Komen is the world s largest breast cancer organization, funding more breast cancer research than any other nonprofit while providing real-time help to those facing the disease. Since its founding in 1982, Komen has funded more than $845 million in research and provided more than $1.8 billion in funding to screening, education, treatment and psychosocial support programs serving millions of people in more than 30 countries worldwide. Connect with us on Facebook at facebook.com/susangkomen and NATIONAL CAPITAL REGION GRANTS The National Capital Region, which includes Washington D.C. and surrounding areas in Maryland and Virginia, has the highest incidence and mortality rates for breast cancer in the United States, with incidence rates 20 percent higher than the national average and mortality rates nearly 40 percent higher than the national average. i In addition, it has one of the highest incidence rates of late-stage breast cancer. ii Komen is working to reduce rates of late stage diagnosis and mortality with an emphasis on eliminating breast cancer disparities. Komen conducted a Community Profile assessment to investigate the health needs and assets in the NCR. The Community Profile assessment includes a comprehensive review of the demographic and breast cancer statistics of the NCR in order to identify target communities. The 2015 Community Profile will not be complete until Summer 2015, but the quantitative data report is completed. Please see Appendix A: Quantitative Data: Measuring Breast Cancer Impact in Local Communities on page 12 for more information. The statistics allow Komen to pinpoint where efforts will have the most impact. The second element of the assessment is an analysis of NCR breast cancer health systems. The health systems analysis focuses on understanding the gaps, needs and barriers that women encounter as they access breast health services. Finally, data is collected from women living in the target communities to gauge breast cancer understanding, knowledge of services, access barriers and the effectiveness of outreach and education efforts. Findings from the 2010 National Capital Region Community Profile revealed two main themes throughout the region: First, education efforts need to do more than raise awareness about breast cancer; they must empower and equip people to take action. Specifically, programs need to provide in-depth education on screening, breast cancer and available resources. In addition, they should be tailored to specific groups, be culturally competent and provide consistent, clear messages that lessen fear and confusion. 2 P age

3 The second theme is a lack of continuity of care throughout the continuum of care for women who are uninsured and underinsured. The continuum of care includes screening, diagnosis, treatment and follow-up care. Throughout the continuum, coordination within and between providers is needed to ensure that all patients receive timely, high-quality and culturallycompetent care and that no patient is lost to follow-up. Thus, Grants will be made in the National Capital Region (NCR) to support high impact breast cancer programs with a strong preference on education, care coordination and reducing fragmentation. Susan G. Komen invites organizations with a focus on breast health services targeting African- American, Hispanic/Latino, medically underserved, and/or low income women in the National Capital Region to respond to this competitive announcement of funding. The target populations for this grant have been selected due to the alarming disparities faced by these groups. For example, although white women are more likely to develop breast cancer than any other racial or ethnic group, iii African American women are twice as likely to die from the disease. Currently, African American women have a 41 percent higher rate of breast cancer mortality (death) than white women. Additionally, African American women under age 40 have a higher incidence of breast cancer than white women. iv African American women are also more likely to be diagnosed with more aggressive tumors than white women. Similarly, Hispanic/Latina women have a lower incidence of breast cancer than white women, but are more likely to be diagnosed with larger tumors and at a later stage than white women. v Priority I - Screening/Reducing Fragmentation Reduce fragmentation and enhance the health care system s capacity to screen and provide followup care in the NCR. Programs will create continuity between referral, screening, diagnosis and treatment within the region. Partnerships and/or collaborations are required and organizations must provide evidence of collaboration. Susan G. Komen supports partnership in which responsibilities, privileges and power are shared. The success of breast health programs also frequently depends upon active community cooperation and collaboration. The organizations involved must demonstrate how they can innovatively address this priority and how they will work together toward system changes. Evidence of a less fragmented system could include decreased time from screening to diagnosis and/or decreased time between diagnoses and treatment. The lead agency applying for Priority I may request up to $300,000 over three years on behalf of the coalition, collaboration or partnership. The project period must be three years. Example: A process or system that increases the percentage of patients who enter, stay-in or progress through the continuum of care. Strong preference will be given to programs that address the lack of care coordination. These programs must implement sustainable practices and policies to reduce fragmentation of care ensuring that patients are successfully navigated from the initial screening referral through survivorship (if required). Priority II - Provider Education Increase provider cultural competency, understanding of breast cancer screening recommendations supported by Susan G. Komen and knowledge of various referral processes to better navigate their patients through the continuum of care. Preference will be given to programs that can demonstrate 3 P age

