1 Albury Wodonga Health Albury Campus Welcome to the Albury campus of Albury Wodonga Health, it is also referred to as the Albury Hospital and occasionally Albury Base Hospital. It s a great place to work so we hope you enjoy your placement with us! This is the person who coordinates your placement and is responsible for your mid placement and final evaluation. During your placement you are likely to work with a range of other health professionals both within and outside your discipline, with a range of professional experiences. It is important to remember that your supervisor(s) continue to carry a full workload during your placement, and you will need to be sensitive to his/her other responsibilities. This will also mean that you may be required to work more autonomously and independently during you placement. Throughout your placement you will receive a mix of direct and indirect supervision. A range of facts such as the complexity of the activity/roles you are undertaking and your level of skills and knowledge determine the degree of supervision. Discuss with your supervisor early the types of supervision that will be provided, considering both the capacity of your supervisor and your own supervision requirements.

2 Weekly student sessions (IPL) Each week a student interprofessional learning (IPL) sesssion is offered at each campus. These are offered to allied health and nursing students and it is strongly suggested you atttend. The sessions range from simulation to case studies to topics of interest designed to help you get to know the other students and their disciplines. The sessions are great fun and hope you can come along! Please let Anna Sullivan (Allied Health Student Coordinator) know if you are not able to attend. Albury IPL Tuesday 3-4pm in the Allied Health Group Room Other useful information: Lunch There are lots of lovely places to eat lunch. Most of us congregate in the allied health courtyard, please come and join us. Computer access: Generic access to computers will be provided for access to your discipline drive, student drive and other useful applications. This login will have internet available for work related purposes. Lockers: There are student lockers available in the student room or in your discipline department. A locker will be assigned to you during orientation, the key must be returned on your last day. Photocopying: the photocopier is located near Allied Health Reception. 3 Allied health Teams at Albury Hospital The Allied Health Department is mostly structured on a discipline basis rather than a program base. Please read through the general information below and then find more information about your specific placement by clicking on the discipline links. Surgical Ward two Surgical ward two is a 30 bed unit. The ward mostly consists of surgical patients particularly minor and major abdominal surgery. o Phone: 4414 Surgical Ward One Surgical ward one is a 20 bed unit. The ward is mostly acute orthopedic however it has 5 designated AGEM beds. o Phone: 4420 Medical Ward Medical Ward has 30 beds. Care is provided to acute neurological, cardiorespiratory, and oncology patients. Patients may also be admitted with general medical or functional decline. Aged care planning is provided in this setting. o Phone: 4410 Rehabilitation Ward The rehabilitation ward is a 21 bed, inpatient sub acute service that uses a team approach when working with patients to achieve optimal independence and quality of life. People are commonly admitted to the rehab ward following an acute hospital admission however direct admissions from the community also occur. Mid 2015 the rehabilitation ward will be moving to Wodonga Campus o Phone 4424 Intensive Care Unit (ICU) The unit consists of 6 ICU/HDU beds, 4 coronary care beds and 2 single rooms to be used as required. o Phone: 4470 Emergency Department The Emergency Department consists of 12 beds and 2 resus bays. Single cubical and a plaster room are also available. o Phone: 4454 Children s Ward Children s ward is an 18 bed unit. The ward is for pediatrics with a wide range of medical and surgical conditions. o Phone: 4404

