To ensure the timely and equitable management of preschool aged children through the Paediatric Physiotherapy Service. What overarching policy the procedure links to? 3 Preschools clinic process New Patient Referral and Triage Any referrals identified as being urgent will be seen within 5 working days. Any appointment identified as ASAP will be brought to the attention of the preschool Physiotherapist so that an earlier appointment can be provided. Any referrals identified as being talipes and/or torticollis and/or plagiocephaly will be triaged into the appropriate clinic. Referrals will be sent for in date order as soon as a space is available in the Paediatric Physiotherapists schedule. An appointment letter will be provided/ sent for the child to attend the appropriate clinic. In order to adhere to the 18 week waiting list policy, referrals approaching this breach will be brought to the weekly service meeting to discuss. New Patient DNA Patients attend the clinic with an adult who has parental responsibility of the child. If the initial appointment is not attended then they are discharged back to the referrer unless the referral indicates a child in need or a child protection plan is in place. If this is the case contact is made to the keyworker/ Social worker/ Health visitor involved in order to see if any support is required following the initial appointment. If following this another appointment is not attended then the patient is then discharged back to the referrer. New Patient No treatment required Patients attend the clinic with an adult who has parental responsibility of the child. A full assessment is undertaken and if child does not require treatment then a report is sent to the child`s parents and the referrer and the child is discharged. New Patient Treatment required A full assessment is undertaken and indicates that Physiotherapy treatment is appropriate. The child will then be offered either a review and/or treatment sessions in an appropriate setting and/or onward referral either internally or externally. A consent form will be signed and discussed with parents. Patient notes will be recorded at the conclusion of every treatment session and goals may be set at the initial assessment or at subsequent sessions. A report will be completed either after the initial assessment or after subsequent sessions. Current patient DNA X2 If a child is currently on treatment and fails to attend 2 treatment sessions or group sessions then they will be immediately discharged from the Paediatric Physiotherapy services. A discharge letter will then be sent to the referrer as well as the parents/ guardian of the child. Discharge Children will be discharged from the Physiotherapy service when Physiotherapy treatment is no longer required or is not effective. 4 Relevant Data Sheets Following each patient contact a Data sheet will be completed. Patients NHS Number, patient surname and forename, time of appointment and appointment outcome. Please refer to the Trust s Mandatory & Risk Management Training Needs Analysis for further details on training requirements, target audiences and update frequencies. Monitoring / Review of this Procedure In the event of planned change in the process(es) described within this document or an incident involving the described process(es) within the review cycle, this SOP will be reviewed and revised as necessary to maintain its accuracy and effectiveness. Outside of the above the procedure will be reviewed 9 months following full implementation and will give way to retrospective audit projects to assess its benefit.