4 provider behavior change that leads to increased referral rates and screening rates within an organization s patient population. Providers can include navigators, physicians, nurses, office staff, community health workers, and other office staff or volunteers who work with patients and can improve patient care. Partnerships and/or collaborations are required and organizations must provide evidence of collaboration. The lead agency applying for Priority II may request up to $100,000 over two years on behalf of the coalition, collaboration or partnership. The project period must be two years. Komen will only support non-medical expenses for this funding opportunity. Example: Training for office staff and health care providers to streamline and improve the screening referral process. Training for health care providers and office staff on the implementation of tools that support guideline adherence to screening among their patient population (i.e., patient reminder systems, etc.). Priority III - Public Education Implement breast cancer education programs to include tailored messages, understanding of the disease and survivorship as well as ensuring that the program is culturally competent. All education programs must demonstrate how their activities will lead to action, such as participants obtaining regular mammograms. Partnerships and/or collaborations are required and organizations must provide evidence of collaboration. The lead agency applying for Priority III may request up to $200,000 over two years on behalf of the coalition, collaboration or partnership. The project period must be two years. Komen will only support non-medical expenses for this funding opportunity. Example: A culturally-tailored education and outreach project that directly links individuals to breast health services and follow-up. Applicants must secure free or low-cost referral commitments for medical services including clinical breast exams, mammograms and follow up care. Applicants must have established relationships with screening services providers. ELIGIBILITY Applicants must ensure that all past and current Komen-funded grants or awards are up-to-date and in compliance with Komen requirements. Organizations with a current Komen Community Health Grant into 2015 from the National Capital Region and/or Komen National are not eligible to apply for funding. 5 RESPONSILITITIES OF FUNDED PROGRAMS All funded programs will be required to submit semi-annual reports and a final report to include final expenditures. In addition, grantees will be required to submit a one page summary report to include notable lessons learned, challenges, successes and overall accomplishments of the program. This document will be shared with other breast health care programs in the region in order to add to the body of literature on the topic and to benefit the breast cancer community at large. All reports will be submitted electronically via Komen s online Grants emanagement System. FUNDING INFORMAITON An organization may serve as the lead agency (agency that will manage the funds and report on the project) on only one application. Only organizations applying for Priority I may request funds for screening. We accept that a program may receive funding from multiple sources. Consequently, Komen encourages leveraging funds from multiple funders to support program implementation to address demonstrated, immense community needs and gaps. If the Applicant organization is receiving in-kind support, please include a letter from a senior official of the institution providing the support, confirming the type of support and value in dollars. This letter should be uploaded under Letters of Support on the budget summary page in the system. Please note that every funding cycle from the Susan G. Komen, Community Grants Program is a competitive process for ALL applicants, including currently or previously funded organizations. Thus, refunding of previously funded programs is never guaranteed. RESTRICTIONS Projects must be specific to breast health and/or breast cancer (e.g. if a project is a combined breast and cervical cancer project, funding may only be requested for the breast cancer portion). Indirect costs (to include costs the organization would incur regardless of the project such as rent, telephone, internet, etc.), if applicable, should not exceed 10% of total direct costs. Salaries, if requested, are for personnel related to this project only and not general work of applicant. Funds may NOT be used for: o Medical or scientific research (costs for program evaluation are allowed and encouraged) o Scholarships or fellowships o Construction or renovation of facilities o Political campaigns or lobbying o Fund raising events o Professional dues or membership fees o Liability insurance o Endowments 5 P age

