1 New Underwriting Guidelines Below is a general outline for Blue Cross and Blue Shield of Texas underwriting guidelines for Individual Health Products. A Acid Reflux Acne Addisons s Disease Adrenal Insufficiency AIDS Alcoholism/Alcohol Abuse (within the last 10 yrs) Allergies ALS (Amyotrophic Lateral Sclerosis) Alzheimer s Disease Amputation Anal Fissure Age 12 and Under Usually decline. Over Age 12 Accept depending on diet and maintenance medication use. Decline if not well controlled or if surgery or further testing is recommended. Accept if treated with medication(s) including Accutane or Retin-A. Accept if well controlled by maintenance medication use. Decline for oral steroid use. Rider Accept if recovered for at least one year, from single episode, without surgical intervention. If present, multiple episodes or operated Angina Pectoris Angioplasty Ankylosis Aortic Regurgitation/Insufficiency/ Coarctation Aortic Stenosis Appendicitis Arteriosclerotic heart Disease Arthritis Ascities (within the last five years) Asthma Accept if single episode and recovered or multiple episodes and recovered for at least six months Osteoarthritis Accept if asymptomatic, or minimal to mild symptoms with occasional over the counter medications or NSAID s. Rheumatoid Accept if minimal or mild, diagnosed at least three years ago, and applicant was diagnosed at age 40 or over. Decline either type if 1) taking Methotrexate or any other immunosuppressives or steroids, or 2) chronic/moderate/severe condition, or 3) any history of surgery. Accept if well controlled by maintenance medication, or is exercised induced and controlled with occasional medication. Decline if diagnosed within one year, hospitalized within two years, ER within two years, combination of smoking and oral steroid use, and all others. 46

Accept if diagnosed at least six months ago, well controlled on one medication only, behavior modification through school, no other counseling needed or recommended, and no other psychiatric diagnosis. Single episode Accept if the applicant has fully recovered with no residuals. Recurrent or chronic Accept if the most recent episode occurred at least four years ago, and the applicant has fully recovered with no residuals. Accept if operated and completely recovered with no recurrence for at least five years., unoperated, operated and fully recovered, no history of multiple surgeries, and no internal fixations in the back or spine. and unoperated Accept if 1) operated at least 12 months ago and fully recovered or 2) the unoperated spuris on a non-weight bearing joint, is asymptomatic, or was found incidentally.. Rider if placed for cosmetic reasons only, no complications and no replacement Accept if single or infrequent attacks, nondisabling, complete recovery between attacks, no significant respiratory impairment Accept if operated and completely recovered with no residuals. Rider if surgery is planned or recommended, and all others. Accept if history of single attack and recovered or multiple attacks and recovered for at least two years for at least five years Accept adults if congenital or traumatic and operated and fully recovered or no treatment needed Rider if unoperated, bilateral or operated without intraocular lens implant Decline if due to intraocular disease, or diabetes or other systemic disease 47

