Step 1: Make a Booking* Choose the type of medical services make a booking with Macau Yinkui Hospital directly according to the contact information below: For AIA Vitality Health Check/designated health check package/advanced screening/vaccination 1. Step 2: Perform health check/vaccination/face-to-face nutrition consultation/dental care service Present your AIA Vitality membership card personal identification document upon arrival pay the respective fee. Step 3: Collect health check results If you go for AIA Vitality Health Check or other health check package, a hard copy of your results will be available for your collection in person as specified below: AIA Vitality Health Check: same day upon completion Other health check packages: approx working days upon completion Step 4: Earn the AIA Vitality Points While you re there for health check/vaccination/face-to-face nutrition consultation/dental assessment, you must sign the consent form provided by Macau Yinkui Hospital for the disclosure of certain proof or relevant test result(s) to AIA Vitality so that points will be awarded to you automatically once we receive your proof or test result(s). 2

3 The consent form re-confirms your agreement for Macau Yinkui Hospital to release certain proof or relevant test result(s) to us in order to calculate AIA Vitality Points. Only minimal relevant information data will be transferred to us for the sole purpose of calculating AIA Vitality Points, AIA Vitality will only use store such information data in accordance with the terms conditions of AIA Vitality Programme AIA Vitality Personal Information Collection Statement. Note: It is voluntary for you to sign the consent form. If you do not sign the consent form, AIA Vitality will not be able to calculate process AIA Vitality Points, we will not be able to award any AIA Vitality Point to you for this activity. However, you can still enjoy the discount offered by Macau Yinkui Hospital while AIA Vitality will have no access to any of your test result(s). Earning AIA Vitality Points Members can earn points for completing the following health check/vaccination/face-to-face nutrition consultation/dental assessment, additional points for results fall within (if applicable). Once we receive the proof/result(s) of your health check/vaccination/face-to-face nutrition consultation/dental assessment, AIA Vitality will automatically award you with the relevant points for each eligible test/result accordingly.
