No.: CD DOI: / CD Background A B S T R A C T Newborn animal and human pilot studies suggest that mild hypothermia following peripartum hypoxia-ischaemia in newborn infants may reduce neurological sequelae, without adverse effects. Objectives To determine whether therapeutic hypothermia in encephalopathic asphyxiated newborn infants reduces mortality and long-term neurodevelopmental disability, without clinically important side effects. Search strategy The standard search strategy of the Neonatal Review Group as outlined in the Cochrane Library (Issue 2, 2003) was used. Randomised controlled trials evaluating therapeutic hypothermia in term newborns with hypoxic ischaemic encephalopathy were identified by searching the Oxford Database of Perinatal Trials, the Cochrane Central Register of Controlled Trials (CENTRAL, The Cochrane Library Issue Issue 2, 2003), MEDLINE (1966 to July 2003), previous reviews including cross-references, abstracts, conferences, symposia proceedings, expert informants and journal hand searching. Selection criteria Randomised controlled trials comparing the use of therapeutic hypothermia with normothermia in encephalopathic newborn infants with evidence of peripartum asphyxia and without recognisable major congenital anomalies were included. The primary outcome measure was death or long-term major neurodevelopmental disability. Other outcomes included adverse effects of cooling and early indicators of neurodevelopmental outcome. Data collection and analysis Three reviewers independently selected, assessed the quality of and extracted data from the included studies. Authors were contacted for further information. Meta-analyses were performed using relative risk and risk difference for dichotomous data, and weighted mean difference for continuous data with 95% confidence intervals. Main results Two randomised controlled trials were included in this review, comprising 50 term infants with moderate/ severe encephalopathy and evidence of intrapartum asphyxia. There was no significant effect of therapeutic hypothermia on the combined outcome of death or major neurodevelopmental disability in survivors followed. No adverse effects of hypothermia on short term medical outcomes or on some early indicators of neurodevelopmental outcome were detected. 1

4 Authors conclusions Although two small randomised controlled trials demonstrated neither evidence of benefit or harm, current evidence is inadequate to assess either safety or efficacy of therapeutic hypothermia in newborn infants with hypoxic ischaemic encephalopathy. Therapeutic hypothermia for encephalopathic asphyxiated newborn infants should be further evaluated in well designed randomised controlled trials. P L A I N L A N G U A G E S U M M A R Y Not enough evidence that induced hypothermia (cooling) of newborn babies who may have suffered from a lack of oxygen at birth reduces death or disability A lack of oxygen before and during birth can destroy cells in a newborn baby s brain, and this damage continues for some time afterwards. One way to try and stop this damage is to induce hypothermia - cooling the baby or just the baby s head for some hours to days. This may reduce the amount of damage to brain cells. This review found that there is not enough evidence from trials to show whether or not induced hypothermia helps newborn babies at risk of brain damage. More research is needed. B A C K G R O U N D Peripartum asphyxia affects 3-5 per 1000 live births with subsequent moderate or severe hypoxic ischaemic encephalopathy (HIE) in per 1000 live births (Levene 1986). HIE is a major problem worldwide as 10-60% of affected infants die, and at least 25% of survivors have long-term neurodevelopmental sequelae (Vannucci 1990). There are no specific treatments proven to decrease brain damage from HIE. Hypothermia is a clinically feasible manoeuvre which may improve the outcome of neonates with HIE. Recent clinical and experimental studies demonstrate that neuronal death occurs in two phases following a reversible hypoxicischaemic global insult (Gluckman 1992; Lorek 1994; Penrice 1996). If the insult is severe, there may be immediate primary neuronal death, related to cellular hypoxia with exhaustion of the cell s high energy stores (primary energy failure). After a latent period of at least 6 hours, the secondary phase of delayed