4 1 Introduction / Background Gastrointestinal temperature can be recorded noninvasively using an ingestible telemetric sensor/pill. The single-use pills transit body temperature measurements wirelessly to an external data logger, which can be stored or monitored continuously. This method offers an alternative way to measure deep body temperature in spinal cord injured persons, as rectal temperature is considered inappropriate for this population. An in-house calibration was established to ensure the accuracy of each telemetric pill prior to use. The process involved removing the pill from its packaging, placing it in an airtight plastic bag, then immersion it in a water bath set to 36 C, 37.5 C and 39 C. The temperature of the water is changed in an ascending, then descending order and readings were taken from the telemetric pill logging device and compared to a reference mercury thermometer each minute for 10 minutes at the target water bath temperatures. Once the calibration process was complete, the pill was re-packaged. To ensure the pill remains sanitary throughout the calibration process, gloves were worn when the pill was being handled, and a hygienic workspace was maintained. More recently, Hunt et al. 1 published a paper that details the importance of calibrating gastrointestinal pills prior to use, and a process that can be followed for future studies using telemetric pills to measure gastrointestinal temperature. It has been recommended that the pill should be ingested six hours before the commencement of monitoring 2. However, if the trial involves fluid ingestion, 10 hours has been suggested as a more appropriate time frame to ensure progression of the pill past the stomach, thus minimising the influence of the fluid temperature on gastrointestinal measurements 3. In order to check if the pill is past the stomach, the participant can swallow a mouthful of cold water prior to commencing an experimental trial. 5 2 Facility and equipment 2.1 Testing facility requirements There are minimal facility requirements for this method, which is why it is such a practical way of measuring core body temperature in the laboratory and field-based environments. 2.2 Equipment Laboratory-based equipment Equipment Water bath Photo/description To be used for the calibration process. 7 4 Workflow 4.1 Before the participant arrives Before the participant arrives for testing, there are several steps that must be taken to prepare. Firstly, the individual information that appears on the packaging that the sensor comes in need to be added to the logging device. Seconding, the logging device needs to be used to test that the sensor is active and working. An additional step of calibrating each sensor was taken for this particular project to be sure that the sensors were measuring accurately prior to being given to the participant. Each of these steps are detailed below. Adding a Telemetric Sensor: 1. From the first menu screen (EXIT/STBY Screen) press the MENU button on the keypad. 2. On the next screen press the READ button until the ADD SENSORS/SENSOR ACT screen displays. 3. Press F1 on the recorder keypad to ADD SENSOR. The following screen will display: Enter 2-digit sensor # of your choice (1-99) 5. When the sensor # is entered the following screen will display: Press F1 on the recorder keypad to program the Serial # from the pill package. 7. Enter the 6-digit Serial #. 8. Press F2 on the recorder keypad to enter the Calibration #. 9. Enter the 8-digit Calibration # from the pill package. 10. When both numbers have been entered, press READ to enter the next sequential sensor. If no other sensors are required, press ENTER to return to the ADD SENSOR / SENSOR screen. 8 Sensor Test: A SENSOR test should be run before administering the pill to the participant to ensure it is active. This test should also be done periodically on ALL stored pills to ensure no magnets have come lose, draining the pills battery before use. From EXIT/STBY mode, press MENU. 2. Press READ until the ID#/SENSOR TEST screen appears: 3. Press F2 on the recorder keypad to initiate SENSOR TEST. 4. The recorder will then activate and continuously check for a signal, displaying SENSOR ON or SENSOR OFF whether a signal is present or not. Place the receiver over the pills in order to ensure they are activated or inactivated. Press ENTER to return to the ID#/SENSOR screen. 4.2 Participant preparation Each participant must be given the core temperature sensor prior to the testing date to ensure they swallow the pill several hours prior to arriving for testing. If possible, this can be given to the participant when they come in for a preliminary session. An example of an information sheet regarding how to take the sensor is attached with this document (Appendix A). The pill should be ingested six hours before the commencement of monitoring. However, if the trial involves fluid ingestion, 10 hours has been suggested as a more appropriate time frame to ensure progression of the pill past the stomach, thus minimising the influence of the fluid temperature on gastrointestinal measurements. Furthermore, when measuring gastrointestinal temperature in spinal cord injured persons, the sensor should be swallowed 10 hours prior to testing under all conditions, as digestion generally occurs at a slower rate in this population. 9 4.3 Data collection Entering SPORTS MODE: This mode is used when taking single recordings at specific time points during a trial. 1. From EXIT/STBY mode, press MENU 2. Press READ until SPORTS MODE appears as a selection 3. Press F1 on the recorder keypad to enter sports mode 4. Enter the player number while holding the sensor at the participant s lower back or in front of their torso (e.g. if the sensor number assigned to the pill was 01, the player number is 01). This should produce a core temperature reading on the screen 4.4 Data handling and management In the case of this study which recorded data in sports mode, readings should be recorded during each trial on a trial sheet. Following each trial, the hard copy of the trial sheet should be stored in a secure location and a copy should be saved immediately after each test to a secure network storage location. 11 6 Appendices 6.1 Gastrointestinal Pill Participant Instruction Sheet Enclosed in the small packet you have received is a core body temperature sensor that looks a lot like a pill. When swallowed, this sensor provides an accurate and non-invasive way to measure core body temperature. At numerous points throughout testing, we will put a handheld data recorder near your body and it will wirelessly receive a signal from the sensor you have swallowed and provide a temperature reading from your body. The following instructions will step you through what to do with the sensor to ensure it is correctly ingested. 1) Take the core temperature sensor out of the small zip-lock bag that you receive it in 2) Remove the yellow paper that the sensor is wrapped in. You will also see a small magnet enclosed in the yellow paper. Dispose of the magnet and the yellow paper.
