Introduction Trunk muscle weakness has been reported as a risk factor for chronic LBP. Lee JH, et al. Spine Strengthening provides an improvement in chronic LBP. Patients with severe LBP cannot continue the exercise due to their pain and muscle weakness, especially in the elderly.
Innovative exercise device for the abdominal trunk muscles used in the treatment for chronic LBP Points of this device 1 Strengthening exercise in sitting position without movement of the painful low back. 2 Accessible to the elderly with a lowered physical function 3 Built-in system for measuring abdominal trunk muscle strength.
Introduction Locomotive syndrome is a condition of reduced mobility due to impairment of locomotive organs among the elderly. Low back pain is significantly related with the locomotive syndrome. Iizuka Y, et al. J Orthop Sci It is unknown whether abdominal trunk muscle strength is associated to chronic LBP and the locomotive syndrome.
Purpose This study aimed to investigate the correlations between abdominal trunk muscle weakness, chronic LBP and the locomotive syndrome in the elderly women. Trunk muscle weakness Locomotive syndrome Chronic LBP
Methods Subjects 160 elderly women, who were scheduled for joint surgery in the lower extremities at our institution. Evaluation Items evaluated before surgery Age, Height, Weight, BMI Grip Power (high value on the left and right) The 25-Geriatric Locomotive Function Scale (GLFS-25) Abdominal trunk muscle strength measured by our device
Evaluation subjects (a mean age of 65 years) Abdominal trunk muscle strength Pearson correlation coefficient analysis Age Height Weight BMI Grip power GLFS-25
Result 1 Abdominal trunk muscle strength showed a positive correlation with grip power, and a negative correlation with GLFS-25 scores (the locomotive syndrome). Abdominal trunk muscle strength (ATMS) [kpa] Average Correlation with ATMS 5.2±3.1 ー Age [Year] 65± Height [cm] 152.8± Weight [kg] 55.1± Body Mass Index [kg/cm 2 ] 23.6± Grip Power [kg] 21.5± GLFS-25 [point] 41.3± p < 0.01 (Pearson correlation coefficient analysis)
Evaluation 2 GLFS-25 Q2 Severe pain Considerable pain Did you have any pain in your back, lower back or buttocks? Moderate pain Mild pain No pain LBP group 60 subjects (average 65.1y.o.) Mann-Whitney U test non-lbp group 100 subjects (average 64.1y.o.) Age, Height, Weight, BMI Grip Power (high value on the left and right) the 25-Geriatric Locomotive Function Scale (GLFS-25) abdominal trunk muscle strength measured by our device
Result 2 The mean abdominal trunk muscle strength in the LBP group was significantly lower than that in the non-lbp group LBP non-lbp P value Abdominal Trunk Muscle Strength [kpa] 4.3± ±3.3 <0.01 Age [Year] 64.8± ± Height [cm] 152.6± ± Weight [kg] 55.4± ± Body Mass Index [kg/cm 2 ] 23.7± ± Grip Power [kg] 21.8± ± GLFS-25 [point] 52.7± ±19.6 <0.01 The mean score of GLFS-25 in the LBP group was significantly higher than that in the non-lbp group
Discussion Abdominal Trunk Muscle Weakness Low Back Pain Locomotive Syndrome Abdominal trunk muscle strength was associated with chronic LBP and the locomotive syndrome. The abdominal trunk muscles has an important role in stabilization of the lumbar spine. The muscle weakness can produce LBP and reduce physical functions in the elderly.
Conclusions We measured abdominal trunk muscle strength in elderly women using our innovative device. We found that the muscle weakness was associated with chronic low back pain and the locomotive syndrome. COI : no conflict of interest associated with this study.
