1 21st Century CARE General Assistance Program Details 21st Century Cancer Assistance, Research and Education (C.A.R.E.) provides assistance, educational programs and screenings, support groups, and supports cancer research. The services offered to patients if eligibility requirements are met are: - Cancer Education - Financial Assistance Program: Assistance for incidental expenses related to transportation to and from treatments, follow-up visits related to cancer-care, childcare during treatment, temporary housing due to geographical distance from the treatment center, medical supplies and much more. - Patient Programs: Patient resource centers and support groups to individuals in need. - Cancer Screenings: Cancer screenings are only available in the Southwest Florida area to those needing financial assistance with a colonoscopy screening. Must complete an application to determine eligibility and that can be found on their website In order to guarantee the patient receive the assistance they need without delay, the first step must be a doctor referral. Applications will be accepted from cancer patients that receive treatment in the following states: AL, AZ, CA, FL, IN, KY, MA, MD, MI, NC, NJ, NV, NY, RI, SC, WA and WV. Website: Comments: Key word: gas card Additional Details Primary Phone: Secondary Phone: (239) info@21stcenturycare.org Service Details Southwest Florida - Colonoscopy Screening Eligibility for a colorectal cancer screening evaluation: - Screenings may be done by CT colonography, colonoscopy, or other approved methods - The patient must be 50 years old or older, unless they are at an increased risk for colorectal cancer (ie. by having a family history of colorectal cancer, Lynch syndrome, inflammatory bowel disease and certain other disorders) - Patient must be no older than 75 years - Patient must be asymptomatic - Patient must not have had a colonoscopy within the past 5 years - Patients at high risk may be under 50 years old and will be individually reviewed for eligibility in accordance with expert guidelines. Eligibility for a colorectal cancer surveillance evaluation: Patients who have been successfully diagnosed and treated for colorectal cancer are at in increased risk for developing recurrent disease and/or a second cancer. These patients are followed by a surveillance regimen and

2 screening guidelines no longer apply. All surveillance evaluations should be performed with colonoscopy (not CT colonography or other means). - A colonoscopy may be done one year following completion of cancer treatment - If normal, a follow up test may be done in 3 years - If the second colonoscopy is normal, then additional surveillance colonoscopies may be done every five years - If at any time the surveillance colonoscopy is abnormal, then the patient falls out of the surveillance category and should seek medical help for their condition.