6 EDUCATIONAL MATERIALS AND MESSAGES Susan G. Komen is a source of information about breast cancer for people all over the world. To reduce confusion and reinforce learning, we require that grantees provide educational messages and materials that are consistent with those promoted by Komen, including promoting the breast selfawareness messages - know your risk, get screened, know what is normal for you and make healthy lifestyle choices. The consistent and repeated use of the same messages will improve retention and the adoption of the action we think are important. Please visit the following webpage before completing your application and be sure that your organization can agree to promote these messages: We do not recommend monthly breast self-exams and therefore will not fund programs that teach monthly breast self-exams or use breast models. Susan G. Komen grantees are eligible to receive preferred pricing for Komen educational materials. Komen materials should be used and displayed whenever possible. To view our educational materials, visit SELECTION CRITERIA Each grant application will be reviewed by at least three independent reviewers. They will consider each of the following selection criteria: Impact: Will the project have a substantial positive impact on increasing the percentage of people who enter, stay in or progress through the continuum of care? Will the project have a substantial impact on the priority selected? How closely does the project align with the funding priorities stated in the RFA? Does the project have a sufficient and documented plan to evaluate its impact? Feasibility: How likely is it that the objectives and activities will be achieved within the scope of the funded project? Is the project well planned? Is the budget appropriate and realistic? Does the budget justification explain in detail the reasoning and need for the costs associated with the project? Capacity: Does the organization(s), Project Director and his/her team have the expertise to effectively implement all aspects of the project? Are the organization(s) and the project team respected and valued by the target population? Additionally, the organization(s) should be able to demonstrate a sincere commitment to cultural diversity in its staffing, activities and programs. Collaboration: Does this project enhance collaboration among organizations with similar or complementary goals? Are the roles of the partners appropriate, relevant and clearly defined? Are appropriate mechanisms in place to ensure shared power, responsibilities and adequate communications between organizations? Sustainability: Is the project likely to be sustained? Are partnerships likely to be sustained past project period? Is the impact likely to be long-term? Applications will be reviewed by a committee of experts in breast cancer care, grantmaking, nonprofit administration, program management and public health. Below please see details regarding each section of the application that must be completed in the online application: Project Profile This section should include basic organization information such as primary contacts for your program and partners. Organization Summary It should include a brief description of the following: 1) organization s history and if your program is part of a larger organization, an explanation of the mission of the larger entity and your relationship to it; 2) the mission of the organization; 3) the current programs and accomplishments; 4) explanation of how your organization seeks to be diverse and inclusive; and 5) number of paid full time staff, volunteers and total annual organization budget. Project Abstract (limit 3,000 characters) This section documents important information regarding the main focus for your project. This abstract should be detailed and address the reason for the program, important project activities and anticipated outcomes, methods for evaluation, and the impact your program will make on breast cancer in your community. Project Narrative (limit 3,500 characters) The Project Narrative is the core piece of your application. Detailed information is required in this section to convey a thorough understanding of the proposed project. After reading the project narrative an individual should fully understand the components of your project, what your project intends to accomplish and why your organization and its partners are best suited to implement the proposed project. Organization Capacity (limit 3,500 characters) 7 P age

8 Explain why the applicant organization is best-suited to lead the project and accomplish the goals and objectives set forth in this application. Describe evidence of success in delivering breast health/cancer services to the proposed population. Describe your organization s fiscal capability to manage the delivery of the proposed goals and objectives and ensure adequate measures for internal control of grant dollars. Statement of Need (limit 3,500 characters) Describe the population to be served. Describe evidence of the risk/need within that population. Provide statistics specific to the target population. Project Description (limit 3,500 characters) Explain how the project s goals and objectives, as outlined in your Project Work Plan, address one of the priorities listed in the RFA. Explain how your proposed program goals