3 Introduction Oxford Health Plans has developed this survey with the hopes of better understanding employed Americans attitudes and behaviors concerning their health. This survey focuses on opportunities and obstacles to good health in the workplace. Specifically, this research addressed the following question areas: - Overall self-evaluation of health and diet - Current eating and exercise habits - Unhealthy behaviors, e.g., smoking and drinking - Presence of stress at work - Opportunity for exercise and healthy eating at work - Participation in specific health programs at work News reports recently noted obesity is about to become the number one preventable health hazard in the country (for the first time overtaking smoking). Many of the illnesses that plague us today are directly and indirectly related to our lifestyle poor exercise and eating habits. When working at a desk job, the lack of physical exercise becomes even more ingrained in our habits, compounding the problem. 3
4 Methodology Telephone interviews were conducted in April 2002 using a random digit dial (RDD) method. - 1,000 interviews nationwide augmented interviews in New York, New Jersey and Connecticut (n=150 from each state) - More details regarding the sampling plan can be found in the Appendix A market segmentation analytical approach was used to cluster the population into groups based on their reported behaviors and self-appraisals on specific health-related questions. See the Appendix for a detailed description of the clustering procedures. 4

The Four Health Wellness Segments of Americans THERE ARE FOUR MAJOR HEALTH WELLNESS SEGMENTS OF AMERICANS BASED ON THEIR HEALTH-RELATED BEHAVIORS AND THEIR SELF- APPRAISALS OF THEIR HEALTH AND HEALTH HABITS. The four health wellness segments are: The Worried Well Those with relatively few unhealthy behaviors but low perceived health. The Truly Healthies Those with relatively few unhealthy behaviors and high perceived health. The Health Evaders Those with a relatively large number of unhealthy behaviors and low perceived health. The Health Illusionists Those with a relatively large number of unhealthy behaviors but high perceived health. 7

14 Worried Well (18%) Overall Health This segment, by definition, is a contradiction. They exhibit fewer unhealthy behavior items, yet they perceive themselves to be less healthy than the other segments. Relatively low perceived health (7.3 on a 10-point scale) Less likely to have any of the conditions included in the survey. The one exception, and possible reason for their lower perceived health scores, is the proportion that say they are overweight (20%). 14
15 Truly Healthies (23%) Overall Health This segment is the healthiest, in self-perception and reality. The highest perceived health (9.6 on a 10-point scale) Least likely to have any of the conditions included in the survey 15
16 Health Evaders (32%) Overall Health This segment exhibits a logical combination of lower perceived overall health with several potentially serious health conditions. Lowest perceived health (6.9 on a 10-point scale) among all of the segments More likely to have high blood pressure (29%), high cholesterol (25%), a heart condition (12%), diabetes (13%), asthma (11%) or arthritis (27%) A majority of the diabetes cases are classified as Adult Onset Diabetes Nearly half say they are overweight (46%) especially 25 lbs. or more (66%) 16
17 Health Illusionists (12%) Overall Health Despite reported health conditions, Health Illusionists believe they are very healthy overall. Among the highest perceived health (9.5 on a 10-point scale) among all of the segments More likely to suffer from arthritis (33%). Also more likely to have high blood pressure (17%) and high cholesterol (18%), but not at nearly the same levels as the Health Evaders One in four say they are overweight (41%) especially 25 lbs. or more (55%) 17

19 Worried Well Overall Diet - Respondents perceive there is room for improvement in their overall diet (7.2 on a 10-point scale) - However, their behavior belies their perceptions. They are more likely to: Drink eight glasses of water per day (50%) Eat five servings of fruits and vegetables (40%) Take vitamins (58%) Eat three meals per day (73%) Eat a balanced breakfast in the morning (52%) 19
20 Truly Healthies Overall Diet - The highest perceived overall diet rating (7.2 on a 10- point scale) - Very similar eating patterns compared to the Worried Well. They are more likely to: Drink eight glasses of water per day (56%) Eat five servings of fruits and vegetables (47%) Take vitamins (60%) Eat three meals per day (76%) Eat a balanced breakfast in the morning (61%) 20
21 Health Evaders Overall Diet - Lowest perceived overall diet (6.0 on a 10-point scale) - Some bad eating habits combined with other healthier eating habits suggest this segment is trying to eat healthier More likely to eat salty or sugary snacks (27%), fried foods (25%), and purchase food and snacks from vending machines at work (36%) Less likely to eat three meals a day (52%), take vitamins (40%), and drink eight glasses of water per day (33%) Also report eating five serving or fruits and vegetables per day (24%) and a balanced breakfast (33%), but these levels are not as high as those from the Worried Well and the Truly Healthies 21

