4 Shared Family Care Project Healthcare for the Homless Preserve intact but at-risk family units by placing entire families in outof-home care Mentor child welfare familieis on appropriate child rearing and caretaking strategies and techniques Provide birth parents with educational and job training opportunities Train Shared Family Care Host Families in problem-solving, conflict resolution, fiscal management, and other skills Identify prospective system partners, e.g., homeless shelters, AODA treatment providers, etc. Design and develop procedures for intake/screening, and referrals for intervention/treatment Develop protocol for case management services Develop a cost effective substance abuse service specifically for the homeless Provide coordinated care with multiple agencies to address all issues leading to homelessness Conduct ethnically and culturally sensitive outreach efforts Reduced length of stay in foster care and quicker reunification of families Birth families will be less likely to have a repeat encounter with the child welfare system Recruitment and retention of SFCP families Decreased alcohol and substance use among homeless Facilitate referral of eligible homeless adults to programs Provide appropriate and coordinated aftercare planning to participants Dedicated case management to improve follow through on treatment referrals Reduced child welfare cases Increased parenting and other skills among adults of SFCP clients Increased community stability Increased exposure for UWM within Milwaukee community for multiple issues Increased child safety If program fulfills its goals, it will be replicated at other child welfare service sites Reduced morbidity and mortality within the homeless population Improved quality of life for segments of the community Reduced intoxication among homeless population

5 Comprehensive Screening Process for W2 Cocaine Families Project Develop system of screening by Financial Employment Planners (FEPs) and referral where necessary Develop screening instrument and interview protocol to screen for substance abuse, domestic violence, learning disabilities, traumatic brain injury and mental health Identify resources either within agencies or external for comprehensive assessment Identify and develop network of intervention/treatment resources Design field test instruments for understandability, reliability and validity Provide consultation with W-2 agencies for administration of screening instruments and writing of a training manual Evaluation of client/family outcomes Evaluation of system outcomes at United Community Center, the first residential treatment program for Latinas in WI.