and objectives relate to those of your state s comprehensive cancer control plan. In addition, describe how your proposed program objectives address your state s cancer prevention plan. Explain how your program will ensure that women will return for regular, ongoing rescreening services. Susan G. Komen encourages programs to utilize evidence-based practices to increase the use of breast cancer screening and throughout the continuum of care for women who are uninsured and underinsured. vi Past Komen Grantees (complete only if funded by Komen in the past limit 3,500 characters) If your organization has received a previous Komen grant please provide a brief summary report on your achievements. Include the date(s) of the funding and level of funding. Collaboration (limit 3,500 characters) Explain how you will work with identified partners. Describe the roles and responsibilities of all organizations or entities participating in the project. Explain how the collaboration strengthens the project and why these organizations are best suited to carry out the project and accomplish the goals and objectives set forth in this application. Sustainability (limit 3,500 characters) What resources (financial, personnel, in-kind donations, partnerships, etc.) will be needed to sustain the effort over time? How will those resources be secured by the end of the funded project period? What are your organization s plans to support the project lead to implement, manage and oversee all aspects of the proposed project? 9 Describe the organization s current financial state. Has your organizational budget increased or decreased from last year? Please explain why. Evaluation (limit 3,500 characters) Describe in detail how the organization(s) will measure the achievement of project goals and objectives and how the impact of the project on the priority selected will be assessed. Describe the evaluation expertise that will be available for this purpose. What resources are allocated for evaluation in the project budget? A strong evaluation plan measures both the quantity and quality of strategy implementation and outcomes. Impact Evaluation: Assesses the changes that can be attributed to a particular intervention, such as a project, program or policy. Impact Evaluation helps us to answer key questions such as: what works, what doesn t, where, why and for how much? Process Evaluation: Assesses the delivery of programs. Process evaluation verifies what the program is and whether it is being implemented as designed. It answers the questions of what is delivered in reality and where the gaps are between program design and delivery. Project Target Demographics This section should include information regarding the various demographic groups you intend to target with your program. This does not include every demographic group your program will serve but should be based on the individuals you plan to focus your program's attention on. Key Personnel This section should include information regarding the personnel that will be needed to complete the project. Any individual playing a key role in the project should be included in this section. Please note: This is also where key staff resumes, curriculum vitae and job descriptions for new or vacant positions will be uploaded into the system. Project Work Plan This section should include the main goals and objectives for the entire project. Goals are high-level statements that provide overall context for what the project is trying to achieve. Objectives are lowlevel concrete statements that describe what the project is trying to achieve. Objectives should be written as SMART objectives. Specific To what you want to achieve. Measurable How much do you want to achieve? Achievable - Are the objectives realistic? Results-oriented Measures outcomes, not activities. Timely By when do you want to achieve the objectives? 9 P age

10 For each goal and objective, please include: Timeline for completion Intended intervention to be accomplished Individual at your organization responsible Number of individuals you intend to serve through the intervention A description of methods and techniques you will use to evaluate effectiveness Budget Provide a detailed total program budget. All funding for this program, including other sources of funding should be included. For each line item in the budget, provide a brief description of how the funds will be used and why they are programmatically necessary. Attachments Evaluation forms, surveys, logic model, etc. - related to demonstrating the effectiveness of your program as defined in your project work plan. Information regarding Key Personnel Provide a resume or curriculum vitae for key personnel that are currently employed by the applicant. Provide job descriptions for new or vacant positions. (Two page limit per individual). Letters of support, collaboration, MOU, etc. Proof of Non-Profit Status Attach your determination letter from the Internal Revenue Service to document you federal tax-exempt status. 