1 FOLLOW-UP AFTER EMERGENCY DEPARTMENT VISIT FOR PEOPLE WITH HIGH-RISK MULTIPLE CHRONIC CONDITIONS (FMC) APPLICATIONS OBJECTIVE Purpose of Measure: ELIGIBLE POPULATION Which members are included? STANDARD OF CARE NCQA ACCPETED CODES HEDIS (Administrative) To ensure members ages eighteen (18) years and older who were seen in the emergency department (ED)for treatment and have high-risk multiple chronic conditions received appropriate followup care within 7 days of discharge. Members age eighteen (18) years and older as of the ED visit. *Members in hospice are excluded. Exclude ED visits followed by an admission to an acute or nonacute inpatient care setting on the date of the ED visit or within 7 days after ED visit. Members eighteen (18) years and older who were seen in the emergency department for treatment and who have high-risk multiple chronic conditions and had a follow-up service within 7 days of the ED visit. Please see code table. The Following are eligible chronic condition diagnoses: COPD and Asthma Alzheimer s disease and related disorders Chronic Kidney Disease Depression Heart Failure Acute myocardial infarction Atrial Fibrillation Stroke and Transient ischemic attack DOCUMENTATION REQUIREMENTS What documentation The medical record should document the DOS for follow-up visit and all aspects of the visit, including but not limited to: physical exam findings, thorough and diagnosis-appropriate mental health

REFERENCES assessment, medication list, medication side effects, compliance with prescribed treatment, questions/concerns member and/or caregiver may have, etc. Follow-up service within 7 days include the following and may occur on the date of the ED visit: An outpatient visit A behavioral health visit A telephone visit Transitional care management services Case management visits Complex Care Management Services For more information, please refer to Passport Health Plan s Clinical Practice Guidelines.
