1 INFORMATION ONLY Changes to Requesting HLA/HPA Selected Apheresis Platelets Customer Letter # Head Office / Siège social Dear Customer, We are writing to inform you of changes to the Request for HLA/HPA Selected Apheresis Platelets form and the information recorded on the Cytapheresis tag attached to the HLA/HPA Apheresis Platelet product selected. The changes will be implemented on Revisions were made to standardize the process for requesting and tagging HLA/HPA selected Apheresis Platelets. The Canadian Blood Services department that your facility submits the Request for HLA/HPA Selected Apheresis Platelets, to will remain the same. The Request for HLA/HPA Selected Apheresis Platelets form has been divided into Sections. Important Information: Section I Patient Information: To be completed by requesting facility Section II Order Information: To be completed by requesting facility o Lab Reports are required. Transcribed results will not be accepted. o If the Prioritize TRALI Risk is indicated as YES the report generated to identify matching donors will exclude ALL female donors not just those with a history of pregnancy. o A Proposed transfusion schedule and estimated quantity should be provided in Section II if known. Section III is for Canadian Blood Services use only. Cytapheresis Tag HLA/HPA selected will be recorded beside Product type Name of Patient HLA/HPA Apheresis Platelet Selected for Name of requesting Hospital A Canadian Blood Services Site may also be recorded in some instances to ensure product is issued to the correct facility If the Donor of the HLA/HPA Apheresis Platelet was a female with history of pregnancy the statement For Intended Recipient Use Only. Female Donor with History of Pregnancy will appear on the tag. Examples of the Request for HLA/HPA Apheresis Platelets form and Cytapheresis Tag are attached and will be available in the hospital form section on