12 APPENDIX A: Quantitative Data: Measuring Breast Cancer Impact in Local Communities Quantitative Data Report Introduction The purpose of the quantitative data report for the National Capital Region of Susan G. Komen is to combine evidence from many credible sources and use the data to identify the highest priority areas for evidence-based breast cancer programs. The data provided in the report are used to identify priorities within the National Capital Region s service area based on estimates of how long it would take an area to achieve Healthy People 2020 objectives for breast cancer late-stage diagnosis and mortality ( The following is a summary of the Komen National Capital Region s Quantitative Data Report. Breast Cancer Statistics Incidence rates The breast cancer incidence rate shows the frequency of new cases of breast cancer among women living in an area during a certain time period (Tables 1 and 2). Incidence rates may be calculated for all women or for specific groups of women (e.g. for Asian/Pacific Islander women living in the area). The female breast cancer incidence rate is calculated as the number of females in an area who were diagnosed with breast cancer divided by the total number of females living in that area. Incidence rates are usually expressed in terms of 100,000 people. For example, suppose there are 50,000 females living in an area and 60 of them are diagnosed with breast cancer during a certain time period. Sixty out of 50,000 is the same as 120 out of 100,000. So the female breast cancer incidence rate would be reported as 120 per 100,000 for that time period. When comparing breast cancer rates for an area where many older people live to rates for an area where younger people live, it s hard to know whether the differences are due to age or whether other factors might also be involved. To account for age, breast cancer rates are usually adjusted to a common standard age distribution. Using age-adjusted rates makes it possible to spot differences in breast cancer rates caused by factors other than differences in age between groups of women. To show trends (changes over time) in cancer incidence, data for the annual percent change in the incidence rate over a five-year period were included in the report. The annual percent change is the average year-to-year change of the incidence rate. It may be either a positive or negative number. A negative value means that the rates are getting lower. A positive value means that the rates are getting higher. 12 P age

13 A positive value (rates getting higher) may seem undesirable and it generally is. However, it s important to remember that an increase in breast cancer incidence could also mean that more breast cancers are being found because more women are getting mammograms. So higher rates don t necessarily mean that there has been an increase in the occurrence of breast cancer. Death rates The breast cancer death rate shows the frequency of death from breast cancer among women living in a given area during a certain time period (Tables 1 and 2). Like incidence rates, death rates may be calculated for all women or for specific groups of women (e.g. Black women). The death rate is calculated as the number of women from a particular geographic area who died from breast cancer divided by the total number of women living in that area. Death rates are shown in terms of 100,000 women and adjusted for age. Data are included for the annual percent change in the death rate over a five-year period. The meanings of these data are the same as for incidence rates, with one exception. Changes in screening don t affect death rates in the way that they affect incidence rates. So a negative value, which means that death rates are getting lower, is always desirable. A positive value, which means that death rates are getting higher, is always undesirable. Late-stage diagnosis For this report, late-stage breast cancer is defined as regional or distant stage using the Surveillance, Epidemiology and End Results (SEER) Summary Stage definitions ( State and national reporting usually uses the SEER Summary Stage. It provides a consistent set of definitions of stages for historical comparisons. The late-stage breast cancer incidence rate is calculated as the number of women with regional or distant breast cancer in a particular geographic area divided by the number of women living in that area (Table 1). Late-stage incidence rates are shown in terms of 100,000 women and adjusted for age. 13 P age

18 Mammography Screening Getting regular screening mammograms (and treatment if diagnosed) lowers the risk of dying from breast cancer. Screening mammography can find breast cancer early, when the chances of survival are highest. Table 3 shows some screening recommendations among major organizations for women at average risk. This information can be used to identify groups of women who should be screened who need help in meeting the current recommendations for screening mammography. The Centers for Disease Control and Prevention s (CDC) Behavioral Risk Factors Surveillance System (BRFSS) collected the data on mammograms that are used in this report. The data come from interviews with women age 50 to 74 from across the United States. During the interviews, each woman was asked how long it has been since she has had a mammogram. BRFSS is the best and most widely used source available for information on mammography usage among women in the United States, although it does not collect data matching Komen screening recommendations (i.e. from women age 40 and older). The proportions in Table 4 are based on the number of women age 50 to 74 who reported in 2012 having had a mammogram in the last two years. The data have been weighted to account for differences between the women who were interviewed and all the women in the area. For example, if 20.0 percent of the women interviewed are Latina, but only 10.0 percent of the total women in the area are Latina, weighting is used to account for this difference. The report uses the mammography screening proportion to show whether the women in an area are getting screening mammograms when they should. Mammography screening proportion is calculated from two pieces of information: 18 P age

