2 Compounding pharmacy definition Pharmacy that works with a variety of practitioners to create customized medication solutions for patients whose health care needs are not met by manufactured medications. Raw chemicals or manufactured products are either combined with other chemicals/products or are used alone. Combined with an appropriate base for delivery Transdermal or topical creams/gels/ointments/solutions Liquids for oral use Buccal or sublingual troches/lozenges Nasal irrigations or sprays Combined with a filler Capsules Topical powders 2

3 A compounding pharmacy is utilized in situation in which: The pharmaceutical industry has limited dosage strengths available One size does not fit all A medication that is commercially available is backordered or discontinued Backordered temporarily or indefinitely Can fill void until available Discontinued by manufacturer due to low profit or other economic considerations There are limited dosage forms Other dosage forms are needed for children, the elderly, or other special patient populations May be a population in which absorption, metabolism, or elimination is a factor A drug could be delivered through a different route with better results Ex: Promethazine Transdermal Gel- ease of administration in patient who is vomiting or has diarrhea as an alternative to oral or rectal route 3

4 Custom Rx Pharmacy Business model changed from retail pharmacy to compounding only pharmacy in 2000 Two locations in Wichita Jan Gerber, RPh, President PCAB (Pharmacy Compounding Accreditation Board) April 11th, 2011 PCAB Accreditation PCAB accreditation ensures that a compounding pharmacy as been validated independently and externally to make sure that it meets or exceeds nationally accepted quality assurance, quality control, and quality improvement standards. Pharmacy voluntarily participated in an off-site and on-site evaluation process Review of facility and equipment Review of standard operating procedures These are reviewed by the staff regularly and modified as needed for improvement in procedures Assessment of pharmacy s system for assuring, evaluating, and maintaining staff competency Review of records and procedures required to prepare quality compounded medications Review of the pharmacy s program for independent, third-party testing of compounded preparations Licensure review There are only four pharmacies in Kansas that are accredited Custom Rx are two of them and JCB Labs is the third 4
