Designed in collaboration with Avon Schools, Hendricks Regional Health, R.E. Sutton and Anthem. Brings together primary care physicians and medical specialists from Hendricks, American Health Network, St. Vincent Health and others to create the most comprehensive access to care possible. 3
4 TIERED NETWORK You will have no co-pay when you or your family members visit any primary care doctor within the network. This includes: family physicians, internists, pediatricians and OB/GYNs. For those managing diabetes, this benefit also applies to visits with your endocrinologist. The Hendricks Tiered Network premium is five percent less than the traditional plan offered. 4
