1 Rebalance MD Update At Rebalance MD we are passionate about musculoskeletal health and strive to provide expert, quality and accurate care, holding ourselves to the highest standard of health care excellence. We believe in a more efficient and effective health care environment, firmly rooted in innovation, research, education, collaboration, and community involvement. As part of our mission to provide the highest quality of services throughout the entire musculoskeletal care spectrum, we have assembled a comprehensive team of accomplished physicians and adjunct health professionals from the fields of Orthopaedics, Physiatry, Sports Medicine and Physiotherapy into a single allied group practice focused on excellence. Rebalance MD is now in its 4 th year of operation. We are the home of musculoskeletal care in Victoria and now employ over 1 people with over 4 physicians working under the Rebalance MD banner in some capacity. Our Services Include: Physiatry: We have 3 full time physiatrists at Rebalance MD providing a wide range of services. We have expertise in all areas of Rehabilitation Medicine, including electrodiagnostics (nerve conduction and EMG studies), complex pain management, point of care ultrasound and injections, medical legal consultations and work- related injuries. Our Physiatrists also participate in our triage modelling to ensure all patients with acute injury, sports related injuries, and concussion are seen efficiently and appropriately. Sports Medicine: We have a very experienced Sports Medicine team at Rebalance MD. They provide full service Sports Medicine and non-operative care. We have also just began a new Sports Medicine fellowship beginning July 216 at Rebalance MD. This will provide interested trainees a one year fellowship compliment in Sports Medicine. We would like to welcome Dr. Christopher Lipp (PGY-4 Department of Emergency Medicine) as our first fellow. Orthopaedics: All of Victoria s Orthopaedic surgeons currently work at Rebalance MD. We provide subspecialty Orthopaedic care for all areas except the neck (neurosurgery) or hand (plastic surgery).

2 Island Osteoporosis Clinic: Dr. Jesse Pewarchuk is pleased to offer internal medicine expertise in the evaluation and treatment of osteoporosis. Referrals for primary, secondary osteoporosis or risk stratification are accepted. Patients with a history of fragility fractures, risk factors for osteoporosis or strong family history are commonly referred. Peri-operative Internal Medicine and Anaesthesia: Rebalance MD has partnered with our colleagues providing specialist medical care for our perioperative patients. We now have Internal Medicine and Anaesthesia providing in-house consultations for appropriate patients awaiting total hip or knee arthroplasty. Our hope is with the success of this program it will expand to include all appropriate patients cared for through Rebalance MD. The New Joint Program: This is the remodelled perioperative total joint program. We have brought it under the Rebalance MD banner and have successfully escalated the throughput from 18 cases per year to over 16 cases in 215. We employ 6 Navigators (nurses, OTs, PTs) who provide pre-operative counselling, group education classes, home visits, and assist each arthroplasty patient through the operative journey. Physiotherapy: We have on-site a full service physiotherapy clinic providing general and specialized physiotherapy care. We have also partnered with Island Health to deliver the PoPP (Postoperative Physiotherapy Program) for total hip and knee replacements in the Capital Regional District. Patients that have had total knee or total hip replacements are covered for physiotherapy and exercise therapy after they have their surgeries- there is no charge to the patients. This replaces the previous program at the Adanac location at Royal Jubilee Hospital. Outpatient physiotherapy services are still being offered at Saanich Peninsula Hospital for its local patients and those from the Gulf Islands. The Arthritis Center is also an option for those patients that have inflammatory disease and require post-surgical intervention for their hips or knees. For patients that are from up island, the hospital will send an e-referral for therapy to their local outpatient hospital therapy services. Patients also always have the option to go to their local physiotherapist at their own expense. Splinting, Bracing and Orthotics: One Bracing is pleased to provide a large variety of products, orthoses and expertise to address your splinting, bracing or orthotic needs. Our services include: Complete Lower Limb Assessments - Including gait and biomechanical assessment of the lower limbs; thorough medical history; examination of current footwear, range of motion tests and video gait analysis. Lower & Upper Limb Splinting & Bracing - Our in-house professionals are qualified to measure and fit a full line of lower and upper limb bracing and casting products. From a custom built

3 knee brace for a sports injury to a wrist wrap for tendonitis, we have the products you need and the professionals to ensure you get the product that is right for you. We work in consultation with your doctor to provide and fit a full complement of over the counter and custom bracing products. Footwear Modifications and Custom Orthoses - We provide a variety of footwear modifications including lifts and stretching for width, length and deformities such as bunions. In addition we provide a full spectrum of orthotic options including custom orthoses which are constructed using a custom mould of each individual s foot and always manufactured inhouse by a certified pedorthotist. Measuring Success: At Rebalance MD we have worked hard to reduce patient wait times for access to specialist care. We utilize the FAAST (First Available Appropriate Specialist Triage) system. This means patients can access the most appropriate physician with the shortest current wait for their particular complaint. The FAAST system has proven a vast reduction in overall wait times in the W1 (wait between GP referral and specialist consult) which is seen in our comprehensive data collection and wait time management tool (see below). We have robust data both retrospective and now also considering prospective scheduling for each of our physicians so we truly know where the shortest access to services are for patients. Below is our trend report, produced each month showing how our wait times have changed since implementing the FAAST system. What you will see is we have greatly increased our capacity to see patients. We have gone from around 4 consults per month in 212 (pre Rebalance MD ) to averaging around 12 consults per month now. In doing that we saw a huge reduction in wait times from over 2 weeks average to around 1 weeks now. In improving access we have also seen a huge increase in the number of referrals coming in from Victoria and else where, rising from around 4 to over 11 per month now. We feel we are likely seeing steady state for the island s need now and are proud to say we are able to meet that need. Number of Referrals per - 212/16 Referrals

