Requiring a level of training generally associated with persons who have completed the residency program prescribed by the merican oard of Internal Medicine, and a level of skill and ability generally associated with persons who are diplomates of that board. asic privileges in general medicine Thoracocentesis bdominal paracentesis one marrow aspiration one marrow biopsy Liver biopsy Sigmoidoscopy, proctoscopy with biopsy spiration of joints & subcutaneous lesions Lumbar puncture rterial puncture Skin iopsy entral Line Placement & Removal riteria: ocumented ompletion of the entral Venous atheter Procedure ducation and Training Module N ocumentation of at least one of the following: 1. oard ligibility/oard ertification in specialties that include V Insertion and/or removal procedure as part of the basic delineated competencies (ie. nesthesia, ardiology, mergency Medicine, General Surgery, Interventional Radiology and Nephrology). 2. ompletion of one-month experience/rotation in Interventional Radiology. 3. ttestation or certification by Service hief or Program irectors. 4. ocumented supervised insertion procedures (5 internal jugular, 5 femoral, 5 PI, 5 subclavian). ocumented supervised removal procedures (2 non-tunneled lines) Muscle iopsy Subspecialty Privileges: Patient management at this level encompasses ategory 1 patient management, the items marked below and similar activities. 3 T as adjunct to another peripheral procedure erebrovascular iagnostic Interventions riteria: Technical competence requirements: a) Peripheral Vascular diagnostic privileges (17a) b) Peripheral Vascular therapeutic privileges (17b & 17c) c) 30 diagnostic cerebrovascular angiogrmas >= 15 as primary doctor 1 rch & arch branch angiography 2 Selective arch branch angiography U erebrovascular therapeutic interventions (excluding intracranial vessels): riteria: Technical competence requirements: a) Peripheral Vascular diagnostic privileges (17a) b) Peripheral Vascular therapeutic privileges (17b & 17c) c) 25 carotid stents with >= 12 as primary operator Maintenance of ompetence: t least 24 procedures every two years, meeting published guidelines for complication rates. 1 Percutaneous transluminal angioplasty 2 mbolic protection device use 3 Insertion of stems and stent-grafts V oronary T ngiography: Perform and interpret (with contrast) oard ertified in ardiology (If not board certified, completion of an GM Interventional ardiology Fellowship) ompetence Requirements: 1) 8 weeks training 2) Perform at least 50 mentored examinations 3) Interpret at least 150 mentored W X Right Ventricular biopsies MNGING VNTRIULR SSIST VIS (V) To apply for privileges as a cardiologist managing V, the applicant must meet the following criteria: 1. e a current member in good standing or meet all requirements for appointment to the Medical Staff Office (MSO). 2. urrent board certification in ardiology as well as board certification or board eligible in dvanced Heart Failure and Transplant ardiology by the merican oard of Internal Medicine or its foreign equivalent. 3. Meet the following criteria:. Trained and experienced in advanced heart failure therapies.. Manages at least 5 ventricular assist device implanted patients a year.. Sufficient competency in evaluating patients for heart transplantation as evidenced by having worked in or trained in a heart transplant center.. Letter of recommendation from the Transplant/Mechanical irculatory Support Medical irector from last place of work or training. 3/5

4 Y T 3 Maintenance riteria: pplicants wishing to retain privileges in managing ventricular assist devices must meet the following requirements: 1. ctive participation in the USF s Ventricular ssist evice program. 2. Maintaining board certification as outlined above. Nuclear Medicine (SPT Myocardial Perfusion Imaging). linical Privileges shall be defined as those standard, usual and customary procedures appropriate to the diagnosis of any and all diseases encompassed by this procedure.. The physician must be able to document proficiency by both training and experience.