24 Worried Well and Truly Healthies Help With Eating Problems - The main difference between this group and the Truly Healthies is that the Worried Well are more likely to have been told by a medical professional that they needed to lose weight (21% vs. 10%) - The Worried Well are more likely to have been to a nutritionist at some point in their lives (36%) 24
25 Health Evaders Help With Eating Problems - 41% of the Health Evaders have been told by a medical professional to lose weight even more (69%) feel they need to lose weight This is not surprising considering two-thirds of this segment is 25+ pounds overweight - This need to lose weight has not necessarily translated into a call to action only 13% have consulted a nutritionist in the past year 25
26 Health Illusionists Help With Eating Problems - 24% of the Health Illusionists have been told by a medical professional to lose weight even more (61%) feel they need to lose weight These proportions are lower than those for the Health Evaders - Similar to Health Evaders, this need to lose weight has not necessarily translated into a call to action only 9% have consulted a nutritionist in the past year 26

29 Health Evaders Vices This segment is more likely to engage in certain behaviors that could lead to serious health problems in the future. More likely to drink more than three glasses of alcohol per day (19%) More likely to smoke cigarettes (31%) They are just as likely as other segments to drink coffee or tea (77%), however, they do drink MORE coffee or tea (25% drink 4 or more cups per day) 29
30 Health Illusionists Vices This segment perceives itself to be healthier overall compared to Health Evaders. However, the fact that they participate in the vice activities gives rise to the theory that they are either in denial, or they are somehow overcompensating for these bad behaviors. More likely to drink more than three glasses of alcohol per day (21%) More likely to smoke cigarettes (31%) They are just as likely as other segments to drink coffee or tea (79%), however, they do drink MORE coffee or tea (29% drink 4 or more cups per day) 30

32 Worried Well Exercise Habits - This segment is one of the more active segments (only 9% say they never exercise) - However, they appear to feel unsatisfied with their exercise habits I should be more active (58%) I would be healthier if I exercised more often (58%) 32
33 Truly Healthies Exercise Habits - This segment is also one of the more active segments (only 9% say they never exercise) - However, where the Worried Well seem unsatisfied with their exercise habits, the Truly Healthies are able to more fully realize the benefits of exercise At the end of the work day, I do some form of exercise (45% vs. 31% for the Worried Well ) I should be more active (41% vs. 55%) 33
34 Health Evaders Exercise Habits - This segment is very similar to the Health Illusionists in terms of their lack of physical exercise - However, they are less likely than Health Illusionists to feel guilty about it 59% feel they should be more active (compared to 66% of Health Illusionists ) 34
35 Health Illusionists Exercise Habits - One-third (32%) never exercise, however, evidence shows that they feel exercise would be beneficial 66% feel they should be more active 70% say they would be healthier if they exercised - Barriers to exercise also appear to exist for this segment They are less likely than other segment to report feeling better after exercise One-third (34%) report some sort of physical discomfort at the end of the work day, which may lead to a sedentary existence over half (52%) spend most of their after work time sitting 35

37 Perception of Work Impact on Health Nearly two-thirds (63%) of employed Americans say they look forward to going to work They tend to be older and from the Southern or Western U.S. However, about one-third report descriptions of their workdays that are not very desirable: - Once I m at work, I have no downtime (35%) - From the time I arrive at work to the time I leave to go home, I never leave the building (35%) - I generally eat lunch and work at the same time (32%) And fewer say: - My family and friends think I am a workaholic (26%) - I sit at my desk most of the day (24%) - I often miss personal family activities due to my job (16%) - I am unable to use up my vacation time due to demands at work (16%) - My job makes me feel older than I am (13%) - My work often causes me to lose sleep at night (13%) - At least one day a week my ability to perform my job fully is restricted due to my personal health or disability condition (6%) 37