neuronal death is initiated (Williams 1991). The mechanisms involved include hyperaemia, cytotoxic oedema, mitochondrial failure, accumulation of excitotoxins, active cell death (analogous to developmental apoptosis), nitric oxide synthesis, free radical damage and cytotoxic actions of activated microglia (Inder 2000). The delayed phase is associated with encephalopathy and increased seizure activity, and accounts for a significant proportion of final cell loss even after very severe insults. In term infants with evidence of intrapartum hypoxia and moderate to severe encephalopathy, magnetic resonance spectroscopy studies are consistent with this biphasic model of neuronal death. They demonstrate normal cerebral oxidative metabolism shortly after birth followed by secondary energy failure, the degree of which predicts outcome (mortality and neurodevelopmental outcome at both one and four years of age) (Roth 1997; Roth 1992). Therefore a therapeutic window of opportunity exists in the interval following resuscitation of the asphyxiated newborn, before the secondary phase of impaired energy metabolism and injury. There are a number of postulated mechanisms by which hypothermia may be neuroprotective. It may modify cells programmed for apoptosis leading to their survival. In neonatal piglets, 12 hours of mild hypothermia after resuscitation significantly decreased the number of apoptotic cells, but not the number of necrotic cells (Edwards 1995). Hypothermia may also protect neurons by reducing cerebral metabolic rate, attenuating the release of excitatory amino acids (glutamate, dopamine), ameliorating the ischaemiaimpaired uptake of glutamate and lowering production of toxic nitric oxide and free radicals. Several term and preterm animal experimental models have demonstrated that a reduction in brain temperature of 2-3 degrees Celsius immediately following a hypoxic-ischaemic insult reduces energy expenditure, improves subsequent performance testing and/or reduces histological neuronal loss (Laptook 1994; Laptook 1997; Thoresen 1995; Gunn 2001). In the term fetal lamb, a significant reduction in histological neuronal loss was seen with extradural temperatures below 35 degrees Celsius (Gunn 1997a). Temperature modelling calculations also suggest that lowering an infant s core temperature to below 35 degrees Celsius is required to produce any reduction in the deep brain temperature 2

5 (Van Leeuwen 2000). For many decades deep hypothermia to less than 28 degrees Celsius has been shown to be valuable for neuroprotection during cardiac arrest for open-heart and neurosurgical procedures. There are three Cochrane systematic reviews of the effect of systemic cooling on outcome of human adults following head injury (Gadkary 2003), acute stroke (Correia 2003) and coronary artery bypass surgery (Rees 2003). There is currently no evidence from randomised controlled trials and these systematic reviews to support hypothermia for treatment of either head injury or acute stroke, or for prevention of neurological injury after coronary artery bypass surgery. In addition, hypothermia may be harmful by increasing the risk of sepsis (Gadkary 2003), as well as perioperative myocardial dysfunction and mortality (Rees 2003). Mild hypothermia appears well tolerated in a variety of animal experimental models, as well as in adult human studies (Gunn 1997b, Thoresen 1995; Thoresen 1996; Haaland 1997; Marion 1997). There have been no reported serious adverse effects in four pilot studies of hypothermia in human newborns (Gunn 1998; Azzopardi 2000; Thoresen 2000; Shankaran 2002). Adverse effects such as sinus bradycardia, increased blood pressure and increased oxygen requirement were all transient and reversible with rewarming (Thoresen 2000). Identification of infants with hypoxic-ischaemic brain injury at risk of future disability who may benefit from hypothermia is challenging. It may be particularly difficult to distinguish between encephalopathy secondary to intrapartum hypoxia, and that related to antepartum factors (Badawi 1998a; Badawi 1998b) or underlying congenital abnormalities not easily recognisable at birth (Felix 2000). Recent newborn animal and adult human studies are consistent with the potential for rescue hypothermia being greatest following moderate, rather than severe, hypoxic-ischaemic insults (Marion 