2 Washington State Office of Rural Health (SORH) The Washington State Department of Health (DOH), Office of Community Health Systems, Rural Health Section, supports health systems planning and development in Washington s rural and underserved communities to improve health and healthcare across the life span. 10 professionals and Executive Director Manage four federal grants from Health Resources and Services Administration (HRSA) and over 60 contracts and procured services. SORH works to provide accurate information from data, sustain high quality rural health systems, facilitate a robust workforce for rural & underserved communities, and instigate improved public policies for rural health & access for the underserved.

3 Population Health Project Purpose The purpose is to fund and support the role of Critical Access Hospitals (CAHs) in the planning and development of population health improvement projects.

4 Population Health Project Strategies To develop skills and knowledge that build capacity for the new incentive for care and payment. Provide an opportunity to assess the determinants of health in their community based on data review & community assessments. Select a project based on data review that can improve population health. Strategize and collaborative with community partners and local health jurisdictions to ensure the success of the project. Develop measureable goals & targets. Report out at the end of the year the overall impact on their community showing both success and lessons learned.

5 Population Health Projects Overview DOH Population Health projects are supported by the Flex grant and based on available yearly funding. Year 1: The invitation to submit a proposal was originally sent out to each CAH in Project year ended 8/31/17. 9 hospitals responded and all project proposals were accepted. $5000 grants for projects. Year 2: Automatic continuation contracts of $8000 were sent out based on the availability of continued funding from HRSA.

6 Overview (continued) Year 3 Begins 9/1/18 Based on available funding we are predicting that our Population Health project initiative will continue in the new grant fiscal year. An announcement will be sent out in September opening up the opportunity to more CAHs. New CAHs will be able to submit a proposal to start a population health project. The current 9 CAHs will have the opportunity to continue their work.
