate cancer, hypertension and more. CareHere Health Coaches Dietician, Exercise, Mental Health and Weight Management. No Deductible or co-pays for CareHere center visits No co-pays for generic medicines at the center (Only requires establishing with CareHere medical provider) No co-pays for laboratory testing ordered at the center (Outside lab order only requires establishing with CareHere medical provider) No co-pays for X-Rays and some Diagnostic Testing ordered through the CareHere provider. Weightman Middle School Campus) Cobra Way Wells Road (opening in March 2015) Hudson, FL Wesley Chapel, FL Before you will have access to the CareHere centers, you will need to register with CareHere. Each individual must be registered separately in the CareHere system. STEP 1 Logon to and click Member Login. Registration Steps STEP 2 On the next screen click I need to register for the first time with my Access Code. STEP 3 Beside First time registration enter the following Access code for the Pasco County Sheriff s office and click Go. PCSD2 STEP 4 Confirm you have entered the correct Access Code according to your chosen Health Plan and click Next.

5 Final Registration Steps for CareHere STEP 5 Read the Privacy Consent, and if you agree, click I agree with the statements above. STEP 6 Finish your registration by entering the required personal information on the pages that follow and select your Username and Password. After completing the above steps, a confirmation will be sent to the address you provided. You may now log back in with your new USER NAME and PASSWORD to book an appointment or update your Behavioral Health information. The Welcome Screen will appear with the menu of services including appointments on the left hand side in the green area. To schedule or change an appointment online: Click Appointments. The standard clinic hours and days will appear as well as a calendar. Click a valid day on the calendar, and all available appointment slots will appear. Click Make Appointment to schedule an appointment in your preferred time slot. A pop-up screen will appear. Enter your symptoms, reason for appointment or comments. Click Print is you want a printed reminder of the appointment. Click Submit to save your appointment in the system. Remember: you may edit or delete your appointment at any time prior to the appointment time. You will only be able to view details about your own appointment. This information is confidential and may not be viewed by anyone else other than the center team. Make sure you logout when you end your on-line sessions. APPOINTMENT GUIDELINES All patients must be registered in the CareHere system before visiting the clinic. A separate appointment must be scheduled for each patient in advance for all visits to the clinic. All medicine refills require an appointment with the provider. For 3+ medicine refills, please schedule 2 back-to-back appointments with the provider. All HRA s & Blood draws require 8-12 hours of fasting. Fasting means water and black coffee only. We encourage you to drink plenty of water before your blood draw to make the blood draw easier. First-time visits, annual physicals, and pap smears, require 2 back-to-back appointments. If you are 10 minutes or more late for your appointment, please know you may be rescheduled due to time constraints. Your FREE CareHere Health Risk Assessment (HRA) A proprietary, unique and valuable offering Provides a current evaluation of your overall health through a 28 panel blood draw, vitals and health and behavior questionnaire Helps identify high cholesterol, diabetes, liver functions, chemistry levels, nutrition, prostate cancer, hypertension and more At your follow-up appointment for your HRA, your provider will discuss your results with you and prepare a plan of action that may include referring you to one of the CareHere wellness programs or to one of the CareHere Health Coaches available to work with you personally and confidentially The CareHere Employee Health and Wellness Centers are generally open Monday thru Friday, along with some Saturday hours. CareHere Connect is a PERSONALIZED web-based platform that implements employee wellness promotion programs. It provides user-friendly and interactive access to health status information, tasks, targets, plans and a social reinforcement network that encourages adoption and maintenance of a healthier lifestyle for improved wellness. When you use the CareHere Connect system, you re participating in the creation of a health-oriented culture that can last a lifetime. To Get Started: Go to Click Member Login, enter your CareHere username and password you chose during registration, and click Go. At the left-hand side of the screen, you will find the CareHere Connect button. Click it, and your CareHere Connect portal will open.

Read about or contact your coaches under COACHES Smartphone APP available for Android & IPhones Your health care provider will work with you to choose one or more plans that will be loaded under MY PLANS. Provider referred plans include weight management, exercise, cholesterol management, diabetes management and tobacco cessation. Share your success or receive support from others across the country on the MY NETWORK page all anonymously if you so choose. 10 Frequently Asked Questions Do I have to pay to use the center? No. Pasco County Sheriff s Office provides access to the center FREE for all employees and their family members on the PCSO medical insurance plans. What are some of the benefits of using the CareHere center? In addition to no charge for using the center, CareHere offers free generic medicines, free lab work and vaccinations, confidential medical records, the convenience of scheduling your appointment online, the ability to access your medical records online, more one-on-one time with the doctor, a convenient location open 6 days/week, and an average wait time of less than 5 minutes in the waiting room. What can be treated at the CareHere center? The clinic providers can treat colds, flu, sore throats, high blood pressure, high cholesterol, diabetes, asthma, annual physicals, school physicals, lab work, EKG s, pap smears, blood work, vaccines and much, much more! Can I use the CareHere clinic doctors as my primary care doctor? Absolutely! Can my doctor order labs and I get them drawn through the CareHere center? Yes, but we require that you make an appointment with the CareHere Provider, so they can sign off on lab orders. The labs can be forwarded to your physician. If I choose to keep my doctor, but I m seen for something at the CareHere clinic, how will my doctor know? You may sign a release of information form at the clinic and request your information be forwarded to your doctor. Can the CareHere clinic doctors refer me to a specialist? You will need to schedule an appointment with a CareHere doctor to discuss your need for a specialist. What do I do if I have a medical emergency, and the clinic is not open? If you have a life-threatening emergency, please call 911. If the medical need is not life-threatening, please feel free to call our 24/7 Call Center at They will help you with determining if you need to be seen in the ER or be given an appointment time when the clinic is next open.

11 GENERIC MEDICATIONS DISPENSED AT THE CAREHERE CENTERS AS OF JANUARY 2015 All medicines dispensed, including refills, require an appointment with a clinic provider. Please make 2 back-to-back appointments when you have 3+ medicines needed We recommend you bring a list of any medicines you are currently taking to your first clinic visit. The best way to take advantage of the new generics coming to market is to ask a very simple question: Is there a generic for that? Check it out during your checkup: At the doctor s office, bring all of your prescriptions, and be sure to ask the doctor or nurse, Is there a generic for that? Give your prescriptions a checkup: Review all of your medications regularly with a doctor or pharmacist, because there may be new, lowercost treatments available. Find out before you fill: When you hand over a new prescription to your pharmacist, or during refill or renewal time, ask, Is there a generic for that? The pharmacist can tell you and then call your doctor to discuss changing the prescription accordingly. Understand the difference between equivalents and alternatives: A generic equivalent contains the same active ingredients as the brandname version; a generic alternative is similar to the brand-name medication you currently use but is not the exact same medication. If there is no generic equivalent available, ask your doctor or pharmacist if there is a generic alternative that would be clinically appropriate for you.
