Getting a checkup even when you are not sick is smart. These visits allow the Doctor to keep a close watch on your physical, mental and social health. Adults need a physical exam once a year at least. 2 2 INFORMATION FOR ADULT PATIENTS 2016 Recommended Immunizations for Adults: By Age If you are this age, talk to your healthcare professional about these vaccines Hepatitis A Hepatitis B Hib Haemophilus influenzae type b Chickenpox Varicella HPV Human papillomavirus Pneumococcal Meningococcal MMR Measles, mumps, PCV13 PPSV23 MenACWY or MPSV4 rubella Shingles Zoster Td/Tdap Tetanus, diphtheria, pertussis Flu Influenza MenB for women for men years years years years years 1 or 2 doses 65+ year get this vaccine if you did not get it when you were a child. get 1 dose of PCV13 and at least 1 dose of PPSV23 depending on your age and health condition. get shingles vaccine even if you have had shingles before. get a Td booster every 10 years. You also need 1 dose of Tdap. Women should get a Tdap vaccine during every pregnancy to protect the baby. get flu vaccine every year. More Information: get HPV vaccine if you are a woman through age 26 years or a man through age 21 years and did not already complete the series. 4 Continued from Page 1 Summer a Great Season to Schedule a Checkup Be sure to visit your Doctor for an annual physical exam this summer. If you do not have a regular Doctor, go on to find one near you. What to expect at the Doctor visit During the physical exam, the Doctor will: 1. Check weight, height and blood pressure. 2. Listen to your lungs and heart. 3. Give needed shots and vaccines. 4. Order any needed tests. 5. Offer advice on how to stay healthy. Tips for your Doctor visit Make a list of health topics or concerns you want to talk about with your Doctor. Then put the list in order of importance to you. Bring any medicines you are taking. Bring any test results, health records and dates of visits with specialists. Key screenings and tests: Find out if you have gotten the health screenings or labwork needed, such as colonoscopy, mammogram and cervical cancer testing. Ask if you need key tests and shots (see pages 2-3). Talk with the Doctor about issues you have, such as pain. 4 Don t forget: Call your Doctor today to set up your checkup each year.

6 Frozen Peach-Berry Squares (Makes 9 servings) Having diabetes does not mean you can never eat desserts. Here s a dessert that s low in calories and a smart way to please your taste buds. Ingredients: 8 oz fat-free cream cheese, softened 12 oz peach fat-free yogurt with artificial sweetener 4 oz frozen light whipped topping, thawed 1 cup peeled fresh peaches (chopped or frozen unsweetened peach slices (thawed and chopped) 1 cup fresh or frozen unsweetened blueberries, raspberries, and/or strawberries (thawed & drained if frozen) Fresh mint leaves or berries to garnish Directions: 1. Place cream cheese and yogurt in a bowl. Beat with an electric mixer until smooth. 2. Gently fold in the whipped topping, peaches and 1 cup of berries. 3. Pour the mixture into an 8x8x2-inch (2-quart) square baking dish. Cover and freeze for about 8 hours or until firm. 4. To serve, let the dish stand at room temperature for about 45 minutes to thaw slightly. Cut into 9 even squares. 5. Garnish with mint leaves and more berries if desired.
