1 CONTRACTOR NEWS CHOOSE PHARMACY UPDATE April 2018 Contractors will be aware from previous communications and the Contractor events held last year that the roll out of the Choose Pharmacy IT platform commenced in October ROLL-OUT The Choose Pharmacy Platform has now ben rolled out to 507 or 70% of contractors across Wales, which surpasses the 50% that was predicted at this stage of the rollout. All LHBs in Wales now have some pharmacies attached to the platform and able to offer the Common Ailments Service. Roll-out paused during the month of March but re-commenced in April It is expected that all pharmacies will have received roll-out by September September to November 2018 will be spent mopping-up and re-visiting pharmacies that have not received the platform (e.g. due to IT issues). REPORTING ISSUES CPW and LHBs have reported a high number of contractor issues in using the platform particularly in relation to speed/ loading and logging on issues during the initial phases of the rollout, however NWIS have no calls relating to this issue since the beginning of Contractors are requested to log all issues as soon as possible with Choose Pharmacy either by phone on or by to primarycare.servicedesk@wales.nhs.uk. The service desk is open 8am to 6pm Monday to Friday and 9am-1pm on Saturdays. Contractors are asked to report unresolved issues to CPW so that we can escalate the issue directly with NWIS (please provide the reference number when raising it with CPW). Printing remains the biggest issue that contractors are reporting to CPW in relation to the use of the platform. The number of clicks-to-print has been reduced from 9 to 8 and the prospective update of electronic GP letters (date TBC) will eliminate this problem. In the meantime contractors who are struggling to use the platform because of difficulties in printing are

2 asked to contact their local NWIS specialist or the helpdesk and they can be talked through a work-around. NWIS have resolved an issue in which some contractors were not receiving payment for DMR s entered through Choose Pharmacy. Some 60 DMR s contained a glitch which meant that they fell into the wrong data table when submitted, resulting in no payment. These pharmacies have been contacted and payment has now been received. However approximately 70 DMR s haven t been closed meaning that they may have the same glitch when submitted. Contractors are therefore encouraged to close any DMR claims that are still open. If the Choose Pharmacy platform is unavailable for whatever reason all of the services can still be provided and details should be recorded on paper versions of forms and then recorded on the platform as soon as possible once it is available. CHOOSE PHARMACY CLINICAL REFERENCE GROUP NWIS reported that there had been 131 reported instances of inaccurate submissions that needed to be amended in the previous 3 months. These were primarily due to inaccurate entry of data relating to flu vaccinations. It is intended that there will be an audit process to allow easier viewing of submitted data in future versions of the choose pharmacy platform ACCREDITATION Contractors are asked to encourage all pharmacists (including locums) working in their pharmacies to become accredited to access the Choose Pharmacy platform and to provide all services. Currently there are 3 services available through the platform: Common Ailments Service, Discharge Medicines Review service and Flu Vaccination however a further 2 services will be added to the platform over the next few months, increasing the need to ensure that all pharmacists have a NADEX log on to be able to access the platform as NECAF will be switched off for claiming once claiming is available through the platform. The following information shows the process to obtain a NADEX account and accreditation in the relevant services:

Details to be sent to the pharmacist s home address Pharmacist to complete NADEX account sign up authorisation via the Choose Pharmacy application

4 The Choose Pharmacy platform provides (or will shortly provide) access to the following advanced and enhanced services where they are commissioned by Local Health Boards: Common Ailments Service (currently available) Discharge Medicines Review Service (currently available) Seasonal Influenza Vaccination Service (Available in Flu commissioned pharmacies for 2018/19 flu season) Emergency Contraception Service (Available late 2018 after reviewing clinical guidelines) Emergency Medicines Supply Service (Available May 2018 after pilot completion) Care Home DMR (TBC) Pharmacists wishing to become accredited to provide these services should follow the agreed All Wales accreditation process for these services in addition to completing the Choose Pharmacy (NADEX) user account process: Service Requirements Common Ailments Service Complete WCPPE Choose Pharmacy IG accreditation Discharge Medicines review Service Valid DBS Check Sign PGDs Complete MUR accreditation Emergency Medicines Supply Service No additional requirements all pharmacists will have access to this module where the service is commissioned. Emergency Contraception Service Seasonal Influenza Vaccination Service Complete WCPPE accreditation Valid DBS Check Sign PGDs Complete WCPPE accreditation Sign PGDs All WCPPE accreditation modules now sit within the NESA hub

