3 New Mexico Rehabilitation Center - ESTABLISHED IN 1968
4 NMRC Service Provision Medical Rehab - Offers the most intensive level of one : one inpatient rehabilitation therapy available for patients in the region who have had strokes, traumatic brain/head injuries, spinal cord injuries, MVA/Motorcycle accidents, Hip and knee replacements, and other ortho impairments that affect mobility and daily functional status. Medical Detox Provides inpatient medical detoxification for patients who are interested in detoxing from drugs and/or alcohol in a safe hospital setting. Intensive Outpatient Program Serves those with primary substance abuse issues. An option for individuals who require structure and support to achieve and sustain recovery while living in their community. Social Rehab - Anticipated start date July Clients will receive individualized treatment plan and educational programs that assist in increasing coping skills and problem-solving, decreasing co-dependent behaviors, increasing relaxation skills, and team building.

Ranked one of the 6 th highest counties in NM for Heart Disease. (290.4 Chaves County, NM State per 100,000) Diabetes 48.8 per 100,000. Higher than NM state (26.2) and U.S. (21.3) Falls/Injury 65 years and older increased 27% 2013 & Eastern New Mexico Medical Center Lovelace Regional Medical Center Artesia General Hospital Presbyterian Hospitals University Medical Center Lubbock TX Covenant Medical Center Lubbock TX University of New Mexico
6 Medical Rehab Highlights Higher facility case number of Stroke patients than national (33.4% NMRC, National 23.4%) Uniform Data Systems. 100% compliant with Center for Medicare and Medicaid (CMS) in admission criteria for patients to the unit. Rank 75 th percentile in high performing rehab centers in the nation (Functional Improvement Measure) 85% of patients are discharged home with high functional status improvement Occupational Therapy, Physical Therapy, Speech Therapy avg. combined 1300 patient therapy hours per quarter. 91% compliance (CMS) with therapy minutes per patient. 7 NMRC Role in Substance Abuse Recovery Alcohol-Related Injury Death ( ) in Chaves County was 28.4 per 100,000 and statewide rate 28.0 Chronic liver disease leading cause. Drug Overdose Death ( ) in Chaves County was 22.1 per 100,000 with statewide rate of Overdose in prescription versus illicit drugs were equal. Opioid Overdose ER Visits ( ) in Chaves County was 53.8 per 100,000 and statewide rate of Adult mental health depression is higher than statewide rate in Chaves County. (14.5 versus statewide rate of 10.3) Substance Abuse societal problems include crime, violence, homelessness, blood borne diseases, and other chronic disease impact community. 8 NMRC Substance Abuse Programming Highlights Intensive Outpatient Program (IOP) Adopted the evidence based Integrated dual diagnosis treatment model (IDDT) and recovery life skills. Addresses substance abuse and mental illness with interventions. Utilize SMART treatment plan (Specific, Measurable, Attainable, Results, Timely and Biopsychosocial that is comprehensive to physical, mental, spiritual, social, and emotional needs. 16 Week Program, 9 hours per week group therapy. Recovery Life Skills psycho-education, motivational interviewing, substance abuse counseling, and cognitive behavioral. Group Therapy development of individual goals, decision making, guidance, instruction and learning, flexibility, group members facilitate or take roles in group, and identify development. Medical Detox 81% of patients report to after care appointments 80% of patients maintain sobriety after 7 days post discharge 50% of patients transition to a lower level of care Intensive Outpatient Program Completed or graduated successfully: 95 Serviced a total of 398 clients (May 2017) Avg Qtrly IOP Unit Hours: S.
