From medical model to social approach for inclusion of Children with Disability - Introduction of the International Classification of Functioning, Disability and Health (ICF- CY) and experience of implementation in the European Union Prof. Dr. Manfred Pretis ICF-CY MedUse has been funded with support from the European Commission. This publication reflects the views only of the author, and the Commission cannot be held responsible for any use which may be made of the information contained therein
Bio-psycho-social model of ICF-(CY) Disease/health problem (ICD) Body functions And structures Activities Particpat ion Environment material social behavioural Personal factors Age, Sex Motivation Life style
What does this model mean in concrete (example G81.1) Levels of abstraction (1) From narrative description towards ICF-CY as common language (Pretis, 2016)
Levels of abstraction (2) From narrative description towards common ICF-CY as common language (Pretis, 2016)
Levels of abstraction (3) Using a ICF-CY as common language WHO Qualifiers:.0 = no problem.1 = mild problem.2 = moderate problem.3 = severe problem +3 = facilitator
Data about recent usage of ICF-CY in Social Pediatric Care Germany A recent empirical study in DE (Nordhausen 2015) pointed out that SPZs in DE (n=93) asseses themselves as ready to use ICF-CY. However concrete tools and training is missing.
How is the ICF-CY used in clinical practice? The German Association of MDs in Social Pediatric Care ( recommends ICF- CY - Because of multidimensional diagnostic representation -Taylor made treatment planning -Documentation of treatment -Resource-orientation
How do we use ICF-CY in Social Pediatric Care? - Structuring anamnestic information (see also the work of Martinuzzi) or description of technical support (Rehakind, 2015) - Using Core Sets (Rowland et al., 2012; Castro & Pinto, 2012) - Defining areas of intervention (Kraus de Camargo/Simon 2015) - Planning, documentation and evaluation of interventions (Pretis 2014) - Epidemiological-statistical analysis (Castro et al., 2012, Pretis 2016b)
Structuring anamnestic data Tick relevant ( ) Postpartal situation Without complications o With complication o Artificial respiration necessary b449 o Coronar disease b429 o Enteral nutrition (PEG) b510 o ( ) o Intermediate diseases Asthma s430 o Seizures s110 o ( ) o Child takes medication e115 o Child obtains special financial allowances due to disability e199 o Vacinations e355 o Activities Your likes to talk d350 o Preferred play d9200 o Attends kindergarten d815 o
Assessing devices and adjuvants BEB Needs and technical aid assessment based on ICF-CY Rehakind (
Usage of Core-set A core set represents a selected number of ICF-CY items dedicated to describe The peculiar life situation of a person with disability, mostly with a specific diagnosis e.g. Core sets for children with ASD (Castro & Pinto 2012) or Rowland et al (2012) for Children with need of augmentative or alternative communication)
Electronic tools coding relevant information of the patients based on ICF-CY
Assessment using WHO qualifiers (Graph. Describes our example Marija, 4;6 with G81.1) Intervention- Goals
Pre/Post comparisions
Statistical analysis Pretis (2016b)
Challenge of ICF-CY Describe the situation of a child with disability within a holitic transdisciplinary way (this means increasing complexity towards VALIDITY. Use the language of ICF-CY and reduce the complexity of codes etc...) Not to become a new secret language language for parents. We will need both: the narrative description of the child and family and the abstract common ICF-CY language.

Goal of ICF-CY MedUse ( Meduse) What is the goal of ICF-CY MedUse: Create electronic tools which a)increase the usage of ICF-CY in transdisciplinary social pediatric care a)to promote and develop comparable training curricula for professionals from diverse sectors (MD, psychologists, nurses, patronage nurses, therapists, educators, parents..)