Accept if over age 40 and well controlled. Individual consideration is given to applicants who are on maintenance medications or less than 40 years old. Decline all types if present, or any use of oral steroids or maintenance medications within five years Children Subject to case specific review. Adults Accept depending on frequency and treatment. Decline if ongoing treatment with antibiotics or history of multiple dilations. Accept if no treatment needed, controlled with over the counter medication or operated and fully recovered for at least 12 months Type I (insulin dependent) Decline Type II (non-insulin dependent) Accept if well controlled by diet only (no insulin or medication) for at least six months, applicant has no related or CV conditions, and is over age 30. Diverticulitis, Diverticulosis, Diverticulum Drug Addiction/Abuse Dupuytren s Contracture Colon Accept if asymptomatic or a single episode. Case specific review for multiple episodes or a history of surgery. Esophagus Accept if operated more than six months ago and fully recovered. Decline if present or if operated within six months. Accept if recovered at minimum five to 15 years depending on the type of drug use. Decline any current use. 48
4 E Endometriosis Epididymitis Epilepsy Eye Prosthesis F Accept if 1) completely recovered from a hysterectomy with both ovaries removed, and no residuals, or 2) if unoperated, asymptomatic, and controlled by contraceptive use for at least one year. Decline with a history of multiple surgeries or if symptomatic within one year. Case specific review for all others. Accept if single episode and fully recovered Rider if chronic or recurrent Decline if cause unknown Accept 1) petit mal, and 2) other seizure disorders if more than five years since last seizure.. Additional rating or decline if due to disease Fatty Liver (unless fully recovered with normal lab results for a minimum of six months) Fibroid Tumor (Uterine) G Gastroesophageal Reflux Glaucoma Goiter Gout Gymecomastia H Headache Heart Attack Height and Weight (Please refer to the chart) Hemodialysis/Peritoneal Dialysis Hemophilia Hemorrhoids Hernia, Abdominal Herpes (Type I and II) HIV Accept if over age 45 and asymptomatic or hysterectomy Rider most others Children usually declined Accept if over age 13 if operated and fully recovered for two years or unoperated and controlled for at least six months on one medication or diet Decline if not controlled, surgery or testing recommended or multiple surgeries or history of Please refer to Thyroid Gland Disorders. Accept if diagnosed after age 30, well controlled, and no history of high blood pressure. Accept if history of and complete recovery Accept if occasional or mild. Treatment for infertility is not a benefit. If an individual is ineligible for coverage due to infertility treatment, coverage on the spouse is also declined. Accept if treatment was completed at least three years ago with no complications and no plans for further treatment. including future treatment anticipated or planned. Sprained or strained ligament Accept if fully recovered and no residuals. Accept if recovered for five years and thickness is no greater than 1.5 mm Accept if recovered or controlled for at least three years and an adequate work up has been done to exclude more serious disease Accept if six or fewer episodes per year and no narcotics or triptans use Rate and/or Rider if 10 or fewer episodes per year and controlled by only one medication. Decline if more than one medication or not controlled, or more than 10 episodes. Accept if grade I-II murmur, no heart enlargement, no other CV impairment (MVP not included), trace or mild only. Accept if 1) no murmur and no current medication other than use of prophylactic antibiotics, or 2) click and/or murmur no greater than Grade I-II, or 3) if controlled by medication other than prophylactic antibiotics, but no symptoms for at least six months. Decline if currently symptomatic and all others. Accept if mild and asymptomatic regardless of applicant s age. Declination may result depending on any related conditions or fracture history Accept if a single episode or tubes were inserted. Decline if tubes were recommended or suggested but not yet inserted. Accept if spontaneous disappearance more than six months ago or operated with full recovery for at least six months Rider if benign (malignancy ruled out) 51

Accept if no follow up colonoscopy scheduled within two years Decline if present and all others Accept if completely recovered. Postpone if not completely recovered. Pregnant or an Expectant Parent (mother or father) May apply eight weeks following the end of current pregnancy. Case specific review for all other shoulder disorders. Accept unless surgery is contemplated. Accept if excised, depending on the depth and diameter of the tumor, and the date of full recovery. Decline 1) if present or with metastasis, or 2) if the excised tumor extended to deep tissue, muscle, bone, cartilage, or other deep structures. If melanoma, refer to Melanoma Accept if asymptomatic and incidental finding, conservative treatment or operated and fully recovered for five years Rider others Accept if asymptomatic and incidental finding, conservative treatment or operated and fully recovered for five years Rider others Accept if symptoms are minor, treatment is conservative and surgery has not been recommended Accept if history of and no residuals Accept 1) if completely recovered or asymptomatic for at least five yrs or 2) applicant has minimal to mild symptoms presently controlled by conservative methods Decline if more than mildly symptomatic, or surgery is contemplated. Rider if diagnosed within two years. Toxic (Hyperthyroidism) Decline if within six months of surgery or completion of treatment with anti-thyroid medication or radioactive iodine. Rider others Accept if 1) operated and fully recovered, or 2) unoperated, infrequent episodes, and no discussion of surgery. Decline if chronic, surgery discussed or recommended, and all others. 53
