3 Summary - The Patient Treatment Register (PTR) The PTR is the first online database in Ireland with information on waiting times for in-patient and day case operations for public patients. This report includes the third phase of the implementation of the PTR and now represents for the first time a national register based on named patients. DEVELOPMENTS 1. Patients and GPs can now access waiting time information for surgical procedures by hospital nationally. 2. The NTPF can now use the PTR to contact named patients nationally on the register with an offer of treatment. 3. The NTPF can now access procedure wait times and volumes nationally to match patients with available beds. 4. Hospitals can now use the PTR to plan elective workload. 5. A comprehensive picture of hospital waiting lists is now available on-line nationally.

4 The Patient Treatment Register Report December Statistical Summary a) WAIT TIMES FOR OPERATIONS - For the Top 20 surgical operations, the average waiting times for adults are 2 5 months. - For 8 out of the top 10 surgical operations for children, the average waiting time is 2 5 months. b) VOLUMES SURGICAL - There are 15,096 adults awaiting surgical procedures 37% 3-6 months 31% 6-12 months 32% over 12 months - There are 2,300 children awaiting surgical procedures 35% 3-6 months 34% 6-12 months 31% over 12 months c) ANALYSIS SURGICAL VOLUMES OVER 12 MONTHS - Five hospitals account for over half of adults waiting over 12 months for a surgical operation. - Three hospitals account for three quarters of children waiting over 12 months for a surgical operation. d) OFFER OF TREATMENT TO PATIENTS ON THE PATIENT TREATMENT REGISTER - 11,379 who are waiting for surgical procedures have been written to with an offer of treatment. - 3,337 replied via the lo-call line - 8,042 did not respond to the offer

7 Introduction The Patient Treatment Register (PTR) is the first online database of public in-patient and day-case waiting lists in Ireland. It was developed with co-operation from the Department of Health and Children, the Health Services Executive and the public hospitals. The National Treatment Purchase Fund has statutory responsibility for the collation, management and publication of waiting list information. There are now 36 hospitals included on the register accounting for 100% of the patient population as measured under the old waiting list system. THE PTR HAS BEEN DELIVERED IN THREE PHASES - PHASE I accounted for 7 hospitals and 40% of the historical patient population. These hospitals have now been reporting since September Included in Phase I were Beaumont Hospital, Connolly Hospital Blanchardstown, Mater Misericordiae Hospital, St. James s Hospital, St. John s Hospital Limerick, St. Vincent s Hospital and Tallaght Hospital (AMNCH). PHASE II accounted for a further 12 hospitals and 74% of the historical patient population. These hospitals have been reporting to the register since April Included in Phase II were The Childrens Hospital, Temple Street, Cappagh National Orthopaedic Hospital, Our Lady s Hospital for Sick Children Crumlin, University College Hospital Galway, Merlin Park Hospital Galway, Mid-Western Regional Hospital, Dooradoyle, Mid-Western Regional Orthopaedic Hospital, Croom, Royal Victoria Eye and Ear, Cork University Hospital, Kerry General Hospital and Mercy University Hospital PHASE III now accounts for a further 17 hospitals and 100% of the historical patient population. These hospitals report to the register from December onwards. Included in Phase III are Cavan General Hospital, Our Lady s Hospital Navan, Our Lady of Lourdes Hospital Drogheda, Midland Regional Hospital at Mullingar, Midland Regional Hospital at Portlaoise, Midland Regional Hospital at Tullamore, Lourdes Orthopaedic Hospital Kilcreene, St. Lukes General Hospital, Kilkenny, South Tipperary General Hospital, Waterford Regional Hospital, Wexford General Hospital, Letterkenny General Hospital, Mid-Western Regional Hospital Nenagh, Portiuncula General Hospital, Roscommon County Hospital, Mallow General Hospital and Sligo General Hospital. Other hospitals including Louth County Hospital, Monaghan General Hospital, Naas and Mayo will be included in early Historically these hospitals had no patients on waiting lists. 9 Introduction continued THE PATIENT TREATMENT REGISTER PROVIDES: An accessible website link for patients (both adults and children), with detailed information on the top surgical procedures and waiting times for each; A GP website link on waiting times for over 7,500 procedures in individual hospitals in order to assist the referral process; A separate link for each hospital with detailed information on procedures, patient status and guidelines for using the Register; Direct interaction capability between the NTPF and the patient. Patients receive a letter and a PTR card from the NTPF so that they can access the information to opt for treatment under the Fund; A comprehensive picture of hospital waiting lists in Ireland, including waiting times, numbers and the status of patients, together with consistent national operating guidelines and definitions. HOW THE PATIENT TREATMENT REGISTER WORKS: 1. Waiting time analysis for different hospitals for the top 20 surgical procedures can be viewed online ( for the shortest waiting times. 2. The GP and patient can use this information to decide which hospital the patient should be referred to. 3. The patient attends their out-patient consultation appointment and a decision is made on the procedure required. 4. Once the patient decides they want to accept this procedure, the hospital places them on its internal waiting list system which is electronically transferred to the PTR. 5. If the patient is still on a waiting list after three months, the NTPF writes to eligible patients and offers them treatment under the Fund in a private hospital. 6. Upon receipt of such a letter the patient can opt for treatment under the NTPF or continue to wait in their original hospital. The PTR now provides wait time information for the most common surgical procedures and gives patient s options on their course of treatment. This can reduce the length of time that they have to wait for their procedure. Report on the Patient Treatment Register page 8

