The principle is based on 2 points: the nutational movement and infrasound. > Nutation is a precession movement that can be observed in a gyratory motion or the motion of a spinning top. The hand piece (lipomatic /evamatic ) has a pneumatic motor that guides the end of the cannula in a nutational movement. This specific movement is used for uniform fat infiltration and aspiration. Nutation creates an emulsion that gently breaks the fat cells apart while preserving all tissues (no tearing). After this step, the emulsified fat can be gently aspirated, with fewer bleeding, oedema and ecchymosis. Aspiration is thus very selective. > Infrasound (low frequency: less than 25 Hz) is generated by the movements of the cannula and acts as an anaesthetic on the treated part, making the treatment more comfortable during and after the procedure. Unlike other techniques, the effects of the infrasound are transmitted way beyond the cannula. His laboratory examined the stem cells collected with the lipofilling n.l.f System and the lipomatic /evamatic The study focused on the viability, the phenotype and the differentiation of the mesenchymal stem cells at the adipose tissue level harvested with the lipomatic / evamatic, and on the autologous transplant of the adipose tissue with the lipofilling n.l.f. System. The study, conducted by Chantal Lechanteur (R&D Manager of the laboratory), concerns the mesenchymal stem cells (MSC) coming from the adipose tissue harvested with the lipomatic / evamatic and stored in the n.l.f System 1 kit. The samples were sent to the gene and cell therapy service of the CHULiege. The stem cells derived from the adipose tissue were first isolated and then cultured so they can be cultivated and expanded via successive trypsinizations until fourth passage. The obtained MSC were then differentiated into adipocytes and into osteoblasts by cultivating them for 3 weeks in a specific environment. The lipomatic / evamatic enables thus to preserve the stem cells of the harvested adipose tissue and to successfully obtain MSC from all tested fractions. ABBOUD Specialist in plastic, reconstructive and aesthetic surgery approved the evasp and the lipomatic /evamatic Immediate largevolume grafting of autologous fat to the breast following implant removal. To optimize autologous breast augmentation, a simple and reproducible surgical approach that maximizes the volume of fat transferred to the breast while minimizing the number of sessions and the operating time is needed. The authors describe a novel approach for largevolume fat grafting to the expanded skin and subcutaneous tissue of the breast immediately after explantation, exchanging the volume provided by the implants with transplanted fat in a single session. For this study, eighty patients (160 breasts) undergoing explantation and autologous fat transfer were evaluated in a prospective study. Fat was harvested with the lipomatic powerassisted liposuction machine (lipomatic /evamatic,evasp ) and was injected with simultaneous vibration and tunnelization of the recipient site by means of the same machine. OTHER PRODUCTS OF THIS RANGE Our advice: use our n.l.f. kit presented on page 5 to collect and reinject the fat. Changes in breast volume were measured in terms of bra cup size, and patients were monitored by mammography and ultrasonography. Patient satisfaction was assessed with a questionnaire administered 6 months postoperatively. Results obtained showed that injected fat volumes ranged from 300 to 600 ml per breast. Operating times ranged from 45 to 90 minutes. For all patients, one injection session was sufficient to replace the volume of the previous implant. Patients were monitored for an average of 2 years, and complications included cyst formation in 9 of 160 breasts (5.6%) and infection in 2 breasts (1.25%). In conclusion, powerassisted transfer of autologous fat to the breast improves the ability of the recipient site to receive the graft in a single session. This approach is suitable for patients who desire a naturalappearing breast that is similar in volume to their previous implant.
