7 339 Quality of Life in Patients with Heart Failure or Myocardial Infarction after Intensive Care Fumi MAKIYAMA (Accepted Nov. 15, 2003) Key words : quality of life(qol), acute heart disease, self completed questionnaire, quik Abstract (Subjects) To assess quality of life (QOL) in patients with myocardial infarction (MI) or heart failure (HF) after intensive care as compared with healthy control. 18 HF, 17 MI patients and 35 randomly selected healthy control were included. (Design) A new QOL questionnaire (QUIK) was given to in-patients at primary care hospital. QUIK is a self-completed and close-ended questionnaire, which covers four domains including physical functioning (20 questions), emotional adjustment (10), interpersonal relationships (10), and attitudes toward-life (10), totaling 50 questions from the perspective of holistic system theory. (Results) 1) QUIK scores in the patients group were significantly worse than the healthy group in physical functioning (p<0.001), emotional adjustment (p<0.01), and attitudes toward life (p<0.0001) of QOL, however, no significant differences were found in interpersonal relationships. 2 In the patients group, factors on related matters of QOL (age, sex, hyperlipemia, hypertension, BMI, smoking, employment status, living alone, having dependent children, recurrence, chest pain and stroke) were not significantly different between the poor QOL group and the good QOL group, although smokers had slightly greater impairment of QOL than non-smokers. 3 HF patients had more impairment in the QOL domain of attitudes toward life than MI patients. Other QOL domains saw no significant difference between the two groups. 4 Diastolic cardiomyopathy (DCM) patients had more impairment in attitudes toward life of QOL than the non-dcm group. Other QOL domains showed no significant difference between the two groups. (Conclusions) Patients with heart disease after intensive care had more impairment QOL as compared with healthy control. My study suggests that we not only have to care for physical and mental symptoms, but also to helppatients remake their attitude toward life.
