3 Preventive Care at Sanford Women s Routine checkups and screenings can help you avoid serious health problems and allow you and your doctor to work as a team to manage your overall health. At Sanford Women s, we are here to support your personal health care goals and provide you with preventive services designed specifically for you. What is preventive care? Preventive care is a type of care that adopts preventive measures and health screenings such as routine physicals, immunizations, diagnostic lab and x-ray tests, pap smears, mammograms, and other early detection testing which is often covered in a health plan. It includes programs or services that can help prevent disease and diagnose a problem early. During your preventive visit, your provider will determine which tests or screens are right for you based on your age, past medical history and current health. Clinic services: Routine physical examinations Immunizations Cervical cancer screening What is the difference between preventive care and diagnostic care? Preventive care refers to services that can help you stay healthy and identify problems early. These services are called preventive because they can help prevent serious health problems. Diagnostic services start when you already have signs of a health problem; therefore, your provider may order tests to further diagnose your condition. What is not considered preventive care? Medical treatment for specific health conditions, ongoing care, lab or other tests necessary to manage or treat a medical issue or health condition are considered diagnostic care or treatment, not preventive care. Your annual preventive exam at Sanford Women s Our team of physicians, certified nurse midwives and certified nurse practitioners are specialists in gynecology and obstetrics providing preventive care designed specifically for you. Each preventive exam, while tailored to you and your health needs, is conducted in keeping with the recommended preventive screening and exam guidelines of the American College of Obstetrics and Gynecology. For women who are otherwise healthy, your annual preventive exam may alternate between your OB/GYN physician and a certified nurse practitioner or certified nurse midwife every other year. 4 Recommended Screenings Use this information to help you stay healthy. Learn which screening tests you need and when to get them, which medicines may prevent diseases, and steps you can take for good health. Get the screenings you need Screenings are tests that look for diseases before you have symptoms. Blood pressure checks and mammograms are examples of screenings. You can get some screenings, such as blood pressure readings, in your providers office. Others, such as mammograms, need special equipment, so you may need to go to a different office. After a screening test, ask when you will see the results and who to talk to about them. Breast cancer. Talk with your health care team about whether you need a mammogram. When to start screening can depend on your family history and other risk factors. How often to screen can depend on your age. BRCA 1 and 2 genes. If you have a family member with breast, ovarian or peritoneal cancer, talk with your provider about your family history. Women with a strong family history of certain cancers may benefit from genetic counseling and BRCA genetic testing. Cervical cancer. Starting at age 21, get a Pap test every three until you are 65 old. Women 30 of age or older can choose to switch to a combination Pap smear and human papillomavirus (HPV) test every five until the age of 65. If you are older than 65 or have had a hysterectomy, talk with your provider about whether you still need to be screened. Cholesterol screening (lipid panel). If you have a family history of cardiovascular disease, or have any other risk factors for heart diseases such as obesity, you should have a cholesterol screening before the age of 45, and annually after age 45. Screening every five beginning at age 45 is recommended for those without family history or other risk factors. Colon cancer. Between the ages of 50 and 75, get a colonoscopy, which is a screening test for colorectal cancer. If you are between the ages of 76 and 85, talk with your provider about whether you should continue to be screened. Depression. Your emotional health is as important as your physical health. Talk to your health care team about being screened for depression, especially if during the last two weeks: You have felt down, sad or hopeless You have felt little interest or pleasure in doing things Diabetes. Get screened for diabetes (high blood sugar) if you are overweight, have high blood pressure, take medication for high blood pressure, or have a family history of diabetes. Diabetes can cause problems with your heart, brain, eyes, feet, kidneys, nerves and other body parts. Hepatitis C virus (HCV). Get screened one time for HCV infection if: You were born between 1945 and 1965 You have ever injected drugs You received a blood transfusion before 1992 If you currently are an injection drug user, you should be screened regularly. HIV. If you are 65 or younger, get screened for HIV at least once in your lifetime. If you are older than 65, talk to your provider about whether you should be screened. High blood pressure. Have your blood pressure checked yearly. High blood pressure can cause strokes, heart attacks, kidney and eye problems, and heart failure. Blood pressure guidelines: Normal <120/80 Pre-hypertension = /80-89 Hypertension >140/90 Lung cancer. Talk to your doctor or nurse about getting screened for lung cancer if you are between the ages of 55 and 80, have a 30 pack-year smoking history, and smoke now or have quit within the past 15. (Your pack-year history is the number of packs of cigarettes smoked per day times the number of you have smoked.) Know that quitting smoking is the best thing you can do for your health.