19 The number of women living in an area whom the BRFSS determines should have mammograms (i.e. women age 50 to 74). The number of these women who actually had a mammogram during the past two years. The number of women who had a mammogram is divided by the number who should have had one. For example, if there are 500 women in an area who should have had mammograms and 250 of those women actually had a mammogram in the past two years, the mammography screening proportion is 50.0 percent. Because the screening proportions come from samples of women in an area and are not exact, Table 3 includes confidence intervals. A confidence interval is a range of values that gives an idea of how uncertain a value may be. It s shown as two numbers a lower value and a higher one. It is very unlikely that the true rate is less than the lower value or more than the higher value. For example, if screening proportion was reported as 50.0 percent, with a confidence interval of 35.0 to 65.0 percent, the real rate might not be exactly 50.0 percent, but it s very unlikely that it s less than 35.0 or more than 65.0 percent. In general, screening proportions at the county level have fairly wide confidence intervals. The confidence interval should always be considered before concluding that the screening proportion in one county is higher or lower than that in another county. Table 4. The screening proportion of the NCR service area was not significantly different than the District of Columbia, was not significantly different than the State of Maryland and was significantly higher than the State of Virginia. For the United States, breast cancer screening proportions among Blacks are similar to those among Whites overall. APIs have somewhat lower screening proportions than Whites and Blacks. Although data are limited, screening proportions among AIANs are similar to those among Whites. Screening proportions among Hispanics/Latinas are similar to those among Non-Hispanic Whites and Blacks. 21 Population Characteristics The report includes basic information about the women in each area (demographic measures) and about factors like education, income, and unemployment (socioeconomic measures) in the areas where they live (Tables 5, 6, 7 and 8). Demographic and socioeconomic data can be used to identify which groups of women are most in need of help and to figure out the best ways to help them. It is important to note that the report uses the race and ethnicity categories used by the US Census Bureau, and that race and ethnicity are separate and independent categories. This means that everyone is classified as both a member of one of the four race groups as well as either Hispanic/Latina or Non-Hispanic/Latina. The demographic and socioeconomic data in this report are the most recent data available for US counties. All the data are shown as percentages. However, the percentages weren t all calculated in the same way. The race and ethnicity data are based on the total population in the area. The age data are based on the total female population in the area (e.g. the percent of females over the age of 40). The socioeconomic data are based on all the people in the area, not just women. Income, education and unemployment data don t include children. They re based on people age 15 and older for income and unemployment and age 25 and older for education. The data on the use of English, called linguistic isolation, are based on the total number of households in the area. The Census Bureau defines a linguistically isolated household as one in which all the adults have difficulty with English. 21 P age

28 Priority Areas Healthy People 2020 forecasts Healthy People 2020 (HP2020) is a major federal government initiative that provides specific health objectives for communities and for the country as a whole. Many national health organizations use HP2020 targets to monitor progress in reducing the burden of disease and improve the health of the nation. Likewise, Komen believes it is important to refer to HP2020 to see how areas across the country are progressing towards reducing the burden of breast cancer. HP2020 has several cancer-related objectives, including: Reducing women s death rate from breast cancer (Target: 20.6 per 100,000 women). Reducing the number of breast cancers that are found at a late-stage (Target: 41.0 cases per 100,000 women). To see how well counties in the Komen National Capital Region service area are progressing toward these targets, the report uses the following information: County breast cancer death rate and late-stage diagnosis data for years 2006 to Estimates for the trend (annual percent change) in county breast cancer death rates and late-stage diagnoses for years 2006 to Both the data and the HP2020 target are age-adjusted. These data are used to estimate how many years it will take for each county to meet the HP2020 objectives. Because the target date for meeting the objective is 2020, and 2008 (the middle of the period) was used as a