3 The WHO Strategic Plan for rational introduction of new TB drugs and regimens in countries Describes key elements of a process aimed at: - producing policy recommendations for the treatment of TB (all forms), according to progress made in the development of new drugs or combinations of drugs, and - assisting countries in the implementation of these recommendations

6 Interim policy guidance on the use of bedaquiline "Bedaquiline may be added to a WHO-recommended regimen in adult patients with pulmonary MDR-TB, under five specific conditions" "conditional recommendation, very low confidence in estimates of effect" / _eng.pdf?ua=1. WHO June 2013
7 1. Proper patient selection Adult (>=18yrs) with pulmonary disease Special caution: HIV+, 65yrs+ (very limited data) Use in children, pregnancy not advised (no data) Use in bacteriologically confirmed extra-pulmonary MDR-TB possible based on extrapolation from pulmonary data
8 2. Patient informed consent required Patient information: novel nature of the drug, benefits and potential harms, reasons for inclusion Documented informed consent process, including signed patient consent Applies to all situations where bedaquiline is used, including compassionate use
9 3. Treatment under close monitoring Sound treatment and management protocols, clear patient eligibility criteria, defined roles and responsibilities of all professionals involved: prospective data capture for effectiveness and safety preferably, approval by national ethics body preferably, independent oversight and monitoring (eg. national MDR-TB advisory group)

Treatment design based on WHO recommendations Premise for WHO-recommended regimen: Z + at least 4 drugs considered to be effective Test for in vitro resistance (NB: shortcomings of conventional DST) Bedaquiline may be indicated if regimen is not feasible due to: additioinal resistance to fluoroquinolones or injectable known adverse drug reactions, poor tolerance, or contraindication Potential role in XDR-TB (Group 5 drugs) Dose and duration to be strictly observed Not to be added alone to a failing regimen!

Active pharmacovigilance Special measures in place to ensure early detection and proper management of adverse drug reactions and DDIs Very limited data available on DDI with ARVs PLHIV who will be receiving bedaquiline as part of MDR-TB treatment should have ART regimens designed in close consultation with HIV specialists Caution with co-morbidities (diabetes, cardiac disease, etc.) Mandatory monitoring required: Cardiac dysrhythmia, QT prolongation Liver dysfunction, renal impairment, other Caution: potential additive effects of drugs causing QT prolongation (eg. moxifloxacin, clofazimine)

Active pharmacovigilance (contd) 3 methods of Pharmacovigilance (PV) Spontaneous reporting Targeted spontaneous reporting (TSR) Active PV: "Cohort Event Monitoring" CEM: The most comprehensive method, similar in design and management to an epidemiological cohort study with prospective baseline and periodic measurement of patient parameters

14 Introduction in countries o Country preparedness: background information on Health system and NTP infrastructure, and on TB epidemiological data ("know your epidemics") o Country support to enable access to new drugs Strengthened capacity for diagnosis (incl. drug resistance), treatment monitoring & pharmacovigilance Sustained system for supply of QA drugs Discuss control mechanisms/regulations to prevent irresponsible use of drugs, particularly in the private sector Focus on pilot sites for initial deployment of new drugs with harmonised methods and surveillance Community/patients' representatives contribution Strong collaboration between key stakeholders Evidence for scale-up
15 Work with early implementing countries A group of countries have expressed interest in working with WHO for monitored introduction of bedaquiline in programme conditions, following WHO recommendations Vietnam, Philippines, Indonesia, Kazakhstan. Initial workshop involving all key stakeholders (NTP, MoH, NRA, NPV, etc.) and TA bodies/donors (GF, USAID, B&MGF, KNCV, etc..) o Outline of National Implementation Plan o Establishment of national framework o Identification of pilot sites o Determination of target cohort o Laboratory aspects o Monitoring including recording and reporting o Establishment of active PV in conjunction with key stakeholders o Discussion with NRAs on import/regulatory aspects/drug procurement o Timeline of activities
16 Key messages Key aspects for WHO/GTB are : to engage with and support national authorities and stakeholders early in the preparation of policies for introduction of new TB medicines at programmatic level (including quality, procurement aspects, etc.); to ensure that new TB medicines/regimens are introduced in an optimal way to protect patients from misuse and prevent emergence of resistance; to ensure that introduction of new medicines follows policy recommendations and appropriate plans are made to ensure feasibility and inform policy-making.