4 Acute Geriatric Evaluation Management (AGEM) AGEM is a multidisciplinary rehab service specializing in the care of patients over the age of 65 years with age related conditions. This is a 10 bed service however patients can be located anywhere in the hospital. Patients are most commonly admitted from an acute ward within Albury Wodonga Health. This is a goal based program with the aim to achieve optimal independent and quality of life. Community Rehabilitation Centre (C.R.C team) The CRC is an outpatient service for people who have a physical disability, are frail, chronically ill, or are recovering from traumatic injury. CRC also helps people who have newly emerging chronic symptoms that require assessment, diagnosis and treatment. This service aims to improve and maintain a person s optimal independence in the community or in their daily life. o Phone: 4830 Cognitive Dementia and Memory Service (CDAMS) The memory clinic is a multidisciplinary team that assesses changes in memory and thinking. They assess early memory changes and provide you with an accurate diagnosis and individual management plan. 5 Allied health Assistant placements (Albury Campus) During your 3 week placement you will be given a co-ordinator/supervisor and mentor whom work collaboratively to oversee and provide a supportive environment for your placement experience. Please note that you will also be given the opportunity to work with all members of the Allied Health Assistant team, including Allied Health Professionals. We have 6 AHAs at Albury; all are experienced and work in specific areas. This will expose you to patients with a range of conditions and levels of abilities enabling you to learn from staff with various teaching styles. At the commencement of your placement you will be provided with a three week roster (See below for example). The first week you are expected to follow a different AHA each day of the week. The second week you will be predominantly working with the AHP. The third week enables a little more flexibility, providing an opportunity to gain more in-depth knowledge and experience within the area/s that you may have a particular interest whether discipline specific or medical conditions. Should you have a preferred area you wish to participate in please advise your supervisor early on in the placement. You will be required to meet specific objectives whilst on placement, which will be provided; this is in addition to the objectives supplied by your RTO. Please Note: Students are required to contact the supervisor no later than 1 week prior to commencement if this is possible to arrange a quick interview. 10 Dietetics and Nutrition placements (Albury Campus) Albury Wodonga Health Dietetic Student Placements Students are expected to contact placement supervisor/s at least two weeks prior to the placement to discuss expectations. The Dietetics Departments can be contacted on: (02) (Wodonga Campus) (02) (Albury Campus) (02) (Albury Community Health) Community Public Health Nutrition Placement Wodonga Campus or Albury Community Health: This placement focusses on students working on a nutrition project and undertaking small group education. The project involves the process of planning, conducting and evaluating a community nutrition program. The topic for the community nutrition program is set by the program supervisor/s and is based on client and service needs. Students will be expected to research relevant topics related to the project prior to commencement of the placement in accordance with the University requirements for Community Public Health Nutrition placement. Food Service Placement- Albury or Wodonga Campus The primary objective of Clinical Services in the Nutrition Department is to provide and promote a high level of nutritional care for inpatients We anticipate the food services projects enhance this care. During your placement here, you will investigate and develop your Food Services Project and present your findings to both the Dietitian s and Food Services. Your project will have been developed in collaboration with Food Service and your University to ensure it will be able to meet your competency standards. Food Service and Dietetics are available for guidance with your work.

11 Clinical Placements at Albury Wodonga Health In the clinical placement you will provided the opportunity to see patients in the acute, subacute and outpatient setting. You will work as part of a multidisciplinary team including physiotherapy, occupational therapy, social work, speech pathology and an allied health assistant. They also work closely with medical and nursing staff. The caseload will consist mostly of adult patients of varying ages and with a wide variety of medical conditions, often with complex socioeconomic backgrounds. All referrals to dietitian must be prioritised referrals may include nutritional assessment for enteral and parenteral nutrition, diabetes, stroke, malnutrition, chronic lung or heart disease and post-surgical patients. Essential equipment includes: o o o Griffith handbook or equivalent Calculator Any books, textbooks, ready reckoners etc. which you feel will be helpful Please research the following to prepare for your placement 1. Research the common conditions we see on the acute wards Diabetes, Stroke, COPD, CCF, NSTEMI, Malnutrition, Nutritional Supplements for Malnutrition, Refeeding Syndrome. 2. How to calculate nutritional requirements for all age groups including enteral and parenteral nutrition. 3. Consider what educational material might be appropriate for patients to receive for each condition. 12 Exercise Physiology placements (Albury Campus) The Exercise Physiologist works full time in the Community Rehabilitation Centre (CRC). CRC provides interdisciplinary outpatient rehabilitation to patients living in the Albury region. Our team consists of; Physiotherapists, Occupational Therapists, Speech Pathologists, Dieticians, Social Workers, Nurses and an Allied Health Assistant. The Exercise Physiologist delivers exercise for the prevention and management of chronic diseases such as cardiac, respiratory, neurological, psychological and metabolic as well as musculoskeletal injuries. The Exercise Physiologist also works with clients and staff in the following CRC groups - Cardiac Rehabilitation, Pulmonary Rehabilitation and the Falls Prevention. 13 Occupational Therapy Placements (Albury Campus) After reading the relevant section don t forget to please return to the generic orientation package in survey monkey to complete the final step of the orientation package Medical Ward Acute placement The OT on the medical ward work as part of a multidisciplinary team including physiotherapy, social work, dietetics, speech pathology, discharge planner medical and nursing staff, in particular the Nurse Unit Manager (NUM). The caseload is adult patients of varying ages (primarily over 65) and with a wide variety of medical conditions. Occupational Therapist may see a patient due to the following; Decline in ability to perform ADLs / difficulty coping at home Decline in ability to mobilise or transfer / change of mobility aid impacting on ability to safely manage in home environment Upper limb condition leading to functional impairment Risk of falls Acute neurological event e.g. Stroke, TIA, Guillen Barre Recent and / or long term changes in cognitive state impacting on function Risk of developing pressure areas The role of OT is to complete assessments to determine the impact of a patient s condition on their occupational performance. Assessments commonly completed include initial assessments, functional assessments, cognitive assessments, neurological assessments, assessment for the provision of equipment for ward and home use, and home assessments. Relevant intervention, including equipment provision, ADL retraining, upper limb therapy, and education, is then completed to facilitate safe discharge home if appropriate. The OT working on medical ward will often refer patients to other services for ongoing management, for example, in-patient rehabilitation, community rehabilitation, transition care program, or the rural allied health team. The OT will also liaise closely with the NUM, flow nurse and discharge planner in relation to service provision on discharge to assist with ADLs including personal care showering assistance, meals on wheels, home help for cleaning and gardening, transport and shopping assistance.