1997; Haaland 1997). Aspects of cooling therapy that remain controversial include: how soon after the insult or birth treatment needs to be started, what level of hypothermia is required, what method (selective head cooling versus whole body cooling) should be used and the duration of cooling required. Effective therapies are urgently required to prevent neurosensory impairment following peripartum asphyxia. This systematic review aims to review the evidence to determine whether therapeutic hypothermia reduces adverse outcome in encephalopathic asphyxiated newborn infants. O B J E C T I V E S In newborn infants suffering from HIE, does therapeutic hypothermia reduce the rate of death and long-term neurodevelopmental disability without clinically important side effects? Secondary objectives include assessment of the adverse effects of cooling and effects on early prognostic indicators of adverse outcome. Subgroup analyses were planned on the basis of: 1. Severity of HIE (mild, moderate, severe) (Sarnat 1976; Finer 1981) 2. Inclusion criteria a) Preterm (< 35 weeks gestation) versus term or near term (> 35 weeks gestation) b) Electrophysiological plus clinical criteria versus clinical criteria alone 3. Timing of commencement of intervention (<3 hours versus 3-6 hours versus > 6 hours) 4. Method of cooling (whole body versus selective head cooling with mild systemic hypothermia) 5. Degree of cooling [core temperature (or surrogate eg rectal temperature) <= 34.5 versus > 34.5 degrees Celsius] 6. Duration of cooling (<= 48 hours versus > 48 hours) 7. Quality of outcome assessment [high quality (> 18 months with formal psychological testing and review by developmental paediatrician for diagnosis of cerebral palsy) versus lower quality] R E S U L T S Fifty term infants with moderate or severe encephalopathy and evidence of intrapartum asphyxia have been enrolled in two randomised controlled trials to determine the effect of therapeutic hypothermia on short term medical (Gunn 1998; Shankaran 2002) and long term neurodevelopmental outcomes (Gunn 1998). Death or major neurodevelopmental disability: One trial (Gunn 1998) with 31 randomised participants showed no evidence of an effect of hypothermia on the primary outcome of death or major neurodevelopmental disability in survivors [RR 1.26 (95% CI 0.46, 3.44), RD 0.08 (95% CI -0.26, 0.42)]. Secondary outcomes: 1. Each component of primary outcome: a) Mortality: Two trials with 50 infants (Gunn 1998; Shankaran 2002) did not demonstrate a significant effect of hypothermia on mortality [RR 0.73 (95% CI 0.26, 2.09)]. b) Major neurodevelopmental disability: One trial (Gunn 1998) reported major neurodevelopmental disability in the 23 survivors assessed and did not show a significant effect of hypothermia on this outcome [RR 3.08 (95% CI 0.40, 23.44), RD 0.21 (95% CI -0.10, 0.52)]. 3

6 c) Cerebral palsy: One trial (Gunn 1998) reported no significant effect of hypothermia on cerebral palsy in the 23 survivors assessed [RR 2.31 (95% CI 0.28, 18.99), RD 0.13 (95% CI -0.16, 0.43)]. d) Developmental delay or intellectual impairment: One study (Gunn 1998) reported no significant effect of hypothermia on developmental delay [RR 5.00 (95% CI 0.29, 86.43), RD 0.23 (95% CI -0.06, 0.52)] in 22 of 23 survivors assessed or on intellectual impairment [WMI (95% CI , 3.31)] in 21 of 23 survivors assessed. e) Blindness: None of the 22 survivors assessed were blind in the one trial (Gunn 1998) reporting this outcome. f) Sensorineural hearing loss requiring amplification: No survivor assessed (n=22) had hearing loss requiring amplification in Gunn Adverse effects of cooling: a) Heart rate i) Sinus bradycardia: One trial of 31 infants (Gunn 1998) did not demonstrate a significant effect of hypothermia on sinus bradycardia [RR 2.21 (95% CI 0.10, 50.33), RD 0.06 (95% CI -0.11, 0.22)]. ii) Prolonged QT interval: Not reported iii) Arrhythmia requiring medical intervention and/or cessation of cooling: None of the 31 infants in whom this outcome was reported had any arrhythmia requiring medical treatment (Gunn 1998). b) Blood pressure i) Hypotension: not reported ii) Need for inotrope support: Two trials comprising 50 infants (Gunn 1998; Shankaran 2002) did not demonstrate a significant effect of hypothermia on requirement for inotrope support for hypotension [RR 2.41 (95% CI 0.82, 