1 SEOW Annual Report Presented to SPF PFS Management Report Timeframe: October 2015 September 2016 Alignment of DE SEOW with SAMHSA Identified Core Tasks The Delaware State Epidemiological Outcomes Workgroup (SEOW), formerly known as Delaware Drug and Alcohol Tracking Alliance (DDATA), collects, synthesizes and makes accessible information on Delaware substance use, its precursors, and its consequences to inform prevention policy, practice, and programming in the State. The SEOW was formed in 2007 with SAMSHA support, subsequently the funded through previous Strategic Prevention Framework State Incentive Grant (SPF SIG), and continues through funding from Strategic Prevention Framework Partnerships for Success Grant (SPF PFS). During the 2015/2016 year, the SEOW provided substance use and related data to communities and state agencies in order to support data based decision making. This report will outline some of those activities as well provide an overview of the major SEOW accomplishments and barriers encountered during this time period. According to SAMHSA, the State Epidemiological Outcomes Workgroups (SEOWs) are groups of data experts and prevention stakeholders responsible for bringing data on substance misuse and related behavioral problems to the forefront of the prevention planning process. To achieve their mission, SEOWs are charged with the following four core tasks 1 : 1. Identifying, analyzing, profiling, and sharing data from existing state and local sources 2. Creating data guided products that inform prevention planning and policies 3. Training communities in understanding, using, and presenting data in an effective manner 4. Building state and local level monitoring and surveillance systems The Delaware SEOW, during the report timeframe, completed or worked towards completing the four core tasks identified above. In response to the first core task, the DE SEOW created and disseminated the 2016 State Epidemiological Profile as well as the Substate Epidemiological Profile. Indeed, the DE SEOW has produced annual Epidemiological Profiles for almost a decade, since These profiles contain data on the consumption, context, and consequences of alcohol, tobacco, marijuana, and other illicit drugs. The 2016 Epidemiological Profile includes data on youth and adults, from a variety of data sources such as the National Survey on Drug Use and Health, the Delaware Behavior Risk Factory Surveillance Survey (Adults), Delaware School Survey, Monitoring the Future Study, The Youth Risk Behavior Survey, and the Youth Tobacco Survey. The 2016 Profile also includes a special section on the Reported Risk Behaviors for Gay, Lesbian, and Bisexual Youth a group identified by the SEOW as being at disproportionately high risk for substance use and other risk behaviors. In response to the second core SEOW task identified by SAMHSA (creating data guided products that inform planning and policies), the SEOW produced a plethora of products geared towards different 1 learning resources/data prevention planning seow 1

2 audiences. These include monthly DDATAGrams (one page sheets that focus on one data point) and Factsheets (customizable data sheet for use by community agencies). Two Gap Reports, which provide assessments on gaps in services and programs for Delaware residents across the lifespan, as well as gaps in research and data to inform these services, were created and disseminated to state agencies to inform prevention planning and policies. The topics of the Gap Reports for this year underscore the association between violence and trauma as a cross cutting issue related to substance use and other risk behaviors: Teen Dating Violence and Substance Use and Children Exposed to Domestic Violence. In addition to these regularly produced reports, the SEOW responds to data requests and inquiries from State agencies and community groups in order to inform prevention planning and policies. Throughout the year, the SEOW provided data to DSAMH, DPH, DOE, DPBHS, State Legislature, as well as community coalitions, schools, school districts, and colleges to inform decision making. The SEOW also collaborates with partners to create these products and provide the most pertinent information. For example, the SEOW collaborated with DPBHS and with the DCADV when creating the two Gap Reports. The SEOW also provides resource and other relevant information along with the data to strengthen prevention efforts, such as adding the crisis line information to recent factsheets. In regards to the third core task identified by SAMHSA (training communities in understanding, using, and presenting data in an effective manner) the SEOW conducted a data literacy training for community partners in 2016 as well as conducted data presentations to the Delaware State Legislature Kids Caucus, community agencies, and coalitions with local data (as available) throughout the state. The SEOW provided data to at least 25 community agencies and coalitions, in all three counties such as Open Door, Inc., the Kids Caucus, UD Sociology and Criminal Justice Department, UD Student Life, Western Sussex Summit for Delaware Educators, DPH, DEMCO, local middle schools, West End Neighborhood House, La Esperanza, LACC, DSU, Wesley College, NCPC, SN4C, Laurel Coalition, Delaware Prevention Coalition, etc. Additionally, the SEOW provided technical assistance to community groups in the form of one on one meetings, support, and