2 Purpose Metastatic melanoma (MM) is common in Australia and is associated with poor survival (1). MM management is complex given its variable clinical course, response to radiotherapy and systemic agents. Tumour BRAF mutational status is now a consideration in the management of this disease owing to the availability of agents targeting this intracellular pathway (2). Palliative RT is commonly utilised in patients with MM to treat symptoms from local progression, improve local control, and prevent pathological fracture. The optimal RT utilisation rate for MM however is largely unknown (3). We performed a retrospective analysis evaluating radiotherapy (RT) utilisation rates, patterns of care, and survival in a contemporary cohort undergoing BRAF-mutation testing. Methods and Materials Institutional human research ethics approval was obtained to undertake a retrospective analysis of a prospectively maintained database of patients with MM who underwent BRAF V600E mutation testing between April 2010 and August 2012 at Princess Alexandra Hospital Melanoma Unit. Clinical records were reviewed for patient demographics, BRAF-mutation status, tumour site(s), RT fractionation schedules delivered, and systemic agents used. All time points were taken from the date of diagnosis of metastatic disease or un-resectable recurrence. Median follow up and survival were calculated according to the Kaplan- Meier method. Differences between BRAF-status with categorical variables were tested using chi-squared test, median tests for continuous variables, and Cox proportional hazards models to compare overall survival (OS). Results 158 patients were identified, two of whom had insufficient tissue for testing and 15 had no follow up data. Of the remaining patients (n=141), 69 (49%) tested BRAF-mutant (BRAF-m) and 72 (51%) tested BRAF-wildtype (BRAF-w). Page 2 of 7
3 Patient demographics, stage, and systemic agent use are shown in Table 1. Median age was 47 years (range 21-79) for BRAF-m and 62 years (range 25-84) for BRAF-w. Median follow-up for the cohort was 12 months (range ); 11.2 months (range ) for BRAF-m and 12.7 months (range ) for BRAF-w (P=0.28). Overall RT utilisation was 68% for BRAF-m and 69% for BRAF-w (Table 2). Of those that had RT, BRAF-m had a mean of 1.70 treatments and BRAF-w 2.36 treatments (Pearson chi-squared 3.92, P=0.05). 51% of BRAF-m and 56% of BRAF-w required #2 RT courses (re-treatment and/or separate RT course). 46% of BRAF-m compared to 29% BRAF-w received brain RT (P=0.04). Median OS was 18 months for BRAF-m and 19 months for BRAF-w (P=0.99) (Table 3). OS according to BRAF-status is shown in Figure 1. No statistically significant difference was observed adjusting for BRAF-status on bi-variate Cox proportional hazard rates (Table 4). 7 Conclusion In this modern Australian cohort undergoing BRAF testing in the targeted therapy era, high RT utilisation rates were observed irrespective of mutation status. BRAF-w patients received significantly more RT, however more BRAF-m patients received brain RT.