10 WEBSITE The PTR website includes - Wait time information in respect of the top surgical procedures for both adults and children. These wait times are displayed on an individual hospital basis nationally; General practitioners can now access wait times in respect of over 7,500 procedures through the ICD10am procedure classification code descriptor and get waiting times for each procedure on an individual hospital basis; Hospitals have new improved information to aid in managing waiting lists more efficiently; Information is updated for all hospitals on a monthly basis. Access to the PTR website is through or PTR GUIDELINES Supporting the Patient Treatment Register is a comprehensive set of guidelines. These have been agreed by a representative group from the hospitals, Department of Health and Children and the Health Service Executive. These are continuously reviewed by a new Guidelines group established in 2006 with operational representatives from each of the hospitals. In addition audit/ reviews have been introduced and are conducted with each hospital for verification of the data. The aims and objectives of the guidelines are as follows - To provide a practical and easy to follow guide for those charged with day-to-day administration of waiting lists; To ensure that all cases are placed on the waiting list and are tracked to closure be it treatment or validation; To ensure consistent reporting of waiting lists for patients, general practitioners, consultants and hospital/health care professionals; To ensure that best practices are adopted in the management of waiting lists and these are continuously reviewed; To ensure that the guidelines are applied consistently and nationally and that within each hospital a named executive is nominated in each hospital to have overall responsibility for waiting lists; To provide a mechanism where processes are reviewed implemented and change recommendations are made for enhanced wait time data management. 11 Introduction continued CONTACTING PATIENTS Patients initially reported in phase I and Phase II eligible for treatment under the National Treatment Purchase Fund (NTPF) have been written to with an offer of treatment. All additional patients will be written to in 2007 with an offer of treatment arranged by the NTPF. Results from Responses to date: 30/11/ ,379 patients have been written to with an offer of treatment. 8,042 did not respond to correspondence 3,337 replied via the lo-call line 2,656 were eligible for treatment 681 were referred back to the hospitals for further verification The Data Protection Commissioner in his Annual Report 2005 indicated that he was satisfied that section 2b of the Data Protection act allows the National Treatment Purchase Fund to collect information in respect of persons on waiting lists in order to manage and facilitate their treatment and that this was compliant with the Acts. 12 Glossary The Patient Treatment Register Report Provides statistics on the number of patients and wait times for patients who are actively waiting for surgical procedures on an in-patient and day-case basis. Actively Waiting Is defined as a patient who is available for treatment and does not yet have a scheduled date for their treatment. A Suspended Patient Is defined as one who is not currently ready (for clinical reasons) or not available (social or personal reasons) for their procedure. The Median The average used in calculating patient waiting times is the median, this is the measure applied by health systems internationally. ICD10AM Classification Procedures are designated by the ICD10AM classification, which is a standardised index for procedure level information. Medical Cases Are those who require hospital treatment but not a surgical procedure. Surgical Cases Are those who require hospital treatment for a surgical operation/procedure. Waiting Times Are measured from the date a patient is placed on a list for in-patient or day-case hospital treatment, surgical or medical. Waiting List A patient is defined as being on a waiting list if they are waiting over 3 months. Report on the Patient Treatment Register page 11