phone calls to discuss data elements and how they can be used to inform programming, identify needs in the community, secure funding, leveraging data to create new partners and stakeholders, using data in norming campaigns and to change family/school/community policies, standards or norms. In addition, the SEOW provided data elements and technical assistance to community groups and to the State as they sought funding for substance use prevention efforts. The final core task identified by SAMHSA is building state and local level monitoring and surveillance systems. To achieve this task, the SEOW meets throughout the year to share reports on new data and new data resources, to discuss data related issues, such as identifying new data sources, discussion of emerging data trends, as well as making recommendations of SEOW products and tasks. During the contract year, the SEOW met four times 2. Additionally, the SEOW pushed out data (national, state, and substate) and related information, such as best practices, through the SEOW listserv, which has over 75 participants. The SEOW seeks to build on the progress made by the SEOW during the past year to further increase the accessibility of state and local monitoring and surveillance systems, so that community members, coalitions, and other interested stakeholders can enhance their data driven decision making, program implementation, and evaluation of prevention efforts. 2 meeting minutes were submitted to SPF Management as part of the contract deliverables 2

3 SAMHSA also notes that SEOWs are responsible for completing specific tasks and developing specific products related to the core tasks identified above in order to support implementation of each of the SPF s five steps, as can be seen in Table 1: SAMHSA Identified SPF Steps and SEOW Tasks Alignment with DE SEOW (Page 5). As noted in the fourth column of Table 1, the Delaware SEOW products and tasks are aligned with the SAMHSA SEOW recommended products and tasks. Identification of Priority Substances: In addition to the tasks outline above, the SEOW reviewed data, and, based on input from the SEOW Workgroups in June 2016, they recommended the priority substances of focus as Delaware makes data driven decisions for planning, policy, funding and programming around substance use. These substances were identified based on prevalence, increasing consumption patterns, and the consequences associated with use. The identified priorities are: 1. Opioids, including Heroin, Prescription Opioids, other street Opioids 2. Marijuana 3. Alcohol In addition to these three priority substance areas, the SEOW is also interested in contextual issues that need to be considered in relation to the priority substances. These include, but are not limited to, mental health, ACES (adverse childhood experiences), violence (community violence and exposure, incarceration, domestic and intimate partner violence, sexual violence). Particular subpopulations are also at a heightened risk for substance use and its related consequences. Because of that, subpopulations are one of the filters by which data can be sorted, in order to inform targeted intervention and programming. Subpopulations of potential interest are LGBQ (due to elevated risk), individuals with disabilities (due to elevated risk), and older individuals (due to an increasingly aging population and lack of data on the elderly). Based on the recommended priorities and contextual factors, the DE SEOW products will include these topics to keep Delaware informed about the consumption, intervening factors, and consequences associated with these priority substances. Major SEOW Barriers Although the SEOW has had many accomplishments since its inception and during this reporting period of October 2015 and September 2016, there were two major barriers encountered: establishing a new sustainable structure and filling the gaps in membership. A major task that took place during this reporting period was restructuring the SEOW in order to better achieve the mission and vision. In order to facilitate a smooth restructuring of the SEOW, the SEOW designated coordinators and DSAMH met to discuss changes in the SEOW so that it is more universal and sustainable. Part of this restructuring was to identify two separate subcommittees of the SEOW: data collection workgroup and data dissemination workgroup. Though challenging to agree on the elements of each group, identify the times these groups would meet to address, and ultimately decide on which topics the DE SEOW should prioritize, through an iterative process members of the subcommittees were ultimately able to prioritize the above noted substance use topics and cross cutting issues. Another barrier encountered by the SEOW was a gap in the membership of the SEOW, particularly medical and school personnel, such as doctors, nurses, school nurses, pharmacists, PTA, teachers, wellness centers, etc. Although the SEOW currently has about 35 members and strong representation from State and Community agencies (Please See Table 2 for a list of current SEOW member organizations), gaps still exist in membership and in level of participation. During the SEOW meetings, it 3

