16 19 Address Specific Needs of Elderly ED Patients Assessment, Dedicated Space, and Clear Pathway Improve Care Components of Geriatric Care in Leicester NHS Trust s ED Rapid Initial Patient Assessment Dedicated Bed Space in ED Comprehensive Geriatric Assessment 1 Post- Discharge Care Planning Patients undergo a quick initial assessment at the first point of triage, determining patient pathway Acute Frailty Unit (AFU) has 8-12 beds in on-site medical assessment unit reserved for elderly patients, depending upon demand Specific geriatric assessment is given to patients over the age of 65 in the AFU 2 to ensure appropriate treatment Based on CGA 3, discharge planning is designed to connect patients with appropriate community resources to prevent readmission Triage determines if a patient can be cared for in the community or needs more specialised care Trajectory assigns a patient pathway and clarifies their health and social care needs Transfer to the most appropriate care setting happens within 24 hours of arrival in ED or AFU 1) Comprehensive Geriatric Assessment is a test administered to determine the appropriate pathway for elderly patients. It assess their ability to walk, home support, and psychosocial issues. 2) Acute Frailty Unit. 3) Comprehensive Geriatric Assessment.