14 Surgical Ward Acute placement The OT on the surgical ward works as part of a multidisciplinary team including physiotherapy, social work, dietetics, speech pathology, discharge planner, nursing and medical staff. The caseload is adult patients (primarily over 65) following general and orthopaedic surgery. Patients typically are referred to occupational therapy for; Change in ability to perform ADLs Change in ability to mobilise or transfer / change of mobility aid impacting on ability to safely manage in home environment Risk of falls Recent and / or long term changes in cognitive state impacting on function Risk of developing pressure areas Head injury The role of OT is to assess the impact of a patient s condition on their occupational performance and provide subsequent recommendations regarding intervention, therapy and/or changes to their home environment. The OT is involved in determining readiness for discharge and making recommendation regarding need for supports or services in the community and/or the need for in-patient or community rehabilitation. Rehabilitation The OT on the rehabilitation ward works within a multidisciplinary team including a rehabilitation consultant, a rehabilitation register, dietetics, physiotherapists, social work, speech pathology, nursing and allied health assistants. We see people of all ages who have a variety of illnesses, conditions and disabilities. The OT in the rehabilitation team completes assessments, treatments and interventions to facilitate participation in the individual s daily occupations. This may include: personal care, meal preparation, upper limb therapy, cognitive therapy, home modifications, prescribing equipment such as shower chairs, over toilet frames and wheelchairs and working with clients to find strategies to manage their occupational roles. We also run group programs such as Breakfast Group and Skills Centre. Our aim is to assist people in achieving their maximum level of occupational function to ensure a safe discharge from hospital.

15 Community Rehabilitation Team (C.R.C. team) The CRC team is a multidisciplinary team which aims to enable clients who are disabled, frail, chronically ill or recovering from traumatic injury to achieve and retain optimal functional independence. Clients with more complex needs and who require more than one discipline are seen by this team. Disciplines include: Speech Pathology, Physiotherapy, Occupational Therapy, Dietetics, Community Nursing, Exercise Physiology and Allied Health Assistant. It is a goal directed service. People identify personal goals to work on while linked into the service. CRC includes General rehabilitation Cardiac rehabilitation and Pulmonary rehabilitation Upper limb/hand therapy clinic Falls and Balance program Lymphoedema Click here to see the CRC brochure Hand Therapy Placement Hand therapy aims to provide an individualised service for all hand, wrist and arm needs, including assessment and treatment of injuries, conditions or post-operative care. Some of the common diagnoses seen in this program include: Hand and finger injuries, tendon/ligament injuries, nerve injuries and conditions, fractures and dislocations, amputations, post-operative care, arthritis, burns, pain, scar tissue, wounds and oedema. These conditions could be from an accident, sporting incident, surgery, or just a recurring pain that is starting to impact quality of life. A placement in hand therapy would include seeing outpatient clients in fracture clinic as well as through the community rehabilitation program at the hospital setting. The Occupational Therapists in hand therapy complete assessment, treatment and intervention to improve or maintain hand and upper limb function. This may include splinting, scar and oedema management, pain management, exercises to increase movement and strength and sensory re-education. Prior to attending placement in hand therapy it would be beneficial to review hand and upper limb anatomy as well as ROM assessment methods (goniometry). 16 Physiotherapy placements (Albury campus) After reading the relevant section don t forget to please return to the generic orientation package in survey monkey to complete the final step of the orientation package 1 st, 2 nd and 3 rd year placements These placements can be in a variety of areas of the hospital depending on where your supervisor is placed. Areas of practice include medical ward, intensive care, surgical wards, community rehabilitation, inpatient rehabilitation and outpatients. Please see below for descriptions of these areas. Placements may also involve time with various groups such as cardiac rehabilitation, exercise groups and pulmonary rehabilitation. Multidisciplinary experiences are also integrated into your timetable and there is the opportunity to observe surgery cases if possible. Please contact the physiotherapy manager (or other provided contact) prior to your placement for details of your placement and determine any pre-reading requirements. In general before coming on placement (1st year): 1. Consider what questions you might like to ask while you are on placement 2. Refresh your subjective assessment skills 3. Review gait and goniometry assessment In general before coming on placement (2nd and 3rd year): 1. Research the common conditions you are likely to see on your allocated ward/area of practice 2. Refresh your subjective and objective assessment skills 3. Review gait and balance assessment 4. Consider what exercises might be appropriate for patients you are likely to see

