4 Part 2 Additions 4 mg Apo-Montelukast montelukast 5 mg (a) Indicated as adjunctive therapy for asthma in cases where: (i) maximum doses inhaled corticosteroids have not effectively controlled symptoms, or (ii) evidence of serious adverse effects associated with corticosteroids exists, eg. adrenal suppression, increased lung infections; (b) Indicated as first line therapy for exercise induced asthma. Part 3 Additions The following products will be considered for Pharmacare reimbursement upon an individual prescriber/patient request basis Aclasta zoledronic acid 5 mg/100 ml Injection NVT For female patients with post-menopausal osteoporosis (PMO) at high risk for fracture and satisfy at least two of the following three criteria: (i) Age > 75 years; (ii) A prior fragility fracture; (iii) A bone mineral density (BMD) T-score -2.5; OR Female patients with PMO with a serious intolerance to oral bisphosphonates or for whom oral bisphosphonates are contraindicated. 3 of 18
5 Actemra (New Indication) tocilizumab 80 mg/4 ml 200 mg/10 ml 400 mg/20 ml Injection For the treatment of active systemic juvenile idiopathic arthritis (sjia) in patients 2 years of age and older who: (i) have responded inadequately to previous therapy with one or more non steroidal anti-inflammatory drugs; and (ii) who have responded inadequately to previous therapy with one or more systemic corticosteroids. HLR Apo-Riluzole riluzole 50 mg APX For the treatment of Amyotrophic Lateral Sclerosis (ALS) Edurant rilpivirine 25 mg JAN For the treatment of HIV-1 infected treatment-naïve patients Gilenya fingolimod 0.5 mg Capsule NVT For the treatment of patients with Relapsing Remitting Multiple Sclerosis (RRMS) who meet all the following criteria: (i) Failure to respond to full and adequate courses of at least one interferon OR glatiramer acetate; OR documented intolerance to both therapies; (ii) One or more clinically disabling relapses in the previous year; (iii) Significant increase in T2 lesion load compared with that from a previous MRI scan or at least one gadolinium-enhancing lesion; (iv) Requested and followed by a neurologist experienced in the management of RRMS; and (v) Recent Expanded Disability Status Scale (EDSS) score. Exclusion Criteria: (i) In combination neither with other disease modifying therapies (e.g. Avonex, Betaseron, Copaxone, Rebif, Extavia, Tysabri) nor in combination with Fampyra. (ii) In patients with an EDSS > 5.5 (iii) In patients who have had a heart attack or stroke in the last six months, or in patients with a history of sick sinus syndrome, atrioventricular block, significant QT prolongation, bradycardia, ischemic heart disease, or congestive heart failure; (iv) In patient less than 18 years of age; and In patients with a needle phobia or preference for oral therapy over injections in patients without clinical contraindication to interferon or glatiramer therapy Jamp-Lactulose lactulose 667 mg/ml Oral Solution JPC For the treatment of portal systemic encephalopathy. 4 of 18

One combination therapy of DMARDs must also be tried. Initial application information should include information on disease activity such as the number of tender joints, swollen joints, erythrocyte sedimentation rate and C-reactive protein value. Ankylosing Spondylitis: For the treatment of patients with active ankylosing spondylitis who have failed to respond to an adequate trial of at least three different nonsteroidal anti-inflammatory drugs (NSAIDs) and, in patients with peripheral joint involvement, have failed to respond to methotrexate or sulfasalazine. Psoriatic Arthritis: For the treatment of patients over 18 years of age who have active psoriatic arthritis who have failed treatment with at least 3 DMARD therapies, one of which is methotrexate and/or leflunomide unless intolerance or contraindications to these agents is documented. One combination therapy of DMARDs must also have been tried. Initial application information should include information on disease activity such as the number of tender joints, swollen joints, erythrocyte sedimentation rate and C-reactive protein value. Request for coverage must be made by a medical specialist in rheumatology. 5 of 18
7 mg Toviaz fesoterodine Tablet PFI 8 mg For patients with an overactive bladder (OAB) with who cannot tolerate or have insufficient response to oxybutynin Tysabri (Revised Criteria) natalizumab 300 mg/15 ml Injection BIG For the treatment of Relapsing-Remitting Multiple Sclerosis (RRMS) who meet all the following criteria: The patient s physician is a neurologist experienced in the management of Relapsing-Remitting Multiple Sclerosis (RMSS); AND the patient: (i) Has a current EDSS less than or equal to 5.0; AND (ii) Has failed to respond to a full and adequate course (i.e. at least six months) of at least one modifying therapy OR has contraindications/intolerance to at least two disease modifying therapies; AND (iii) Has had one of the following types of relapses in the past year: o The occurrence of one relapse with partial recovery during the past year and has at least one gadolium-enhancing lesion on brain MRI, or significant increase in T2 lesion load compared to a previous MRI; OR o The occurrence of two or more relapses with partial recovery during the past year; or o The occurrence of two or more relapse with complete recovery during the past year, and has at least one gadolium-enhancing lesion on brain MRI, or significant increase in T2 lesion load compared to a previous MRI Votrient pazopanib 200 mg Tablet GSK For patients with advanced or metastatic clear cell renal carcinoma who, based on the mutual assessment of the treating physician and the patient, are unable to tolerate ongoing use of an effective dose of sunitinib.
