2 Purpose To facilitate high quality translational research in childhood leukaemia: clinical and basic science Centralised specialist national bank for childhood leukaemia Safe repository for samples located at UK Biocentre Collects samples for banking from all children with leukaemia, regardless of clinical trial participation Collects samples for ALLR3 trial and other relapse patients Collects samples for Asparaginase, Dasatinib and Genomics (formerly known as cytogenetics) sub-studies of ALL2011
3 Consenting for CellBank Separate consent form Range of patient and parent information leaflets Consent for CellBank is independent of trial consent
4 Collection of samples for CellBank Additional samples of bone marrow, peripheral blood and CSF taken at trial time points Excess samples from MRD testing also banked Samples for Asparaginase and genomics sub-studies of ALL2011 collected by CellBank and distributed to relevant laboratories. Sample handling will be provided for IntReALL trial Samples not required for these studies are banked (provided consent has been given)

10 Release of samples for research Applicants make enquiries via website to determine if suitable samples are held in the Bank and may also contact the CellBank manager Completed applications submitted to CellBank by Independent peer review by CellBank Application Review (CBAR) panel (rapid response to applicants, no set meeting dates) Release of samples subject to completion of signed Material Transfer Agreement and any conditions imposed by CBAR. Annual reports submitted to Steering Committee.
