1 Introduction to Intrauterine Insemination (IUI) Service Assisted Reproductive Technology Unit The Chinese University of Hong Kong Prince of Wales Hospital 2017
2 Treatment Procedures IUI involves six main stages: Counselling Natural Cycle Monitoring Ovarian Stimulation Monitoring Oocyte Maturation Semen Collection Luteal Phase Support
3 Counselling Purpose Understand the causes of infertility Treatment plan and procedures Contents Implication of the procedures on couple and their families Financial implication Alternative options other than IUI treatment Possibility of treatment failure
4 Natural Cycle Monitoring Call on period Day 1 (fresh bleeding with normal flow) Cut off time ~ 4:00pm Daily monitoring of blood from ~ Day 8-10 onwards to check for ovulation Sometimes a transvaginal ultrasound scan may be needed to track the development of follicles The test results will determine the best time for IUI
5 Ovarian Stimulation Call on period Day 1 (fresh bleeding with normal flow) Cut off time ~ 4:00pm Attend IVF clinic for ultrasound scan and blood tests on period Day 2 or 3 Require around 5-10 days of injection (may choose either highly purified gonadotrophin injection or intramuscular injection) stimulate the ovaries to produce 1-2 mature follicles If there are more than three mature follicles, the treatment will be abandoned May self inject highly purified gonadotrophin so as to reduce the number of visits
6 Monitoring Oocyte Maturation Regular ultrasound scanning to check follicle development Ensure the hormone level continues to increase and reach acceptable level through blood taking Target: 1-2 mature follicles
7 When follicles mature Night Injection (HCG trigger) 34 to 36 hours after injection Inject Sperm into uterus Intercourse at night
8 Semen Collection No ejaculation of sperm for 2-5 days before semen collection Take a shower, change clothes & underwear before coming to the clinic Husband produces a sperm sample by masturbation Sperm will be washed and the good quality one will be separated out and inject into uterus May consider sperm banking beforehand if husband is not in HK or has history of difficulty in producing a sperm sample

10 Luteal Phase Support HCG Injection Progesterone Suppositories Blood Test for Pregnancy Positive Success Negative Failure
11 Complications Ovarian Hyperstimulation Syndrome (OHSS) Polycystic ovary Abdominal bloating & pain Electrolyte imbalance Nausea/ vomiting/ shortness of breath Decrease urine output usually occurs during luteal phase support
12 Complications Pelvic Infection Seldom occurs Ectopic Pregnancy Multiple Pregnancy Especially for woman with more than one mature follicle
13 Medication Side Effects Breast tenderness, mild abdominal bloating and increase in vaginal discharge May experience redness, swelling and pain at the injection site after intramuscular injection
14 Treatment Information Doctor reviews medical history and prescribes medication accordingly (gonadotrophin injection) Allow to choose the type of gonadotrophin injection Additional charge incurred
15 Points to Note Keep relax Avoid taking Chinese herbal medication Balanced diet & normal daily activities No vigorous exercises Normal sexual activity Abstinence for 2-3 days before semen collection
