1 21 st November 2016 Working Group on Cancer Services A meeting of the Working Group on Cancer Services was held on 3 rd November 2016 at 11am, The Royal College of Pathologists, 4th Floor, 21 Prescot Street, London, E1 8BB Minutes Dr Rachael Liebmann Registrar Present: In attendance: Dr Brian Rous (BR) Dr Naveena Singh (NS) Dr Paul Barrett (PB) Dr Roger Hunt (RH) Dr Murali Varma (MV) Dr Jon Oxley (JO) Dr Usha Chandran (UC) Andrew Murphy(AM) Maria Marrero (MM) Heidi Rogers (HR) In the Chair BDIAP representative ACP representative COSD representative Senior CE Co-ordinator Temporary CE Administrator BR welcomed two new members of the WGCS, Dr Jon Oxley and Dr Usha Chandran (representing the ACP), to their first meeting. Declaration of conflict of interest No declarations of interest were made. WG.09/16 Minutes of the meeting held on 9 June 2016 A minor typographical error in the meeting notes was identified and required amendment. WG.10/16 Matters arising from the meeting of 9 June 2016 i) WCGS membership and terms of reference MM reported that BR and RH had completed their three year term of office. Both agreed to extend their term of office for a further two years. ii) Registration document of cancer cases when equivocal coding/terminology is used BR reported that the paper (Update on categorisation of pathology reports to clarify equivocal terminology) that was circulated for a second time to the WGCS could potentially be a useful tool in clarifying uncertainty for clinicians and cancer registries. There was discussion as to how useful it would be as it might just highlight differences CEff V1 Draft
2 in confidence levels between individual pathologists. An alternative approach would be to try and influence how pathologists use particular words to ensure a uniformity in written reports. Action MV to write some guidance on how to interpret the uncertainty and certainty of histopathology specimens. iii) Set up notification list of datasets for software companies & Twitter feed communication to inform IT suppliers of release of new dataset/tp. MM reported that an advert was put on the College website for software companies to join the mailing list. To date we have only received one request from a software company to join the list. We are also tweeting notifications of all published cancer datasets/tissue pathways with a hashtag that software companies will be able to follow to keep up to date, alongside the ing system. The WGCS agreed that a new time frame should be added on the front page of new or full reviewed dataset indicating that we expect IT suppliers to implement changes within 3 months after publication. Action: MM to notify registered software companies when a new dataset is published. MM to add in each published dataset document implement changes within 3 months after publication. iv) National Symposium on datasets/tissue pathways 2017 The WGCS agreed that the one-day symposium should be held on 30 March Following the previous WCGS meeting the revised programme was redistributed and minor amendments were made. v) Changes to College consultation proforma and feedback MM reported that some minor changes were made (suggested by MV) to the College consultation feedback proforma. The next step is to develop an online consultation response system that will substitute for the current external consultation process and feedback form. The WGCS also discussed the introduction of a new pre-consultation stage for cancer datasets and tissue pathways, for documents that are going through full review. It was agreed that this will give College members the chance to identify areas they think need to be focused on or to provide new evidence that may be relevant for inclusion in the update. The authors will be asked (when a document is due for full review) if they would like to inform the members to provide pre-consultation comments. Action: MM to ask authors in the full review letters if they would like to inform the members to provide pre-consultation comments. MM to add information with an on the website for members to respond to pre-consultations. vi) Survey of histopathology members on datasets/tissue pathways BR agreed to write and circulate draft questions to produce a survey for authors to provide feedback on how they view the cancer datasets and tissue pathway process and to identify areas where it can be improved. 3 Action: BR to write the survey questions and then send them out to the WGCS for discussion. vii) NIFTP the UK Endocrine Pathology Society Interim Position Statement A statement regarding NIFTP is now available on the College website: pdf.html. WG.11/16 COSD report AM reported that the new version of COSD (v.7) had been signed off by NHS Digital, for implementation from April COSD was to be revised bi-annually going forward. It was noted that SNOMED CT will be the official coding system. After publication of v.7 there will be a 4-week consultation from 25 November and from then onwards it will be revised bi-annually. AM agreed to send the revision to the WCGS before the deadline in January WG.12/16 Report on ICCR activity BR reported that the Head and Neck datasets and the Liver dataset are currently being worked on by the authors. The Cervix dataset has now been completed and will be available for consultation shortly. Work on the Brain dataset will commence shortly. WG.13/16 Progress on datasets We will be consulting lead authors of cancer datasets with regards to adopting TNM8 when it is published later this month. It is unlikely that national systems will be able to adapt to data collection for TNM8 from January, so the likely recommendation will be that we will collect TNM8 data from 1 July We are therefore likely to recommend that for national data collection users should use current staging systems until 30 June 2017, but that for clinical purposes it may be useful to cite both older staging systems and TNM8 in reports between 1 January 2017 and 30 June Action: BR to write something for the newsletter in January 2017, to inform all members of that they should use current staging system until June The WGCS discussed the process for consulting the Lay Governance Group (LGG) and they agreed that the LGG could do their consultation at the same time that we do our open consultation on the College website. Action: MM to check if this is possible and change the LGG consultation process. a. Dataset revisions Soft tissue sarcoma - currently in open consultation on the College website. Primary bone tumours currently need to find a new lead author. Action: MM to write to Dr David Mangham (next abridge review) to ask for a new lead author. CEff V1 Draft