17 5. Review progress notes SOAP format 4 th year placements Fourth year placements are usually in a speciality area but may involve time in other areas to broaden your clinical experience. Notes are written in the SOAP format so please revise this format before you arrive. Acute Care placement Fourth year physiotherapy acute care placements take place on the Albury Campus of Albury Wodonga Health. The placements typically run for 4 or 5 weeks depending on the specific requirements of the tertiary institution. The major areas of practice are the surgical wards, medical ward and intensive care/ high dependency units. Depending again on the university requirements your experience may be split across general surgery and orthopaedic surgery or may concentrate purely on cardiorespiratory physiotherapy. Pulmonary and cardiac rehabilitation are not typically included however attendance at a session may be negotiated with supervisors. During the acute care placement you may also be required to provide physiotherapy to paediatric patients and to patients in the emergency department. Most commonly you will be seeing patients post -surgery (abdominal or orthopaedics), patients with respiratory illnesses (both acute and chronic), patients with functional decline and impaired mobility with complex discharge planning issues and patients whose condition requires a higher level of monitoring and support in the intensive care setting. Your principle supervisors will be Sarah White (Cardiorespiratory) and Nicole Humphreys (Orthopaedics). It is much appreciated if you contact the supervisors via the physiotherapy department on (02) at least one week prior to placement commencing to discuss any questions/ concerns and to confirm attendance and start times. You will be required to have sound working knowledge of the theory of physiotherapy in the acute setting and as such should revise the prescribed texts from your university relating to acute care physiotherapy and should revise lecture and practical notes in preparation for the placement. All students are required to bring a stethoscope to placement.

18 Musculoskeletal Placement Staff- Senior physiotherapists: Jackson Boyd, Alana Wilson, Rachelle Lowe, Nicole Humphreys (inpatients) plus rotating Level 1-2 Physiotherapist. This placement will involve work in the outpatient department, fracture clinic, hydrotherapy pool, Community rehabilitation Centre (CRC) and possibly the inpatient orthopaedic ward. The Outpatient Physiotherapy Department is a busy and dynamic environment that provides physiotherapy to a vast range of referred clients, in addition to managing fracture clinic and aquatic physiotherapy programs. Students would be likely to have exposure to any of the following: post surgery including TKR, THR, rotator cuff repairs, anterior cruciate ligament reconstruction and Achilles tendon repairs. post fracture back and neck pain shoulder pain, RC tears and impingement acute soft tissue sprains and strains OA, RA and joint assessments Deconditioning following long hospital stay Aquatic physiotherapy (up to four times per week, so please bring your swimmers) Neurological deficits Application and removal of plaster with supervisor or AHA Prior to commencing placement, it would be advised that students revise the following: Basic goniometry, strength, length and special orthopaedic tests Stages of bone and soft tissue healing Principles of aquatic physiotherapy and effect of immersion Anatomy Best practice management of some common presentations (as listed above)

19 Rehabilitation placement Your placement will occur on the 21 bed mixed inpatient rehabilitation ward. Case load includes stroke, SCI, MS, other neurological conditions, orthopaedics, amputees and deconditioning. On a neurological core competency placement we will endeavor to give you predominately a neurological case load. Senior physiotherapy staff are trained in the Bobath concept so a good introductory text to review would be "Steps to Follow" by Patricia Davies and chapter 14 "Out of Line" is a great resource on Pusher syndrome. Also be familiar with outcome measures in the "Manual of Clinical Outcome Measurement in Adult Neurological Physiotherapy" by Keith Hill. During your placement you may be involved in hydrotherapy (bring your bathers), group programs, interdisciplinary treatment sessions, team meetings, family meeting and goal setting.

20 Speech Pathology placements (Albury Campus) Speech Pathology placements will take place in a variety of areas in the hospital depending on which wards your Clinical Educators are working on. Areas of practice include the Emergency Ward, Intensive Care Unit, Medical Ward, Surgical Wards, Inpatient Rehabilitation, Community Rehabilitation Centre and Speech Pathology Outpatients. Your Clinical Educator will discuss with you prior to the placement the areas you will be working in and provide guidance regarding preparation activities for your placement. Please return to the generic orientation package in survey monkey to complete the final step of the orientation package Social Work placements (Albury Campus) 4 th year social work placements are 70 days and are based on the wards at Albury Hospital. You may also spend time in the community rehabilitation program such as cardiac rehabilitation, the emergency department, Nolan House (mental health residential facility) and on home visits with clients. You will be involved with family meetings, discharge planning, goal setting and will work closely with other members of the nursing, medical and allied health teams.