7.08)]. c) Full blood examination: One trial with 31 participants (Gunn 1998) reported no significant effect of hypothermia on anaemia requiring blood transfusion [RR 3.68 (95% CI 0.19, 70.88), RD 0.11 (-0.08, 0.30)], leukopaenia [RR 2.21 (95% CI 0.10, 50.33), RD 0.06 (95% CI -0.11, 0.22)], or thrombocytopaenia [RR 2.53 (95% CI 0.62, 10.25), RD 0.24 (95% CI -0.06, 0.53)]. d) Coagulation i) Any coagulopathy: Not reported ii) Coagulopathy resulting in major thrombosis or haemorrhage: One trial with 31 infants (Gunn 1998) did not find a significant effect of hypothermia on major thrombosis or haemorrhage secondary to coagulopathy [RR 3.68 (0.19, 70.88), RD 0.11 (-0.08, 0.30)]. e) Hypoglycaemia: There was no significant effect of hypothermia on hypoglycaemia in the 31 infants in one trial (Gunn 1998) [RR 1.8 (95% CI 0.41, 7.90), RD 0.12 (95% CI -0.16, 0.41)]. f) Hypokalaemia: In one trial with 31 infants (Gunn 1998), there was also no significant effect of hypothermia on hypokalaemia [RR 1.16 (95% CI 0.49, 2.73), RD 0.06 (95% CI -0.29, 0.41)]. g) Elevated lactate: Not reported h) Renal impairment i) Urea: Not reported ii) Creatinine: Not reported iii) Oliguria: All 31 infants in Gunn 1998 experienced oliguria. The meta-analysis of the results of the two included trials found no significant effect of hypothermia [RR 0.67 (95% CI 0.22, 2.03)]. i) Culture proven sepsis was reported in one trial with 31 infants (Gunn 1998) and there was no significant effect of hypothermia on this outcome [RR 0.72 (95% CI 0.05, 10.52), RD (- 0.20, 0.16)]. 3. Early indicators of neurodevelopmental outcome: a) Severity of encephalopathy was reported in one trial with 31 participants (Gunn 1998) without any significant effect of hypothermia on moderate or severe encephalopathy [RR 1.17 (95% CI 0.70, 1.97), RD 0.11 (95% CI -0.23, 0.44)]. b) Severity of electroencephalogram (EEG) abnormality: Not reported c) Seizures: Both studies, with a total of 50 participants, reported no significant effect of therapeutic hypothermia on the incidence of seizures [RR 1.37 (95% CI 0.67, 2.80)]. The number of anticonvulsants required was not reported as an outcome in either study. d) Diffusion weighted imaging on early MRI: Not reported. e) Basal ganglia, posterior limb of internal capsule and/or white matter injury, parasagittal neuronal necrosis on late MRI: Not reported. f) Standardised neurological assessment day: Not reported g) Days to full sucking feeds: Not reported In summary, there was no significant effect of hypothermia on short term adverse outcomes or on neurological status in the first three days of life (severity of encephalopathy or incidence of seizures), although many secondary outcomes planned to be included in this review have not been reported. Planned subgroup analyses on the basis of severity of HIE, gestation, timing of commencement of cooling, as well as the method, degree and duration of cooling could not be performed. 4

7 D I S C U S S I O N This systematic review demonstrates neither benefit nor harm for mortality and long-term neurodevelopmental disability from therapeutic hypothermia in newborn infants with hypoxic ischaemic encephalopathy. In addition, there is no significant effect of hypothermia on potential adverse short term medical outcomes. However, the small number of infants included in this review and the wide confidence intervals for all reported outcomes mean that cautious interpretation of these results is warranted. There are at least five ongoing randomised controlled trials of whole body cooling in term infants with HIE (ICE; Shankaran/ NIH; Simbruner; The CoolCap Trial; TOBY). The results of these trials will be incorporated into future updates of this review. newborns with hypoxic ischaemic encephalopathy. Although the limited available evidence does not suggest harm from controlled hypothermia, current evidence is inadequate to assess either safety or efficacy of therapeutic hypothermia on newborn infants with hypoxic ischaemic encephalopathy and suggests that cooling should only be performed in the context of randomised controlled trials. Implications for research Well designed and executed studies with appropriate power are required to assess the effectiveness and safety of therapeutic hypothermia, particularly with respect to mortality and neurodevelopment, as well as short-term outcomes.