4 was discussed how the members would reach out through their networks to see if we could generate buy in from these groups, which will be especially important for the medical community membership given the growing emphasis on prescription drug misuse and abuse. It was also discussed how to tailor products to these groups to be most useful to them. Based on these conversations, the SEOW will be conducting an assessment to help determine what products should be geared to what audience. Additionally, the assessment will be used to identify potential new data stakeholders and include them in SEOW efforts. Major SEOW Accomplishments The SEOW has had a very busy year and, as can be seen in the SEOW Logic Model (Figure 1, Page 7). It is working its way towards the short and long term goals of the Delaware SEOW, sustainable institutionalized data surveillance system, increased knowledge of consumption and consequence data, increase in data driven decisions at the community and state level, and finally, the reduction in substance use, related risk behaviors and consequences. Although many of the accomplishments and activities have been noted throughout this report, the following is a recap of some of the major accomplishments and activities. Collaboration continues to be a strength of the SEOW. The SEOW met with different entities in Delaware to discuss data needs and resources, Delaware surveys regarding survey items, collection, and sustainability, and gaps in research and services. Some of these entities included the Division of Services for Aging and Persons with Disabilities, DPH, DSAMH, Advisory & Advocacy Council of the Department for Children, Youth, and Their Families, and Trauma Matters Delaware. The SEOW is participating in the Delaware Virtual Forum organized by Daniel Maas from HIDTA (Delaware is now included in the Philadelphia Camden Delaware federal High Intensity Drug Trafficking Area) and DSAMH. The SEOW collaborated with the DPH in their successful application to the CDC to establish a National Violent Death Reporting System (NVDRS) for Delaware, a group to which the SEOW will be both a provider to and receiver of data. The SEOW will be represented on the Delaware Violent Death Reporting System Advisory Board. Additionally the SEOW collaborated with DSAMH to provide data for the successful SPF Rx application. The SEOW team was appointed to the Advisory Committee for the DE SIM (State Innovation Model) evaluation that is being supported by the Delaware Center for Health Innovation, an initiative of the Governor and DHSS and supported by the federal CMMI. The SEOW continually assesses the magnitude of the substance abuse related problems (consumption/consequences) in Delaware. The SEOW, during October 2015 through September 2016, has conducted many activities assessing substance abuse related problems in Delaware including, but not limited to, mapping of SPF PFS hotspots related to alcohol and prescription drug past 30 day use, assessing risk perceptions, and emergency department visits with Delaware School Survey data and prescription monitoring program data for prescription drug use. The SEOW produced two gap reports, one on risk behaviors and children exposed to domestic violence and another on teen dating violence and its association with substance use. The SEOW reviewed data from the National Poison Data System on the rate of poisonings to prescription drugs and alcohol across the state of Delaware. Each month the SEOW created and disseminated a DDATAgram and Factsheet on a topic related to the substance use prevalence, patterns, consequences, and other related issues among middle and high school students in the state of Delaware. This information is disseminated through many channels, which includes twitter feed, website posts, s, and during meetings. 4

5 The SEOW proactively provides data and also responds to data requests from State and community agencies. Data sources used by the SEOW include the NSDUH, YTS, YRBS, DSS, CRBS, NPDS, BRFSS, TEDS, Overdose data, PMP, HDMP, DOE School Profiles, and more. The SEOW team provided data to the State about trauma indicators to be used in a state presentation as well as to DSAMH to support their federal core review and for inclusion in grant and funding applications. The SEOW collaborated with DSAMH and the CAPT to provide data for a Behavioral Health Disparities training, which were conducted in English and in Spanish. The SEOW also presented at the Delaware Prevention Summit on behavioral health disparities in Delaware Youth, using a variety of data sources such as the DSS, YRBS, CRBS, BRFS, and DOE. The SEOW provided other community organizations with data as well, such as DSS data and census data. Additionally, the SEOW held a training on data literacy, titled "How to Question Data: An Analytic Look at the Results of the 2015 Delaware Youth Risk Behavior Survey." This training presented highlights from the recently released findings of the YRBS administered in Delaware public high schools in the first half of 2015, focusing on findings relating to the use of tobacco, alcohol, marijuana, and illicit prescription drugs. Through interactive discussion, the SEOW shared with participants how to take a more analytic look at the data, examining sub groups, comparisons to larger groups, trends, and relationships among data points to tell a data story. Building upon these accomplishments, the SEOW will continuously reevaluate and refine data needs and seek relevant data sources, strengthen the membership network, and promote strategic dissemination of salient information vital to Delaware s prevention efforts in the coming year. 5
