Cardiac Arrest Its about time. Improving survival rates in Cheshire Presented by Alex Hayes
Part 1: Current Performance Part 2: How do we Compare? Part 3: Visions & Aspirations
Part 1: How are we performing now?
This presentation will last 15 minutes So What?
There are approximately 30,000 Out of Hospital Cardiac Arrests (OHCA) in the UK every year Which equates to 83 per day Or 4 per hour Or 1 every 15 minutes.
Their chance of survival is..?
8.6%
This is Bill. Bill is about to have a Cardiac Arrest
The clock is ticking

For every minute that passes without Cardio Pulmonary Resuscitation (CPR) Bills chances of survival reduce by

In a Nutshell - without any intervention, Bill has 10 minutes until zero survivability
Statistically, Bill only has a 43% chance that a by-stander will begin CPR
2%

Nationally, the Ambulance Service have difficulty responding quickly to rural communities Holmes Chapel is no exception to this rule
The target attendance time for a life threatening medical emergency is 8 minutes In Holmes Chapel this target is achieved 16% of the time But because we have a OC firestation within this community our attendance time for this location would be approximately 5 minutes
5 minutes = 50%

With High Performance CPR
Part 2: How do we compare?

Bills Chance of surviving a OHCA: 8.6% 21% 25% United Kingdom North Holland Norway 62% Seattle

Factors that determine survival from a cardiac arrest SYSTEM PATIENT EVENT
Factors that determine survival from a cardiac arrest SYSTEM PATIENT EVENT Gender Age Comorbidity
Factors that determine survival from a cardiac arrest SYSTEM PATIENT EVENT Location Rhythm Witnessed collapse
Factors that determine survival from a cardiac arrest SYSTEM PATIENT EVENT Education Response Clinical Excellence

Norway: CPR taught in schools since 1961 Rates of by-stander CPR: 73% UK = 43%
Seattle, USA 62%
MEDIC 1 SCHEME 1970 All 3 Emergency Services respond to OHCA On scene within 4 minutes on 90% of occasions. High Performance CPR by emergency responders Increased availability of public access AEDs High rates of CPR training for local residents
Part 3: Visions & Aspirations
Improving survival rates in Cheshire
What can we do? Cheshire Fire & Rescue Cardiac Response Pilot Scheme 6 Months 2 x Whole-time stations 2 x On-Call stations In addition to the North West Ambulance Service New Zoll AEDs Improve CPR standards New Resusci-Anne mannequins Introduction of high performance CPR in Cheshire Fire & Rescue Service
Standard CPR: Slows the dying process High Performance CPR: Virtually suspends the dying process -Allows time for additional shocks and Advanced Life Support to succeed.
Why should we do this?
Cheshire Fire & Rescue Service can respond quicker than NWAS 74.7% Up to 6 minutes 35 seconds quicker in some locations
Saving life it s our core business!
A thousand lives a year could be saved if firefighters responded to cardiac arrests with defibrillators Professor Andy Newton Director of Clinical Operations, South East Coast Ambulance Service
Recognised that the quality of CPR is of vital importance Quality CPR requires a minimum of 3 persons Ambulance service staff welcome our response
Our excellent fire safety work has led to a reduction in Cheshire Fire & Rescue Service calls we responded to 15,127 incidents we responded to 7,821 52% reduction in 9 years We have the spare capacity
Estimates taken from North West Ambulance Service data Crewe / Warrington: 2-3 cardiac medical emergency calls per station per Week Holmes Chapel: up to 15 per year Frodsham: up to 55 per year
The future? Provide CPR and AED training to the community Provide CPR and AED training to all school age pupils Help to increase availability of public access AEDs Stations and elsewhere All Cheshire Fire & Rescue Service assets responding to Cardiac Arrest emergencies
What have we learnt?
It s all about time Every second counts! We can put the right people with the right skills in the right place at the right time The Fire & Rescue Service can enhance the SYSTEM and help to improve survival rates
If you are going to have a Heart Attack, have it in Seattle