5 DELIVERY OF COMMON AILMENTS SCHEME (CAS) Currently 82% of pharmacies are providing at least 1 CAS consultation per month and 450 pharmacies have at least 1 patient registered. LHBs have raised concerns in relation to the number of contractors who have gone live with the Common Ailments Service but are not actively providing the service. NWIS have agreed to send LHB s monthly reports on CAS activity so that they can monitor this going forward. CAS will form part of the November 2018 Public Health Campaign. It is expected from this the number of patient self-referrals may increase. Pharmacists therefore should be proficient in the delivery of CAS so as not to damage patient confidence in the service. Contractors are reminded that they will not receive any service fees for patients registered on the service during any month where there is no activity. Contractors are advised that once they have gone live with CAS and pharmacists are comfortable in using the service that they should begin discussions with their local GP service to discuss referrals, feedback from pharmacies already actively working with GP practices is that it is useful to start working on referring a small number of conditions initially. Support materials for contractors to discuss CAS with GP practices can be found here. Contractors should also ensure that once the local GP practice is aware of the service that they inform them at the earliest opportunity if they are unable to provide the service on any particular day due to any reason, as unavailability of the service for a patient will reduce both GP and patient confidence in the service. Feedback from optometrists during the pathfinder phase of the CAS service was exceptionally positive and Optometry Wales has asked CPW to encourage contractors to let their local optometrist know once they have gone live. The service fits very well with the Welsh Eyecare Service (WECS) and patients can be referred easily between the two. The majority of optometrists provide WECS, but you can confirm availability on COMMON AILMENTS SERVICE FORMULARY REVIEW The All Wales Common Ailments Formulary has been finalised and will be released on the 1 st May Two new products have been added and PGD s for these will be sent out in due course by LHB s. Pharmacists are encouraged to read, sign and return these to their Local Health Board

6 delegate as soon as possible. Pharmacists are reminded that they will not legally be able to supply these items without signing the appropriate PGD s. DISCHARGE MEDICINES REVIEW SERVICE With the roll-out of Choose Pharmacy, hospitals have the opportunity to send electronic discharge letters directly to pharmacies to support the DMR service. This however can only be done by hospital wards that have gone live with MTeD (Medicines Transcribing and e-discharge). MTeD allows the sending of edals (electronic Discharge Advice Letter) between hospitals and community pharmacies. Contractors are reminded that they can pre-register patients for the DMR service on Choose Pharmacy to ensure that they will then automatically receive edals from hospitals on their discharge. Pharmacists may wish to gain consent to pre-register patients on Choose Pharmacy as part of the MUR process. EMERGENCY MEDICINES SUPPLY (EMS) SERVICE PILOT EMS service is currently being piloted in 11 pharmacies in Wales who have access to the summary Welsh GP Record. Each pharmacy on the pilot has committed to accessing the record for each supply (even though in some instances there may have been no need) so that NWIS could have thorough and appropriate feedback. NWIS have been in weekly contact with pilot sites and to date all feedback has been very positive, particularly on being able to access summary care record. 39 consultations were done in Feb from all 11 pharmacies. The pilot is on track for completion, and the rollout to all pharmacies with the Choose Pharmacy platform is expected in May FLU VACCINATION SERVICE The 2017/18 flu season highlighted an issue with the accuracy of the record that some pharmacists were submitting in relation to flu vaccination. For example, in some cases the incorrect vaccine or injection site was selected. The Choose pharmacy record is not just a claim form but is also designed to be an electronic patient record that will become more complete as additional services are added to the platform. It is also intended that in the near future the platform will be able to send information directly to the patient s GP record.
