Capture of information on individual support provided to smokers attempting to quit. Signposting to other stop smoking services and specialist advice as required. Introduction All pharmacies are expected to provide essential and advanced services they are contracted to provide to all their patients. This enhanced service specification outlines the more specialised service to be provided when supporting smokers who are attempting to quit smoking and specifically capturing of information for the MK PCT. The specification of this service is designed to cover the enhanced aspects of these interventions which are beyond the scope of essential services. No part of the specification by commission, omission or implication defines or redefines essential or advanced services. This service is commissioned under paragraphs 4 and 5 of The Pharmaceutical Services (Advanced and Enhanced Services) (England) Directions This document constitutes the agreement between the pharmacy and the PCT in regard to this LES. Background Evidence shows that in the UK one person dies from a smoking-related disease every four minutes. The Department of Health estimates that care of people with smoking-related diseases costs the NHS 1.7 billion each year. In Milton Keynes around 21% of the adult population are smokers with a greater proportion coming from the more disadvantaged areas. The location of community pharmacies and the accessibility through the opening hours will assist the PCT to reach people in those areas for stop smoking interventions. In January 2010 the National Strategy for Tobacco Control indicated the aspiration to halve all smoking rates in all groups by 2020 and to develop a more radical and flexible approach to work within the community. There is a correlation between the proportion of smokers in a population and the level of deprivation. Evidence also shows a correlation between levels of deprivation and success in stopping smoking with individuals from areas of higher deprivation having lower levels of success. To account for these factors, which relate directly to the corporate aim to reduce inequalities in health, this LES will pay Pharmacies per successful quit on a progressive scale depending on IMD ranking (index of multiple deprivation). Pharmacies have been placed into one of three groups according to IMD ranking (see appendix 1 for pharmacy groupings and associated payments). The Aims of the Service To both opportunistically and in a targeted approach identify smokers and offer the opportunity of a 1:1 clinic.

2 To provide access to trained Advisors on a no appointment basis (immediate presentation) where feasible and appropriate, in disadvantaged areas in a setting other than a GP Practice. To deliver timely and appropriate advice to those who are not supported elsewhere. To accurately record and report stop smoking activity through the monitoring form within DoH timeframes. Contractors must agree to the following before a Pharmacy will be approved to deliver the service: (i) The contractor will ensure that all staff delivering smoking advice and support are trained to a minimum Level 2 standard by MK Stop Smoking Service and are able to provide the certificate for that accreditation. Contractors should ensure availability of accredited staff to attend update sessions and annual audits. (ii) The contractor will ensure that potential quitters have access to a 1:1 stop smoking clinic as soon as possible and within a week of referral at maximum. It is the responsibility of the contractor to develop capacity in the pharmacy to facilitate timely access to clinics (within a week of point of referral at maximum and where appropriate on immediate presentation). (iii) The contractor will provide a private consultation area that meets contractual requirements for Advanced Services. (iv) The contractor will report activity through the monitoring form in an accurate and timely method. Description of the service: Each pharmacy will be responsible for the recruitment of potential quitters. The Pharmacy will develop its own internal recruitment pathway for customers to receive support within their pharmacy. The PCT will provide promotional literature as required. The pharmacy will pro-actively offer brief advice on stopping smoking to customers who are purchasing NRT or who present with prescriptions for NRT. The customer will be encouraged to have a consultation with the accredited advisor who will help them to set a quit date and will complete a monitoring form. Customers will be offered a 30 minute assessment appointment and a minimum of four 15 minute follow up appointments on a weekly basis thereafter. The Advisor will assess what medication and psychological support is most suitable and will supply a Recommendation for Prescription letter to the GP, which will then be taken by the customer to the practice. The advisor will complete the monitoring form days after the quit date and send the top copy to the Tobacco Control Lead, retaining a photocopy within the pharmacy for audit purposes. Forms received after the quarterly DoH deadline will be ineligible for payment. The accredited advisor must endeavour to measure and record CO levels at each appointment. CO validation must be completed at 4 weeks post quit to confirm smoking status where feasible.

3 The accredited advisor will endeavour to follow up the customer 4 weeks after the quit date and record the outcome. If the follow up is not possible then 2 attempts should be made one of which should be days after quit date. This date is then entered into the 4 week follow up box and the form submitted with lost to follow up box ticked. The Pharmacy will signpost customers to the Specialist Service as required. The following customers should be referred to the Specialist Service for stop smoking support: pregnant ladies, the partners of pregnant ladies (only if they smoke), those under 18 years of age, those with mental health issues or learning difficulties, those who have had numerous supported quit attempts. Definitions A smoker is defined as anyone who smokes at least one cigarette, pipe, cigar etc per day. A successful four week quit is anyone who has completely abstained from smoking for the last two weeks of their supported quit attempt. Brief advice is the initial contact with a smoker lasting for 1-3 minutes to encourage and enable the potential quitter to make an appointment with a Level 2 Advisor. Level 1 Advice is the provision of generalised advice regarding smoking medication and symptoms and side effects etc and is principally for encouraging and signposting. Level 2 accreditation allows the provision of a specific tailored quit programme one to one for that individual. Level 3 accreditation is for a Specialist Stop Smoking Advisor whose sole job is the provision of more complex advice and support and group work. Payment for the Service The pharmacist will complete a monthly audit form and enter the claims on the Monthly Summary Claims Form. An initial supply of these forms will be provided by NHS Milton Keynes (an electronic version will be available on the pharmacy pages of the PCT website ), but the pharmacy is responsible for copying these for ongoing claims. The completed claim form will be submitted to Rosa Morris at NHS MK and NHS Northamptonshire Cluster 1 to be received by 4 th of the following month. The claim form may be submitted electronically to rosa.morris@northants.nhs.uk. The Pharmacy Team will process claims from approved service providers to PPD (Prescription Pricing Division) via SBS (NHS Shared Business Services). Payment will be included on contractors monthly statements from PPD. Payment will be included on contractors monthly statements from PPD identifiable as LS3. The Tobacco Control Lead will inform Pharmacy Team NHS Milton Keynes & Northamptonshire of any necessary payment adjustments or discrepancies (e.g. incomplete forms, late receipt etc) these adjustments will appear in the next PPD statement at end of quarter. MKPCT reserves the right to enter a pharmacy and obtain access to relevant records to undertake an audit of the enhanced service at any time. The objective of the audit would be to ensure that this contract provides value for money to the PCT. The audit would check the adequacy of controls, the integrity and reliability of information for billing in the area under review. This is to ensure that payment is only made for valid work completed. The scope of the audit does not include checks on the quality of activity. Pharmacies would be given one weeks notice of attendance and will be required to make the necessary records available. Each auditor would have appropriate authority from the PCT, treat any records viewed with confidentiality, and provide clear ID. 4 Variations to and termination of the agreement The PCT may vary the terms of this service specification or terminate this agreement with three months notice in writing to the contractor. The contractor may terminate this agreement with three months notice to the PCT. Signatures: Before payment will be made each contractor must sign the contracts portfolio (separate document). Two signed copies of the contracts portfolio must then be returned to Anne-Marie Frost Associate Director Directly Commissioned Services at NHS MK & Northamptonshire. The relevant PCT Manager will then countersign the agreement & return your copy back to the pharmacy.