starting point, a county has 12 years to meet the target. Death rate and late-stage diagnosis data and trends are used to calculate whether an area will meet the HP2020 target, assuming that the trend seen in years 2006 to 2010 continues for 2011 and beyond. Identification of priority areas The purpose of this report is to combine evidence from many credible sources and use the data to identify the highest priority areas for breast cancer programs (i.e. the areas of greatest need). Classification of priority areas are based on the time needed to achieve HP2020 targets in each area. These time projections depend on both the starting point and the trends in death rates and late-stage incidence. Late-stage incidence reflects both the overall breast cancer incidence rate in the population and the mammography screening coverage. The breast cancer death rate reflects the access to care and the quality of care in the health care delivery area, as well as cancer stage at diagnosis. 28 P age

29 There has not been any indication that either one of the two HP2020 targets is more important than the other. Therefore, the report considers them equally important. Counties are classified as follows (Table 9): Counties that are not likely to achieve either of the HP2020 targets are considered to have the highest needs. Counties that have already achieved both targets are considered to have the lowest needs. Other counties are classified based on the number of years needed to achieve the two targets. Table 9. Service Area Healthy People 2020 Forecasts and Priority Areas The results presented in Table 10 help identify which counties have the greatest needs when it comes to meeting the HP2020 breast cancer targets. For counties in the 13 years or longer category, current trends would need to change to achieve the target. Some counties may currently meet the target but their rates are increasing and they could fail to meet the target if the trend is not reversed. Trends can change for a number of reasons, including: Improved screening programs could lead to breast cancers being diagnosed earlier, resulting in a decrease in both late-stage incidence rates and death rates. Improved socioeconomic conditions, such as reductions in poverty and linguistic isolation could lead to more timely treatment of breast cancer, causing a decrease in death rates. 29 P age

32 Data Limitations The following data limitations need to be considered when utilizing the data of the Quantitative Data Report: The most recent data available were used but, for cancer incidence and mortality, these data are still several years behind. For some areas, data might not be available or might be of varying quality. Areas with small populations might not have enough breast cancer cases or breast cancer deaths each year to support the generation of reliable statistics. There are often several sources of cancer statistics for a given population and geographic area; therefore, other sources of cancer data may result in minor differences in the values even in the same time period. Data on cancer rates for specific racial and ethnic subgroups such as Somali, Hmong, or Ethiopian are not generally available. The various types of breast cancer data in this report are inter-dependent. There are many factors that impact breast cancer risk and survival for which quantitative data are not available. Some examples include family history, genetic markers like HER2 and BRCA, other medical conditions that can complicate treatment, and the level of family and community support available to the patient. The calculation of the years needed to meet the HP2020 objectives assume that the current trends will continue until However, the trends can change for a number of reasons. Not all breast cancer cases have a stage indication. Quantitative Data Report Conclusions Highest priority areas Two areas in the Komen National Capital Region service area are in the highest priority category. Alexandria City, VA is not likely to meet either the death rate or late-stage incidence rate HP2020 targets. Falls Church City, VA is not likely to meet the late-stage incidence rate HP2020 target. Late-stage incidence trends in Falls Church City, VA (48.7 percent per year) are significantly less favorable than the NCR service area as a whole (-0.9 percent per year). 32 P age

33 High priority areas One area in the Komen National Capital Region service area is in the high priority category. The District of Columbia is not likely to meet the death rate HP2020 target. The incidence rates in the District of Columbia (139.7 per 100,000) are significantly higher than the NCR service area as a whole (126.0 per 100,000). The death rates in the District of Columbia (29.8 per 100,000) are significantly higher than the NCR service area as a whole (23.5 per 100,000). The late-stage incidence rates in the District of Columbia (58.0 per 100,000) are significantly higher than the NCR service area as a whole (45.2 per 100,000). The District of Columbia has a relatively large Black population, high poverty rates and high unemployment. Ward-level data show that death rates are highest in the District of Columbia in Ward 2 (35.7 per 100,000), Ward 5 (33.0 per 100,000), Ward 8 (30.9 per 100,000) and Ward 7 (30.1 per 100,000). In the District of Columbia the death rates for Blacks (33.1 per 100,000) are higher than that for Whites (24.7 per 100,000). 33 P age
