15 Primary and Secondary Outcomes Primary Outcome COPD symptoms Secondary Outcomes COPD knowledge Use of COPD action plan Cigarette cessation Inhaler technique Physical activity Exacerbation rate Socioeconomic outcomes Instrument/variables to compare MH-COPD and UC-COPD CAT and MMRC score at the baseline Questionnaire on the COPD knowledge Questionnaire and data recorded in the app Questionnaire. Number of cigarettes recorded in the app. Questionnaire at baseline and six-month time points. 6-minute walking test, and questionnaire on daily physical activities Questionnaire and records from the electronic healthcare system in Queensland Health (6-month period prior to trial, 6-month period during, and 6-month period post) Hospital readmissions and visits of emergency department. 15
