3 Definition (International Skin Tear Advisory Panel : ISTAP) A skin tear is a wound caused by shear, friction and/or blunt force resulting in the separation of skin layers. A skin tear can be partial thickness (separation of the epidermis from the dermis) or full thickness (separation of both the epidermis and the dermis from the underlying structures Oct-18 3

The International Skin Tear Advisory Panel (ISTAP) provides a comprehensive suite of validated tools for the interdisciplinary team to guide the risk assessment, prevention, assessment and treatment of skin tears, even in the most complex situations.