As we have improved our efficiency we have seen a growth in surgical wait time (W2 wait from decision for surgery and operative date). Fortunately, as one of the strengths of our organization we have rallied the government to see this issue and provide improved resources for access to surgical care. We are expecting a 6 per cent increase in our total hip and knee arthroplasty cases as a result of this, an increase from 18 joints in 214, to 14 in 215 and over 16 in 216. We have recently rolled out a PROMS (Patient Reported Outcome Measurement Score) and PREMS (Patient Reported Experience Measurement Score) to allow us to document our patients experience with Rebalance MD. We are very excited about this project and are very interested in our patients opinion of the services we provide, as well as objectively measuring their outcomes through the treatment journey. Patients will fill out questionnaires specific to their disease classification using standardized outcomes tools through a web based patient portal. This portal will also allow patients to track their appointments, results, wait times, etc through their own secure login portfolio. Results from this data collection will allow us to show impacts of wait times on patient outcomes, impacts of our care on their outcomes, and their overall experience through our clinic. This will allow us to respond and provide the absolute best care and experience possible to our patients.

6 Education: Rebalance MD prides itself in its participation in education. We contribute to education in all facets of our organization. From medical students, residents, and fellows to students in physiotherapy, kinesiology, nursing, athletic therapy and MOA programs, Rebalance MD serves over 1 learners annually. We are currently piloting a core medical student rotation in musculoskeletal care to replace the pre-existing Orthopaedics block. Students will have the opportunity to spend time with Orthopaedics, Sports Medicine, Physiatry, and Physiotherapy. We hope this will give students a broader exposure, understanding and appreciation of the options available for musculoskeletal care. We are also now proud to offer a Sports Medicine fellowship as described earlier. The addition of fellowship level learners allows us to expand our capacity while providing broad spectrum education across all of our available specialties and subspecialties. We embark to provide not only an excellent, well balanced educational experience, but also pride ourselves on sharing and exposing others to our models of care as part of their education at Rebalance MD. Community Involvement: Rebalance Legacy Society Rebalance Legacy Society is a charitable organization with both community and international focus. Our doctors and physiotherapists provide injury prevention training to local youth sports teams by working directly with the coaches and the players. We also provide sponsorships for exceptional teams and athletes. Abroad, our surgeons work with medical professionals to provide opportunities for further training, implement new technology, and streamline inefficient healthcare systems. Our goal is partnership at both the community and international level. Collaboration is key in both making competitive sports safer for youth and bringing medical aid to those in need. Our team will visit Uganda twice this year, providing new technology and training. We are also hosting Ugandan colleagues in Victoria for training this summer. Our Partners The Comprehensive Rehabilitation Services for Uganda (CoRSU) Hospital is a not-for-profit non-government organization located in Kisubi, Uganda. CoRSU Hospital provides free orthopaedic surgery to children in need and develops programs to prevent further orthopaedic injuries. The Uganda Sustainable Trauma and Orthopaedic Program (USTOP) is a Vancouver-based charitable organization, founded by the University of British Columbia s Faculty of Medicine. USTOP aims to reduce suffering, disability, and poverty caused by traumatic injuries by improving the education of surgical trainees at Mulago Hospital Uganda s national referral hospital.

Young Artist Competition We believe in excellence and improving the relationships between patients and health care professionals throughout the Vancouver Island community. We aim to engage with the local community by encouraging creativity and culture in Vancouver Island s youth through the Young Artists Competition. Each year Rebalance MD decks its halls with the artwork of the very talented youth of Vancouver Island. We host the annual Young Artist Competition, a high school-based competition, and opportunity for young artists to produce and showcase their artwork. Each year we produce a theme (this year our theme was Balance ) and artists are challenged to produce work inspired by the theme. At the end of the competition Rebalance MD offers to purchase the artwork of our winners and this artwork is what our waiting areas and hallways are decorated with. We encourage you to come in and enjoy some of the magnificent work. Rebalance MD Coaches the Coaches At Rebalance MD we value taking an active role in the community. We are particularly proud of our involvement with youth soccer programs on the island. Neurological and movement training are important components of injury prevention. Ankles, knees and Achilles tendons are often compromised due to compensation mechanisms in young soccer players with poor mechanics. To prevent such injuries, we have teamed up with local soccer teams, including the Vancouver Island Wave elite soccer program.
