Circle of Caring SUSAN G. KOMEN ORANGE COUNTY S GIVING CIRCLE

Circle of Caring is Susan G. Komen Orange County s Giving Circle formed by caring and committed individuals coming together and contributing their philanthropic dollars to specific areas of need within the programs and services of Komen Orange County. Circle of Caring members enjoy additional benefits to learn from experts and attend lectures by leaders in the areas of research and treatment advances, breast health, and the needs of uninsured and underinsured Orange County women.

Our Goal is to create a collective stream of philanthropic giving from the Orange County community so we, the supporters of Susan G. Komen Orange County, can ensure that every local woman (and man) who needs help with her breast health will get that help. We will reach this goal by engaging other women and men who value saving lives now and energizing research to find the cures to breast cancer through the power of our shared passion, our voices, and our collective philanthropy.

Circle of Caring Philanthropic Annual Giving Annual gifts of $1,500 and more, including gifts of publicly traded securities, real property and bequests are accepted. Donations accepted from donor advised funds, family foundations, United Way s Tocqueville Society or Women s Philanthropy Fund. Donations may be arranged as a recurring monthly credit card charge.

BENEFITS PINK CIRCLE OF CARING $10,000 + GOLD CIRCLE OF CARING $5,000 SILVER CIRCLE OF CARING $2,500 MAMMOGRAM-A- MONTH CLUB $1,500** Opportunity to designate your annual gift to specific programmatic initiatives and/or research Exclusive invitations with subject experts in breast cancer research, imaging and treatment Complementary seating at the Komen Grant Awards Breakfast TABLE OF 10 6 TICKETS 2 TICKETS 2 TICKETS VIP Status at the Orange County Race for the Cure including reserved parking and breakfast Invitation-only research update Bouquet of pink long stem roses to the special individual of your choice ** May be set-up as a continuing $125 per month credit card charge.

Choose Where You Want to Put Your Support to Work Donors of $5,000 and above may designate their gift locally and/or to research. Donors of $2,500 and above may designate their gift to specific areas of local impact. Mammograms for the Rarely or Never Screened and Uninsured Every $125 donated can provide a mammogram for an uninsured woman. That first mammogram can lead to a life-long change in breast health care behavior enrolling with a community clinic, returning for annual mammograms, being aware of changes in one s breasts all resulting in earlier diagnosis of breast cancer and lives saved. Using mobile mammography, Komen brings mammography to community sites where women in need work, shop and pray making it easy and non-threatening with no hassles to have that all important first mammogram. Breast Health Care Funding This is a safety-net-of-last-resort for the sole purpose of funding medically necessary breast cancer diagnostic procedures for uninsured low income women (and men) who are symptomatic or at high risk for breast cancer and have no other means of paying for the procedure. Education & Outreach Komen reaches out into the many communities that make up Orange County, in all the gateway languages including Mandarin, Tongan, Vietnamese, Farsi, Hindi, Spanish, Japanese and others, to educate and engage rarely or never screened women particularly those at high risk of late stage diagnosis, in best breast health practices. The goal is to help a woman have her first mammogram, and then continually rescreen over her lifetime. Multi-generational approaches often are effective having the young woman in the family encourage their mother and grandmother to begin attending to their breast health. Special Impact Initiatives We aim to level the playing field for medically underserved populations and address the disparities right here in Orange County. These special initiatives are part of our effort to bring health equity for all. African American Initiative Circle of Promise, a California initiative aimed at breast cancer disparities among African American women, is an intensive grassroots effort to identify evidence-based strategies to reverse trends of high mortality rates because breast cancer is a leading cause of cancer death for African American women. Asian Community Outreach Asian and Pacific Islander communities are one of Susan G. Komen s four priority populations with Orange County housing the third highest population in the United States. By funding six non-profit health organizations Komen is working towards achieving breast health equity within this community. Latina Initiative Latinas are more likely to be diagnosed with breast cancer at a late stage, placing them at increased risk of mortality due to breast cancer. To improve survival rates, we bring mobile mammography to where Orange County Latino women live, work, shop, and pray to motivate behavioral change and demonstrate that Latinas can and do survive breast cancer when diagnosed and treated early.

The Orange County Executive Director will facilitate a discussion between the donor and Komen research grant decision-makers to discuss areas of interest and most promising research projects. Some opportunities donors may consider include: Career Catalyst Research Grants (CCR) CCR Grants provide unique opportunities for scientists who have held faculty positions for no more than five years at the time of Application to achieve research independence. CCR grants provide support for hypothesis-driven research projects that have significant potential to advance our understanding of breast cancer, lead to reductions in breast cancer incidence and/or mortality, and move us toward the goal of a world without breast cancer. Basic and Translational Research For research projects that are laboratoryor field- based and and studies focusing on quality of life or survivorship issues are not appropriate for this mechanism. Clinical Research Provides support for junior faculty for patient-oriented research projects that involve direct interactions with human subjects/patients. This includes studies such as therapeutic interventions or clinical trials. Postdoctoral Fellowship Grants (PDF) PDF Grants are intended to attract and preserve today s young scientists and clinicians who will fuel the future and become the next generation of leaders in the field of breast cancer research. These leaders will play key roles in reducing breast cancer incidence and mortality within the next decade, with the ultimate goal of ending breast cancer forever. Basic and Translational Research Appropriate for both basic (i.e. PhD) or clinical (i.e. MD) fellows proposing research projects that are laboratory- or fieldbased. Clinical Research Appropriate for clinical (i.e. MD) fellows proposing clinical, patient-oriented research projects that involve direct interactions with human subjects/patients. Includes studies such as therapeutic interventions or clinical trials. Graduate Training in Disparities Research (GTDR) GTDR Grants are intended to establish and/or sustain a training program for a minimum of three graduate students who are seeking careers dedicated to understanding and eliminating disparities in breast cancer outcomes across populations groups. Komen provides funding to outstanding and exceptionally creative training programs to ensure that a diverse pool of highly trained scientists will emerge as the next generation of leaders in the field of breast cancer research focused on disparities in breast cancer outcomes.
