1 Age Range 7 3% 12 5% 26 92% Transitional Age Youth, Adult, Older Adult, 6+ Graph A The Telecare and Orange (TAO) program is a Full Service Partnership (FSP) adult program designed to help awaken individual s hopes and dreams for a better quality of life. As Graph A illustrates, the TAO program primarily serves the Adult population (Ages 26-59) in Orange County. However, based on the clients individual needs TAO has the ability to serve members who meet the Transitional Age Youth (16-25) age criteria as long as they are 18 years of age or members who meet the Older Adult age criteria (6+). As part of TAO s commitment to a recovery culture, we strive to make our members feel welcomed from the minute they walk into the program. By disseminating welcoming surveys to members, as well as staff members and analyzing the results we are able to ensure that TAO generates a welcoming atmosphere. TAO is designed to use a multidisciplinary team, which includes a psychiatrist, a nurse, a Masters prepared Team Lead, and Personal Service Coordinators with experience in both drug and alcohol and mental health to provide services regardless of gender. The program is designed to provide culturally competent services to adults with serious mental illness. One of the ways to ensure this is through trainings in cultural competency and the promoting of a recovery culture throughout the program. The staff at TAO have the capability to provide cultural competent services in several languages so that the program is able to meet the cultural and linguistic needs of the members. Currently, TAO can provide services in Orange County s four threshold languages: English, Spanish, Vietnamese, and Farsi. In addition, TAO staff can also provide services in Korean and Chinese. 4 Living Arrangements Graph D Transitional Housing Permanent Housing Other Emergency Housing Graph D illustrates TAO s member living arrangements. One of the areas of focus at TAO is the reduction of member homeless days. Clients living situations get addressed immediately as they are enrolled into the program. In order to help restore each individual member s dignity, self worth, and self respect TAO features a full-time Housing Specialist on staff to ensure that progression is made to resolve the living situation needs from day one. In some cases the specialist addresses this from the first initial contact with any potential new members. The Housing Specialist also works diligently with our existing members and The Personal Service Coordinators to solve any housing issues that may arise as a team. TAO s Housing Specialist is always looking for innovative ways to solve any housing issues. The 28 members that are categorized as Other represent members that are missing, incarcerated, hospitalized, or homeless. For the month of June 21, TAO had 41 homeless days out of a total 6,68 member days or roughly 6%. Page 4 of 1

5 Incarcerations Graph E Incarcerated Days Number of Consumers Incarcerated Number of Episodes of Incarceration As Graph E illustrates, there were 71 incarcerated days out of a possible 6,68 member days in the month of June 21, which is roughly 1%. Due in part to the whatever it takes approach in the services that are provided by the staff at TAO, 98% of the members did not spend any days incarcerated during this period. The decrease of incarcerated days and incarceration episodes is one of the performance indicators that TAO strives to reduce. The Personal Service Coordinators at TAO provide support and education in the form of intensive case management which includes 24 hour a day, 7 days a week availability. This is to ensure that TAO s members know that where to turn to for support in times of need regardless of the day and time. Page 5 of 1

6 Psychiatric Hospitalizations Hopitalized Days Number of Consumers Hospitalized Number of Episodes that Led to Hospitalizations Graph F As Graph F illustrates, for the month of June 21 out of a possible 6,68 member days, TAO had 153 hospitalized days or approximately 2%, this total reflects a decrease of 86 hospitalized days from the previous analyzed month of March 21. To help increase member resiliency in their recovery journey the TAO program provides medication support and education. Medication is a part of some, but not all, members recovery. TAO features psychiatrists and nurses on staff that provide support and knowledge to it s members when medication issues arise. The psychiatrists and nurses are an integral part of the program; TAO strives to reduce the number of member psychiatric hospitalizations through adequate medical care and the connections that are developed as a result of the continuous engagement spear headed by The Personal Service Coordinators. 7 Employment Graph G Paid Employment Non-paid (Volunteer) Work Experience Other Gainful / Employment Activity Graph G illustrates the number of currently enrolled members engaged in employment activities. The total number of 45 members engaged in employment activities represents an increase of 8 members from the previous analyzed month of March 21. The TAO program features a full-time Employment Specialist on staff to provide assistance, education, and support to its members. The Employment Specialist works diligently along side The Personal Service Coordinators to find opportunities to engage members in employment. Our consumer designed and operated Wellness Recovery Center features computers with internet access, printing capabilities, and employment groups/seminars headed by our employment specialist that can be accessed by members that choose employment as one of the goals in their recovery journey. The Personal Service Coordinators at TAO work along side the Employment Specialist to help interested members become engaged in gainful employment activities. Page 7 of 1

8 Education Graph H Community College/4 Year College Technical/Vocational School Graph H illustrates the number of members currently engaged in Educational activities. The TAO program also provides help with linkage to vocational and educational services and support to those members that choose education as a part of their journey to recovery. Education plays a vital part in awakening some of the program members hopes and dreams for the future as well as being a meaningful activity of their daily lives. The Personal Service Coordinators and the Educational/Vocational Specialist work with those members interested in becoming engaged in educational activities to accomplish this goal. Page 8 of 1

The psychiatrists that are on staff at TAO diagnose and treat each member with therapy or medication when needed in accordance to each individual s diagnosis and needs. The Personal Service Coordinators continuously engage the program s members to empower them to be able to deal with the rigors that come with living with a mental illness. TAO s staff commitment and focus on the recovery process allows for an improvement in member s quality of life. About 9% of the program s services are provided outside of the office so that members may be engaged out in the community instead of a clinical setting. Page 9 of 1

10 Secondary Diagnoses % % Both Severe Mental Illness and Chronic Substance Abuse Severe Mental Illness Chronic Alcohol or Other Drug Abuse Graph J Graph J illustrates the number of members with a dual diagnosis. At TAO there are a higher number of Dually Diagnosed members compared to those that only have a Severe Mental Illness diagnosis. The TAO program was designed to be capable of providing services to consumers that were dually diagnosed with both a severe mental illness and chronic substance abuse. For this reason TAO features a full-time Substance Abuse Specialist that is able to provide services and support to members that are dually diagnosed. The Substance Abuse Specialist support and education, empowers members to learn how to make choices that lead to self control and self responsibility. Substance abuse group sessions are also held at TAO to help educate and provide knowledge to members dealing with a dual diagnosis of mental illness and chronic substance abuse. The Wellness Recovery Center at TAO also provides dual recovery services that may help in relapse prevention. All clinical staff at TAO are trained in being able to recognize and work with dually diagnosed members. Page 1 of 1
