O-goig cosultatio is available by phoe to help you uderstad the itet of each stadard ad what it meas i the cotext of your orgaizatio s stroke program. 3 The o-site survey takes place. The survey is scheduled i cosultatio with your orgaizatio ad coducted by a cliicia with experiece i stroke care. The survey experiece is positive ad educatioal. Cocers or questios will be idetified while the surveyor is o-site ad a report of ay deficiecies is provided withi 10 days of completio of the survey. 4 Provide your pla of correctio. Tell us how you pla to address the deficiecies idetified. 5 Certificatio decisio is made. Certified orgaizatios are ivited to participate i quarterly peer-led commuity of practice telecofereces. 6 Maitai compliace with the certificatio stadards. Certificatio lasts for three years ad requires submissio of quarterly data measures. For Primary Stroke ad Thrombectomy, certificatio icludes a mid-cycle desktop review.
