2 WHY HEALTH BEHAVIOURS MATTER QUESTIONS: Inherited Genes Epigenetic Factors Q1. SOCIAL CONNECTION Secure relationships single most predictive variable in well being in late life. NOT AN ADD ON : WE ALL DO THIS WORK ALREADY As clinicians we already invite behavior change every day. We: Discuss patient s lifestyle, physical activity, social supports. Review community activities & resources. Offer reading for new learning.. But most of us lack a systematic approach. 4 FOH OFFICE GUIDE (LAMINATE) S.M.A.R.T GOAL SETTING Goal should be S.M.A.R.T.: Specific What concrete step will you take? Measurable How can success be measured? Action Oriented What action or behaviour is needed? Realistic How realistic is it to accomplish? Time limited When will it be done? (what time, day, how often) A SMART goal will set a patient up for success! NO GOAL IS TOO SMALL! STEP 2: SET A S.M.A.R.T. GOAL small GOALS CAN LEAD TO BIG HEALTH BENEFITS OVER TIME Examples of small smart goals: PHYSICAL: SOCIAL: Walking to the mailbox once a day Getting up during the ads in the 6 o clock news Calling a friend or family member once a week Setting up a coffee date once in the next 4 weeks Based on your questionnaire results, it looks like (being physically active/ socially connected/ brain challenge) is an area you feel has room for improvement. What do you think of setting one small goal in that area to work on for next time? COGNITIVE: Reading the paper 3 mornings a week Listening to a radio program twice a week 4

5 KEEP A RECORD! STEP 3: FOLLOW UP VISITS 2 AND 3 (ABOUT 4 WKS APART) VISIT 2: Check in and record degree of goal attainment, revise or set new SMART goal. Take a look at the Fountain of Health tools and try one out. VISIT 3: Check in on degree of goal attainment Repeat the Self Assessment Questionnaire to look for changes in self ratings and evaluate progress. BARRIERS: MINIMIZING SHAME AND GUILT Changing anything is hard. Sample Script: Needing to rework a goal, or change it up is absolutely normal and expectable! Connect with us at info@fountainofhealth.ca FOH FEASIBILITY PROJECT FOR HEALTHCARE PROVIDERS Clinician Tools Are: Quick and simple to use (about 10 minutes). CBT based (with SMART goal setting). Evidence based (with outcome measures). How To: STEP #1: Get a baseline STEP #2: Set a SMART goal, keep a record STEP #3: Follow up (over 2 visits) & evaluate impact YOUR COMMITMENT REFERENCES Target Population: At least 5 community dwelling outpatients. Over age of 40 years (no dementia, may have MCI). May be a family member of client who will benefit. 3 visits (~ 1 month apart) x 10 minute approximately. Can be done as part of usual health visit. 2 nd and 3 rd visits can be by phone if needed. Complete tools and provide feedback on using FoH clinical material.