4 Adult adrenal gland full revision was due in January Action: PB to contact lead author to find out when revision will be submitted. Conjunctival melanoma full revision was due in October Action: BR to contact the lead author to find out when revision will be submitted. Neuroendocrine tumours of the GI full revision was due in September 2015 and it will require a new lead author. Action: BR to ask Dr Roger Feakins if he could recommend a new lead author. Oesophageal and gastric carcinoma - with authors awaiting publication of TNM8. Colorectal full revision due in July 2017 after TNM8 has been published. Liver authors are waiting on publication of the ICCR dataset before starting a review. Carcinomas in the pancreas, ampulla of vater and common bile duct - open for consultation on website. Gastrointestinal stromal tumour authors are working on the draft. Action: MM to contact lead author to find out when revision will be submitted. Ovary with authors revising the dataset. Action: BR to contact authors to find out when revision will be submitted. Cervix work has started. Dr Raji Ganesan and Dr Naveena Singh are the new lead authors. Vulval revision is almost complete. BR sent comments to lead author in October Endometrium - with LGG until November Uterine sarcomas currently under abridged consultation. Head and Neck series is waiting for the publication of the ICCR dataset. Another coauthor will need to be identified. Lung cancer - was published in September Thymic epithelial tumours was published in September Mesothelioma revision was due in April Action: BR to contact lead author. Peripheral neuroblastic tumours revision was due in December Need a new coauthor. Action: BR to send a reminder to the lead author. Skin series - waiting for the BAD audit review. CEff V1 Draft
5 Penis waiting for AJCC and UICC TNM8. It will be discussed at the national penile meeting in January 2017 to see if a revision is appropriate. Adult kidney draft in place. Action: JO to contact lead author. Urinary collecting system full review was due in April Action : MM to send full review letter to authors after TNM8 in December Testis review due shortly. Action: MM to write to lead author after TNM8 has been published. Prostate published in June b. New datasets Breast cancer non-operative - was published in June Anal cancer Draft ready to be sent to CE. Action: JO to send reminder to lead author. Appendiceal tumours draft is progressing. Action: JO to send reminder to lead author. Small intestine still waiting on confirmation of lead authors. WGCS wanted to move this as an appendix for the next revision of colorectal. Action: BR to contact Dr Roger Feakins and suggest that small intestine might be included in the next colorectal dataset Cutaneous lymphomas- draft is progressing. Action: MM to send reminder to authors to find out when the draft will be submitted by authors. CUP - with WGCS following review. Action: MM to collate all comments and send them to PB for discussion with the authors. WG.14/16 Progress on tissue pathways a. Tissue pathway revisions Breast full revision due since November Action: BR to Professor Ian Ellis again to decide if an update is needed. CEff V1 Draft
6 Cardiovascular review has been underway since March Action: MM to send a reminder to the lead author. Cytology due to be revised. Action: MM to send a reminder to the lead author. Endocrine: full review due since January Placenta full review due since September Action: MM to send a reminder to the lead authors. Lymph node, spleen and bone marrow is currently with the LGG for review. Inflammatory and non-neoplastic dermatoses with authors following open consultation. Urology MV reported that the document is ready to be submitted to the CE. WG.15/16 Any other business i) Standard dataset and tissue pathway titles MM presented paper 4 - a list of standard titles and text to use in datasets/tissue pathways- to the WGCS to decide on which terminology should be used in College documents. Agreed: MM to update the standard text in all new datasets/tissue pathways. ii) NICE Accreditation programme MM reported that the NICE Accreditation programme will no longer accept and process new applications from April The arrangements for renewal of currently accredited processes will continue as they are now until about approximately November 2016, when NICE will be issuing a revised lighter touch process which does not utilise a Committee for consideration and recommendation. The current committee will cease operating in March The programme is only closing to new applications; this will not affect our current, approved process for production of datasets and tissue pathways. iii) Problems with current RCPath and ICCR datasets MV presented a paper 4 outlining current issues with the RCPath and ICCR datasets. It was agreed that both datasets and tissue pathways undergoing a full review will have a new pre-consultation stage process. Members will be able to suggest key areas that the authors need to focus on in the update. This will be optional for authors who will be asked about this change in letters advising them that an update is due. WG.16/16 Date and time of next meeting The next meeting was planned to take place on 8 th June 2017 with a starting time of 11.00am. CEff V1 Draft
