2 Surface Skin Inpection Keep Moving Incontinence Nutrition & Hydration What i a preure ulcer? Preure ulcer, alo known a bed ore, are area of damage to the kin and tiue below caued by itting or lying in one poition for too long. They can alo be caued by: ill-fitting footwear; liding down the bed; inappropriate wheel/arm chair and poor peronal hygiene. Who i at rik? Some people are alway at rik of preure ulcer. Everyone i at ome rik in hopital becaue you move around le. Thi could be due to tiredne, medication or pain for example. However you are more at rik if you: Have poor or retricted mobility for example taying in one poition in bed or a chair for a long period of time without moving. Currently have a preure ulcer or previouly had a preure ulcer. 2
3 Have a lo or lack of feeling or enation in your kin which may be due to dieae uch a multiple cleroi, diabete, troke, poor circulation, heart dieae, vacular or blood veel dieae. Are on trong pain killer uch a morphine or have an epidural in place. Have memory problem. Don t eat or drink enough. Are overweight or underweight. Have vulnerable kin, eg. dry, moit, paper thin, redne. Have a low mood or emotional tatu which can lead to inactivity. Skin inpection - get to know your preure area Thee are the area of the body where preure ulcer are mot likely to occur: Lying on your back Lying on your ide Sitting up in bed Sitting in a chair or wheelchair 3
4 Surface I am pleaed I told the nure that my father had a pecial mattre at home becaue they got him one traight away Some people are alway at rik of developing a preure ulcer but everyone i at ome rik whilt in hopital becaue you move around le. Preure relieving equipment may help reduce thi rik. Preure relieving equipment, uch a a mattre or chair cuhion, may be provided for you to help reduce the amount of preure on your body. Pleae inform the nuring taff if the equipment you have been provided with i not working or i uncomfortable. Alo let taff know if you normally ue a pecial cuhion or mattre at home. Preure ulcer can alo occur under medical device ued to treat your condition. For example an oxygen mak, tubing, catheter, cat or neck collar. It i important that you inform the nuring taff if you have a poorly fitting or painful medical device. 4
5 Keep Moving If you have been identified a being at rik of developing a preure ulcer thi may be becaue of reduced mobility. Regular movement i key to prevention, whether you re in bed or at out in a chair. By moving ide to ide and lifting the bottom, preure i relieved for a hort time. Good poture help to pread the weight of the peron and reduce preure on bony area. Lift the bottom regularly if you are afe to do o to relieve preure. Ue your call bell if you need aitance with thi. 6 Incontinence Too much moiture can be damaging to the kin, for example urine, faece or weat. Thee can all increae the effect of preure, hear (liding) and friction, and make kin more fragile. Pleae tell the nuring taff if you are having incontinence problem a there may be aid we can provide which can help to manage thi, for example pad. Keep kin clean, dry and moituried. Good peronal hygiene i important, however oap i very drying. A foam cleaner can help to keep kin clean and a barrier product protect and hydrate the kin. Enure your kin i patted dry not rubbed after cleaning. I would be o upet if one my patient developed a preure ulcer, I make ure everyone looking after them know if omeone i at rik, all taff have a role to play. 6

You hould be drinking at leat 6 to 8 drink a day to help keep your kin healthy. Thi can include a mix of the following: water, juice, quah, tea, coffee or milky drink. Weight lo can increae your rik of kin breakdown. Being underweight or overweight can alo increae your rik. Staff will monitor your weight when you firt come into hopital and weekly. If taff have any concern or if you already have a preure ulcer, you may be referred to the Dietitian. See the leaflet Eating well for wound healing and preventing kin breakdown. Smoking damage blood veel and affect overall kin health and circulation. If your kin i at rik it may be adviable to top moking. 7
8 Remember, it important to look after your kin by: Moving regularly. Eating and drinking well. Telling taff if you have any pain or dicomfort. There i a leaflet available Staying Active In Hopital. Staff are able to refer you to the Tiue Viability Service, Phyiotherapit, Dietitian, Occupational Therapit and Back Care Advior if your need are complex or if your condition i deteriorating.