5 Overweight and obesity. The best way to learn if you are overweight or obese is to find your body mass index (BMI). You can find your BMI by using the BMI calculator below. A BMI between 18.5 and 25 indicates a normal weight. Persons with a BMI of 30 or higher may be obese. If you are obese, talk to your provider about getting dietary counseling and help with changing your behaviors to lose weight. Being overweight or obese can lead to diabetes and cardiovascular disease. Osteoporosis (bone thinning). Have a screening test at age 65 to make sure your bones are strong. The most common test is a DEXA scan a low-dose X-ray of the spine and hip. If you are younger than 65 and at high risk for bone fractures, you should talk with your provider to see if you should be screened and how to lower your risks for bone fractures. are older than 25, talk to your doctor or nurse about whether you should be screened. Ask your doctor or nurse whether you should be screened for other sexually transmitted infections. Thyroid testing (TSH screen). Thyroid screening is recommended if you are symptomatic. Symptoms may include thyroid nodules or enlarged thyroid, irregular periods, miscarriages, severe nausea during pregnancy, anxiety, depression, or irregular heartbeat. It is also reasonable to screen if symptoms of fatigue, weight gain or hair loss are present. You know your body better than anyone else. Always tell your health care team about any changes in your health, including your vision and hearing. Ask them about being checked for any condition you are concerned about, not just the ones here. Sexually transmitted infections. Sexually transmitted infections can make it hard to get pregnant, may affect your baby and can cause other health problems. Get screened for chlamydia and gonorrhea infections if you are 25 or younger and sexually active. If you BMI (Body Mass Index) Chart for Women (lbs.) Severely Underweight (BMI <17.5) Underweight (BMI ) Optimal Weight (BMI ) Overweight (BMI ) Obese (BMI ) Severely Obese (BMI >40.1)
6 Updated Cervical Cancer Screening Recommendations What is the cervix? The cervix is the opening of the uterus (womb). What causes cervical cancer? The human papillomavirus (HPV) can cause normal cells on your cervix to turn abnormal. Most times, cervical cancer grows slowly (over a 10 to 15 year period). Cervical cancer often does not cause symptoms until it is advanced. Women with advanced cervical cancer may have abnormal bleeding, discharge or pain. Cervical cancer is not hereditary. What should I know about HPV? About 40 types of HPV are passed on through genital or skin-to-skin contact, most often during vaginal or anal sex. It is one of the most common sexually transmitted infections. About 75 percent of sexually active people will get HPV sometime in their life. Since HPV rarely causes symptoms, most people don t know they have the infection. Most of the time, HPV goes away by itself within two and does not cause health problems. It is thought that the immune system fights off HPV naturally. Who should have the HPV test? HPV is very common in women younger than age 30. It is not useful to test young women for HPV, since most HPV found in these women will never cause them health problems. It is currently recommended to test women age 30 to 65 for HPV. The HPV test will not tell you if you have ever had HPV, but only if you currently have HPV. If I have HPV, do I have cervical cancer? No, HPV is not the same as cervical cancer. HPV is the virus that can cause cervical cancer. Many women have HPV; few of them get cervical cancer if they follow their doctor s advice for more testing and/or treatment. Does having HPV or abnormal cervical cells affect my chances of getting pregnant? It does not make it harder to get or stay pregnant, but if you need treatment for your cervical cell changes, the treatment could affect your chances of carrying a baby to term, in rare cases. Will I pass HPV to my current partner? Condoms may lower your chances of passing HPV to your new partner, but HPV can infect areas that are not covered by a condom. If you have been with your partner for a while, your partner likely has HPV too. Your partner likely has no signs or symptoms of HPV. There is no way to know if your partner gave you HPV or if you gave HPV to your partner. Recommendations: Current guidelines recommend that women should have a Pap test every three beginning at age 21. Women ages 30 to 65 should have an HPV and Pap co-testing every five or a Pap test alone every three. The combination of Pap and HPV test is less likely to miss an abnormality Women with certain risk factors may need to have more frequent screening or to continue screening beyond age 65.
7 Recommended Immunizations and Vaccinations 2015 Recommended Immunizations for Adults: By Age If you are this age, talk to your healthcare professional about these vaccines Flu Influenza Td/Tdap Tetanus, diphtheria, pertussis Shingles Zoster Pneumococcal Meningococcal MMR Measles, mumps, rubella HPV Human papillomavirus PCV13 PPSV23 for women for men Chickenpox Varicella Hepatitis A Hepatitis B Hib Haemophilus influenzae type b or of Tdap* 1 or 2 1 or more Flu vaccine every year Td booster every or year 1 or 2 More Information: There are several flu vaccines available. Talk to your healthcare professional about which flu vaccines is right for you. * If you are pregnant, you should get a Tdap vaccine during the 3 rd trimester of every pregnancy to help protect your babies from pertussis (whooping cough). You should get zoster vaccine even if you ve had shingles before. Recommended For You: This vaccine is recommended for you unless your healthcare professional tells you that you cannot safely receive it or that you do not need it. May Be Recommended For You: This vaccine is recommended for you if you have certain risk factors due to your health, job, or lifestyle that are not listed here. Talk to your healthcare professional to see if you need this vaccine. There are two different types of pneumococcal vaccine: PCV13 (conjugate) and PPSV23 (polysaccharide). Talk with your healthcare professional to find out if one or both pneumococcal vaccines are recommended for you. Your healthcare professional will let you know how many you need. If you were born in 1957 or after, and don t have a record of being vaccinated or having had measles, mumps and rubella, talk to your healthcare professional about how many you may need. If you are traveling outside the United States, you may need additional vaccines. Ask your healthcare professional about which vaccines you may need at least 6 weeks prior to your travel. Recommended for you if you did not get it when you were a child. There are two HPV vaccines but only one HPV vaccine (Gardasil ) should be given to men. If you are a male 22 through 26 old and have sex with men you should complete the HPV vaccine series if you have not already done so. Your healthcare professional will let you know how many you need.
