2 What is CHADIS? CHADIS is a web-based screening, diagnostic and management system that administers and analyzes pre-visit, online questionnaires completed by parents, teens or teachers and provides Clinicians with instant access to valuable clinical data and resources. Through its process, CHADIS improves the diagnosis and management of health, emotional and behavioral issues, helping Clinicians address parents concerns about their child s development while streamlining other routines of Pediatric care. CHADIS generates and supports: Online pre-visit questionnaires completed anywhere Internet access is available Scored results accessible in a user-friendly Electronic worksheet Risk scores and provisional diagnoses based on screened responses Clinical decision-support and management information Web-based, multiple language platforms Handouts and Community resource lists specific to child and family needs A local and national resources database of over 16,000 listings Data collection for quality improvement efforts
3 How does CHADIS work? It s easy 1. CHADIS delivers the doctor s pre-selected questionnaires and families complete them online at home or in the waiting room 4. CHADIS selects informational handouts and community resources specific to the child and family needs 2. CHADIS collects all the answers, and automatically scores and tabulates them in an user-friendly electronic worksheet 5. CHADIS captures additional Clinician comments and information during the office visit 3. CHADIS presents provisional diagnoses and links to decision-support information based on the responses 6. CHADIS stores all visit data, creating a full record that can be copied into EMRs for referrals, billing, positive screen registry and patient diagnoses tracking

TCH is a for-profit Limited Liability corporation incorporated for the specific purpose of promoting the usage of CHADIS in Pediatric care. Barbara Howard, MD President, Total Child Health Inc. Assistant Professor of Pediatrics, The Johns Hopkins University School of Medicine. Dr. Howard is a developmental-behavioral pediatrician, past president of the Society for Developmental and Behavioral Pediatrics and a contributing author for Bright Futures and DSM-PC. She has served on national committees of the American Academy of Pediatrics. Raymond Sturner, MD Executive Chairman, Total Child Health Inc. Associate Professor of Pediatrics, The Johns Hopkins University School of Medicine Dr. Sturner is a developmental-behavioral pediatrician, the former Director of the Developmental and Behavioral Pediatrics Fellowship Training Program at Johns Hopkins University. He was previously Director of the Child Development Unit at Duke University, and has been the principal investigator of some 60 research grant years.
